Anna Nicole's Death Certificate

TMZ has obtained the official certificate of death for Anna Nicole Smith, filed earlier today in Broward County, Florida.

On the document, Howard K. Stern is named the "Informant," and describes his relationship to Anna as "Nominated Executor." What's interesting is that he did not list himself as Anna's husband, boyfriend or even lawyer. An official from the Broward County Office of Vital Statistics told TMZ, "We would have not given him that title without his request."

The certificate also states that Anna's level of education is "8th or less."

The former Playboy Playmate and TRIMSPA spokesperson was discovered unconscious in her hotel room at the Seminole Hard Rock Hotel and Casino in Hollywood, Florida on February 8. Paramedics rushed her to Memorial Regional Hospital where she was pronounced dead shortly after arrival. She was 39.

62. I was reading an old tmz article from when anna first died and here is what her sister(who hated her) even said in the press release.
Even she says how much anna wanted out of texas....

wonder if they could usethat in court. It shows even the family new she didnt want to live there...let alone spend eternity there!!

71. Don't forget she let her son lay on a cold slab for 39 days...But you never heard anyone say he was deteriorating and needed to be buried right away ! what goes around comes around....Hope they don't allow that Coward Howard run the funeral show !!

72. I am with you #25. I have already thought about the fact that once Howard is kicked out of the house that he is staying in that he will go back to California and Anna Nicole and Daniel will be left alone in another country where all family members will have to travel long distances just to visit them. Maybe is this case(and only this case)Howard is doing the right thing by Anna Nicole in carrying out her wish to be buried in the Bahamas along with her son.

75. Heard on the news that the funeral is planned for 10:30 on Friday...and that ALL parties have been invited to participate, by words or writings for the program.
