Anna Nicole's Death Certificate

TMZ has obtained the official certificate of death for Anna Nicole Smith, filed earlier today in Broward County, Florida.

On the document, Howard K. Stern is named the "Informant," and describes his relationship to Anna as "Nominated Executor." What's interesting is that he did not list himself as Anna's husband, boyfriend or even lawyer. An official from the Broward County Office of Vital Statistics told TMZ, "We would have not given him that title without his request."

The certificate also states that Anna's level of education is "8th or less."

The former Playboy Playmate and TRIMSPA spokesperson was discovered unconscious in her hotel room at the Seminole Hard Rock Hotel and Casino in Hollywood, Florida on February 8. Paramedics rushed her to Memorial Regional Hospital where she was pronounced dead shortly after arrival. She was 39.

91. I agree with #85 and #87. I hope Howard gets it all. Virgie and Birkhead deserve nothing. Where were they when she needed them. Only Howard stuck by her through it all.

99. Come on TMZ stop putting fake docs up. The death certificate is signed and dated on 2/26/07 - now the court just handed down on 2/28/07 that she will be buried in the bahamas (your even reporting that just came down from the court)

The medical examinor said he was not going to release the body until it was decided by a court of law. So again, they just decided on 2/28/07 and you are showing a fake document dated on 2/26/07 which is two days earlier.

Harry come on your a lawyer - but are you one of those that makes up crap to win your arguments?

People people this is a fake document and that is why the scan job is so fake. TMZ wants you to believe them. Sorry TMZ but with todays scanners you can even scan a penny and the resolution will be better than this one.

102. If the decision by the judges was just a few minutes ago on 2/28, why was it registered on 2/26? Think the appeal judges already knew they planned to affirm Judge Larry's decision?

Wonder if Vergie is planning to get paid to go to her daughter's funeral. Vergie will probably give photos to the highest bidder.... Thank goodness she can now rest in peace beside her sweet son.

103. #95. Not to say that you are stupid.....but the body was to be released on Monday, therefore the date 2/26. The only thing the judges did today was UPHOLD the earlier judgement of Seidlen's. NICE!

104. Hollywood, Florida is a suburb of Ft. Lauderdale, Florida and it is the location of the Sminole Hard Rock Casino and Hotel where Anna died. It is also the location of the hospital where she was taken.

105. Looks like the Certificate is dated February 28, 2007, to me. TMZ does NOT show fake documents.
