Anna Nicole's Death Certificate

TMZ has obtained the official certificate of death for Anna Nicole Smith, filed earlier today in Broward County, Florida.

On the document, Howard K. Stern is named the "Informant," and describes his relationship to Anna as "Nominated Executor." What's interesting is that he did not list himself as Anna's husband, boyfriend or even lawyer. An official from the Broward County Office of Vital Statistics told TMZ, "We would have not given him that title without his request."

The certificate also states that Anna's level of education is "8th or less."

The former Playboy Playmate and TRIMSPA spokesperson was discovered unconscious in her hotel room at the Seminole Hard Rock Hotel and Casino in Hollywood, Florida on February 8. Paramedics rushed her to Memorial Regional Hospital where she was pronounced dead shortly after arrival. She was 39.

811. You seem to be very keen of blaming Howard K. Stern for her death. Why can't you be rational enough to put the blame on the one responsible for this beautiful woman's unpeaceful life. The blame should go to her mother who only seem to be interested in making money off of her daughter's death; and failed to love her as a good mother should. as for Larry who is so pretensious, where was he when this woman he pretends to have loved so much was going through the pain of losing her son. Give Howard a break, his only crime is loving Anna unconditionally, which seems to be an impossibility for the average American such as your self. Let Anna rest in-peace. Find some one else to pick on.

Her Mother is lier, sad woman and a Bad Mother. Her Bodyguard shouldn't lie like that. She was a great woman, and a Loving mother of her late son and her preious daughter.You people on the News, TMZ.com. Its bad enough she is gone and her son to. Leave her in peace. 814. i just look up the address in California the one in Los Aneglos in a insurance company and i also seen the same address for Santa Monica and there is no address for there. i look it up on white pages.com with the reserve address icon. so Howard k. stern is a lair and i some Larry Birkhead gets Dannielynn an raised her up good. and may god Anna Nicole Smith rest in peace. love ya girl

815. While so many of you are bashing Virgie, none of you seem to realize that in spite of her inappropriateness, she is STILL Anna's mother & Daniel's grandmother. There isn't any doubt to the dysfunctionalism in this family, BUT ... Virgie is doing what "she" thinks is ok. She's not perfect, but neither do I feel she loved her daughter and/or grandson any less. I do not, nor will not, judge Virgie. Her actions may be questionable, perhaps even "highly" questionable, but what dysfunctional behavior would you act out after the death of your child & grandchild? Or perhaps you have already "acted out" behavior that would not be considered appropriate.
