Anna Nicole's Death Certificate

TMZ has obtained the official certificate of death for Anna Nicole Smith, filed earlier today in Broward County, Florida.

On the document, Howard K. Stern is named the "Informant," and describes his relationship to Anna as "Nominated Executor." What's interesting is that he did not list himself as Anna's husband, boyfriend or even lawyer. An official from the Broward County Office of Vital Statistics told TMZ, "We would have not given him that title without his request."

The certificate also states that Anna's level of education is "8th or less."

The former Playboy Playmate and TRIMSPA spokesperson was discovered unconscious in her hotel room at the Seminole Hard Rock Hotel and Casino in Hollywood, Florida on February 8. Paramedics rushed her to Memorial Regional Hospital where she was pronounced dead shortly after arrival. She was 39.

47. People, for the love of God, get a life and let the court system resolve this mess. It’s none of our business when she is buried or cremated. Anna Nicole Smith's defiant gesture towards her mother on that television video is just one disturbing peice to this puzzle. She became a drug addict and the victim of the sex trade industry. This is how females are discriminated against in year 2007. Instead of disparaging Virgie Arthur, point your poison arrows at billionaire Howard Marshall, other millionaires, Hugh Hefner of the Play Boy Magazine, Howard Stern with a juris doctorate degree, etc. --- all those monsters were feeding off a girl with the equivalent of an 8th grade education. SICK!!

I'm guessing he had to give a legal relationship which I don't think boyfriend qualifies as and he has always admitted they were not legally married. 8th grade is probably the last grade she completed/passed. She probably went to High School but did not earn enough credits to pass the 9th grade so 8th is her legal status. They say she was working and married at 17 right? so I'm guessing those are the correct legal answers required.

52. I have a question Stern lists himself as the Nominated Executor. He also lists his place of resident as Los Angelas, Calf. He is not and did not see himself as a resident of Nassau, Bahamas. At least at the time Anna Died.
What's his position now? If he considers himself the father of Dannielyn who lives in the Bahamas and supposedly where they all planned to live, why did he give the LA address as his place of resident ??? A slip up I think ??

Howard the Rotter is representing himself as having a business relationship on the death certificate. But when it furthers his aims he represents that he had a deep personal relationship with her.

59. It's only a Certificate confirming she's dead. The cause of death has not been officially determined yet (or at least, hasn't been released publicly yet) so it couldn't be put on the certificate. This isn't uncommon.
