EXCLUSIVE: Suri Cruise Birth Certificate

TMZ has obtained a copy of the official birth certificate of the elusive Suri Cruise, daughter of Tom Cruise and Katie Holmes.

The document, filed with the Los Angeles County Register-Recorder/County Clerk, was filed May 8, 2006.  (Click on document to download) There are several interesting items of note:

First, St. John's Hospital has a policy of filing birth certificates within 10 days of birth.  In this case, Suri was born on April 18 but the certificate was not filed until May 8, 20 days later.  An official from the hospital told TMZ that the delay occurred because they needed a signature from the parents or a representative of the parents certifying birth, and that person did not come in until May 4.  The person who signed is labeled "friend."  The signature is not legible.

Also, the "Attendant or Certifier" who signed the certificate -- Anne Heffernan, RNC -- was not in the delivery room and did not see the baby.  But a hospital rep tells TMZ that Heffernan is authorized to sign when the doctor is not available.  A hospital rep said "normally the doctor signs" but it isn't a requirement.

Finally, the hospital rep said that the circumstances that triggered the eventual signing of the birth certificate were that Suri needed a passport and a birth certificate is a prerequisite to obtaining one.

91. Give me a break people! If something did happen to that baby, that is a tragedy, extremely personal, and not gossip. Otherwise, it still equates to what should be a private life. Leave it alone, already. Worry about yourselves.

92. i thnk hollywood people (most of them...there are exceptions)
have no moral standards and are in and out of so called love every other week...its a disgrace !!!..and i'm sure tom and Katie will not be any different..prove me wrong i'll be delited.
the problem is all that lip locking and sex play in their movies...they get the hots for their leading lady or leading man and BAM..Marriage is over to their signifacant other.

oh and whats their answer...but honey its my job..it doesn't mean a thing....riiiiight ! those few actors and actresses who don't cheat on their partners are to be commented highly and God Bless them...they are my heroes.

93. They wanted a lot of money for the baby's first pictures and the magazines wouldn't pay. Maybe they are holding out for the money. Maybe they are trying to torment the magazines by keeping the baby out of the public eye.

Maybe the papparzi is trying to smoke them out by creating questions about the baby. Maybe Tom & Katie are creating the mystery. At any rate, this article seems like they media is trying to make something out of nothing.

94. disgusting!!! Our soldiers are raping Iraqi Women, Burning the bodies and murd their families which included a 5 year old little girl. and you guys are concerned with a poor babies existance. Just because of his/her parents famous existance. Babies in other contries are starving and dying of diseases. Being a regular mom i would protect my child from creeps like this. how do you think they feel. would you really want to have you child in the spotlight 24/7. get a life and clue into the REAL WORLD!

95. hey...with all the hoopla and hype leading up to the birth, isn't it odd that it went dead quiet since? OK...so the public got quite sick of T & K but like Brad and Angelina (also sick of...along with Britney, Jen and Ashlee) why don't we see pics of the spawn???????????????????? Now.... they want privacy..... should have thought about that before went PR crazy 9 months ago! Stars invite the fame and fortune.... and that has a price...no privacy. OK...some are better at living privately than others - aka George Clooney, Jodie Foster... but then they don't have a publicist that they tell to go to the media with every detail of their lives.

97. None of these findings are either abnormal or unheard of. They are things that happen every day in hospitals. And as far as what Mimi said about his legal last name being Mathoper, this America. If the parents are named joe and Marge Smith, they have every right to name their child (even if it a daughter) Bob Jones. People give their children family names as a tradition, not a law. DUH!

98. You can put ANY name you want on a birth certificate. It doesn't have to be the legal name (sirname) of one of the parents. Since he goes by 'Cruise' onscreen, he can put 'Cruise' as his children's last name on a birth certificate if he wants to. He may not necessarily 'parade' his child around anywhere either. MANY celbs are keeping their kids out of the paparazzi's OVERWHELMING GLARING lens. Some jobs should be outlawed. EVERYONE has a RIGHT to privacy. Just because someone is fortunate enough to become a celebrity - doesn't mean the rest of the world has a RIGHT to peer in their windows and leave them NO privacy. have some common sense and common decency. What type of beings are you? Human can hardly be used to describe.

The comment about not choosing the spotlight was about the children. They did not choose the spotlight but were rather born/adopted into it. They are hounded very much against their will and they don't have the same rights as their parents. They don't know how to "end" a situation the way an adult does. This is not something they ever asked for.

100. To all that are screaming that it is not our business what the celebs do, and we should leave them alone, Didn't you for some reason click on that story to check it out? Personally I did out of morbid curiosity, just to see what kind of bull they are saying now, but I still checked it out. I agree that the media carries it to far, but we still read it, and talk about it. If you want to promote celeb privacy, start a web page. Don't read a story and than ridicule others for expressing their oppinions about it.

101. Why do people who do not care about these things post here. While the world in general and soldiers dying is of big concern to me on a daily basis, I come here to escape my worries. Not have them thrown in my face. If you do not care about such things or feel that they are not important in your world do not come here and ruin for others looking for a little escape. Go to some anti war or political sites and post there. What do you think you are going to find on an entertainment website? Anti Bush sentiment, political commentary, financial news, peace talks,no you are looking in the wrong place. Just leave us be!

104. First, his "Legal" name is Mapother, not Mathoper.

Secondly, in some states you can give your child any surname you please, it does not have to be the father's surname - legal or otherwise.

Third, I think Tom is doing the right thing (for once) by keeping suri out of the spotlight.

Lastly - TOM IS CRAZY!!! and not in a good way...
