Quaid Medical Screwup -- How It Happened

Sources tell TMZ that a pharmacy technician at Cedars-Sinai Medical Center mistakenly stocked a massive dose of a drug that ended up being given to Dennis Quaid's newborn twins.

Thomas Boone and Zoe Grace are in stable condition. But a well-placed source at Cedars tells us they are "still very concerned because of the bleed out," adding they won't know for another week if the mistake will cause "longterm effects."

Sources tell TMZ that pharmacy technicians stock the drug Heparin, used to prevent clots and flush out IVs. The drug comes in vials -- 10 units for babies, up to 10,000 units for adults. Protocol at the hospital is to keep the different units separated, but a technician accidentally put 10,000 units in the drawer where the 10 units were stored.

Last Sunday, both infants -- born November 8 by surrogate -- were each given two, 10,000-unit dosages. They began to bleed out just before midnight and were transferred to the neo-natal intensive care unit.

Cedars issued a statement last night, acknowledging the mistake and calling it a "preventable error." That's highly unusual. Also, the hospital claims seven patients were given the wrong dosages. Our sources say 13 patients got the wrong dosage.

17. first of all it wasnt the PHARMACY TECHICIANS fault,...it was the DOCTORS AND NURSE WHO DIDNT DOUBLE CHECK THE MEDICATION..GET THE STORY RIGHT..PPL..A PHARMACY PUTS THE MEDS ON A PATIENT BUT THE DOCTOR PRESCRIBES IT, AND THE NURSE(RN) DOUBLE SEES...IT...DAMN THANKS DRS..FOR MAKING THE PHARMACY TECH'S LOOK BAD TO SAVE YOUR BUTTS...WELL IM GLAD THE BABIES R DOING WELL..HOPEFULLY YOU FEEL MUCH BETTER SOON...

18. I spent alot of time at this hospital last summer, and you can be sure...the nurses are all socializing at the front desk most of the time. They might run off, one at a time, here and there...but they are not focused whatsoever. JMHO.

19. You would be shocked to learn how much of your health and safety are in the hands of pharmacy techs. They are paid low wages and do tedious, detail oriented work. It's surprising more accidents don't happen.

21. #5, How Dare You attribute this tragic medical error to the parents' decision to use a surrogate?!? Tell us how the surrogacy directly caused the error? Are the pharmacy techs and nurses more likely to err on babies who were born to surrogate parents?

How do you even know for certain that her decision to use a surrogate was vanity? There could have been some pressing medical reason she had to use a surrogate mother, and she shouldn't have to answer to you by sharing her medical issues with the whole world.

By your logic, all babies who suffer illness or death must have had vain or selfish parents causing their "bad karma." What a disgusting, small-minded thing to say.

22. AND the ONLY reason we're hearing about this is because of the famous dad. AND Kanye West's mom isn't the first person to die after having plastic surgery. Because of their celebrity is the only reason we're even hearing about it.

24. I love how the reports/hospital are alluding to the fault lying with pharmacy tech, where in reality, the fault lies with the nurse. It doesn't matter where he/she grabbed the heparin from; it is an absolute necessity to triple-check yourself EVERY single time you give a dose to a patient (particularly in pediatrics!!!). That's basics -- nursing 101-- and it is his/her fault that these babies may have lasting repercussions from their massive anticoagulant overdose. Nurses get lazy-- I see it every day. Hopefully this is a wake-up call for others to be more wary.

The RN administering the medication should have checked the name of the medication and the DOSE on the vial before administering heparin or ANY medication....5 RIGHTS...right patient, right medication, right dose, right route, right time & frequency. Had this safety guideline been followed...as is taught in nursing school...this incident would not have happened. The person administering the medication is responsible.

Why wasn't this mistake caught the FIRST time?? Looks like there will be several lawsuits filed on this hospital......all because of a stupid pharmacy tech not paying attention to what they were doing. Now, this means that the next poor schmuck that walks in for treatment will be charged out the ass to recoup the costs of the lawsuits filed and the next guy after that and the next guy and so on.........and we wonder why people can't afford medical treatments, insurance, etc.....ALL BECAUSE A STUPID PHARMACY TECH DIDN'T PAY ATTENTION TO THEIR JOB!!!!!!! Thank you stupid pharmach tech......

30. And...., this is the hospital for the rich & famous. Where a simple Aspirin tablet given to the patient is charged $35.00.

Yet, the level of care is almost equal to a third world nation. God knows how many mistakes like mentioned and cost the patients life occur, without ever reporting.

Our medical system is a joke! The only thing top level is they take top Dollars that is all.

I know! My mother was hospitalized there!
