Anna Nicole's Death Certificate

TMZ has obtained the official certificate of death for Anna Nicole Smith, filed earlier today in Broward County, Florida.

On the document, Howard K. Stern is named the "Informant," and describes his relationship to Anna as "Nominated Executor." What's interesting is that he did not list himself as Anna's husband, boyfriend or even lawyer. An official from the Broward County Office of Vital Statistics told TMZ, "We would have not given him that title without his request."

The certificate also states that Anna's level of education is "8th or less."

The former Playboy Playmate and TRIMSPA spokesperson was discovered unconscious in her hotel room at the Seminole Hard Rock Hotel and Casino in Hollywood, Florida on February 8. Paramedics rushed her to Memorial Regional Hospital where she was pronounced dead shortly after arrival. She was 39.

18. I noticed on the certificate it stated she died in the Emergency Room/Outpatient...I thought that one of the Casino's witnesses said that she was already covered when she arrived at the hospital? wouldnt that mean that she was DOA??

20. How can a Death Certificate be filed without a cause of death listed? Does Florida allow that?

It even has a notation at the bottom that says "Void if Altered". If it's been filed, does that mean there will never be a cause of death listed?

22. I work at a hospital and the Death Cert always has has the COD. Is it different in Florida?

24. a death cert can be filed even with a cause of death still under investigation. It would be cited at deferred and a 2nd cert issued once cause of death is determined.

27. I BELIEVE THIS IS WHAT THEY WERE TALKING ABOUT WHEN THEY SAID TO RELEASE A BODY THEY NEED CERTIFACATE OF DEATH SO THEY CAN ORDER A TEMPORARY ONE..... ALSO DOES ANYONE AGREE THAT HOWARD SHOWS THAT HE IS THE INFORMANT AND EXECUTOR AND HE LISTS HIS MAILING ADDRESS AND ONE OF CALIFORNIA... NOT BAHAMA'S SO MAYBE HE DOSENT HAVE INTENT TO RESIDE IN BAHAMA'S OR KNOWS SOON HE WILL GET CAUGHT UP..... SO THIS MEANS ANNA & DANIEL ARE ALONE IN BAHAMA'S AM I CORRECT??
