Anna Nicole's Death Certificate

TMZ has obtained the official certificate of death for Anna Nicole Smith, filed earlier today in Broward County, Florida.

On the document, Howard K. Stern is named the "Informant," and describes his relationship to Anna as "Nominated Executor." What's interesting is that he did not list himself as Anna's husband, boyfriend or even lawyer. An official from the Broward County Office of Vital Statistics told TMZ, "We would have not given him that title without his request."

The certificate also states that Anna's level of education is "8th or less."

The former Playboy Playmate and TRIMSPA spokesperson was discovered unconscious in her hotel room at the Seminole Hard Rock Hotel and Casino in Hollywood, Florida on February 8. Paramedics rushed her to Memorial Regional Hospital where she was pronounced dead shortly after arrival. She was 39.

76. Hmm..... why does it say she died in Hollywood? And where is the cause of death? I've never seen a death certificate that didn't list the cause of death. Odd.

It seems Howard is realizing his meal ticket is gone. He also listed her as a widow, so he couldn't list himself as her husband.

I think this is a fake... but by law he is her executor, so unless he intervened in the gathered information of the certificate, that's what they'd automatically name him per the registered will in California. Do you actually think he had time to sit around waiting for the ME to fill out this paper???? I do not.

Personally I do not believe they would issue a death certificate without first completing her cause of death investigation because a cause of death is a required element to officially issue a death certificate (in any state and most countries). I do not see ANY REFERENCE to a cause of death listed on this ceritificate --- sooooo that smells fishy to me.

DO NOT BELIEVE IT!!!

Virgie Arthur is vile, vile, vile. Anna spoke of physical, verbal and sexual abuse (last by one or more of her mother's husbands). Arthur also has a son in prison. None of her children are supporting her. In fact, no relative is flying around with her except one man with a crew cut. A husband? That disgusting woman was not welcomed in Anna's life for many years. Thanks to Arthur, Anna wasn't allowed a proper burial, either. That woman made Anna's life miserable almost all of her 39 years. Virgie is trying for top dollar here. There can't be any other motivation. She's a pig.

82. Dr. Perper said that a preliminary death certificate could be issued without the cause of death.
A new one will be issued once ALL of the test results are back and a cause of death has been determined.
At least the good doctor has some integrity........................

83. that's a pending death certificate. Dr. Perper said on NG show I believe that until they have the final toxicology report a real one couldn't be done, so they do a pendiing one for burial I believe.

84. I just looked up the reverse adress. It is that of Ronald Rale. More decieptfullness and hiding from Coward. Maybe he really doesn't have an existance without Anna. Hope there is a view for him of the cemetary from his jail cell! Can convicted felons in the Bahamas profit off of their crimes? That 's the only Anna money he'll get a chance at.
