Quaid Medical Screwup -- How It Happened

Sources tell TMZ that a pharmacy technician at Cedars-Sinai Medical Center mistakenly stocked a massive dose of a drug that ended up being given to Dennis Quaid's newborn twins.

Thomas Boone and Zoe Grace are in stable condition. But a well-placed source at Cedars tells us they are "still very concerned because of the bleed out," adding they won't know for another week if the mistake will cause "longterm effects."

Sources tell TMZ that pharmacy technicians stock the drug Heparin, used to prevent clots and flush out IVs. The drug comes in vials -- 10 units for babies, up to 10,000 units for adults. Protocol at the hospital is to keep the different units separated, but a technician accidentally put 10,000 units in the drawer where the 10 units were stored.

Last Sunday, both infants -- born November 8 by surrogate -- were each given two, 10,000-unit dosages. They began to bleed out just before midnight and were transferred to the neo-natal intensive care unit.

Cedars issued a statement last night, acknowledging the mistake and calling it a "preventable error." That's highly unusual. Also, the hospital claims seven patients were given the wrong dosages. Our sources say 13 patients got the wrong dosage.

Question: Why are babies given Heparin in the first place when at birth newborns already have clotting problems (immature liver, relying on mom to do all the work of getting rid of wastes) and are are given a dose of Vitamin K? I do not understand the logic that any newborn should receive ANY anticoagulants. A concerned RN

92. Ultimately it is the person giving the medication who is at fault. Remember the five rules of med giving? One is to check the MED before giving it.

94. That is crazy I live here in Indianapolis, Indiana and that happen but the babies ended up losing their lives. People need to get it together. Half of the problem is the have these QMA course that are fast pace and they are not learning proper medicene. They really need to get it together and train these people right.

95. As an RN of 30 years, I can say without hesitation that there is NO EXCUSE for an error like this to have occurred--not once, but 13 times !!!! Can't read, don't know. don't care, too busy to care--pretty damn sad excuses for potentially, if not actually causing permanent damage to a helpless individual. I work in a hospital that has multiple levels of checks and balances to prevent just this type of error. What--can't Cedars-Sinai afford it ??!!

96. As a 28 year nurse (RN), I take exception to those that write that nurses are more interested in getting their cigarettes and donuts than patient care. We are suppose to get two 15 minute breaks and a 30 minute lunch in an 8 hour shift. Most days, I am lucky to get a 15 minute break for the whole shift and that is a potty break and a quick sandwich. From what I see, the problem with our health care is the shortage of nurses and the hiring of para-professionals (patient techs, medication techs etc.) to do what was once our job since there are not enough of us to go around. We are in a dire shortage of RNs. LPNs and CNAs across the entire country.

97. This is sadly not a surprise...errors are common--this time brought to light by celebrity status and thankfully brought to light! However, you should really check-out the truely AWFUL stuff that happens/happened at Martin Luther King Jr./Harbor UCLA county hospital, also in Los Angeles!! NOW there's a real bunch of screw-ups...that has cost more than one life!!!

99. And to Flyonthewall who thinks that you should only trust Doctor's. HA! When it is the middle of the night and you clinically DIE, don't thank a Doctor who by the way is home sleeping, thank your RN and the Respiratory Therapist and an ICU RN who handles the code until an overworked E.R. doc might and I mean might get there. It is our knowledge and training that keeps you alive while another RN calls the Doctor at home to tell him or her what has happened and the outcome. 9 times out of 10 it is those dedicated people who keep your heart beating and your brain functioning - NOT a Doctor. Please don't thank the Doctor when you wake up when it was the nursing and the respiratory staff that kept your butt alive. We don't get paid for saving you, it's all in a day's work but I bet the absent Doctor will charge you for his absence. Oh by the way, Pharmacist's who get paid way more than RN's stock all the Code carts in most hospitals but we have to double and triple check them. No wonder there is a nursing shortage!

Wow, it sounds like you need to find another profession! You seem so bitter and outraged! You want all the glory and the honor for being "the nurse" How about being THANKFUL for being BLESSED with your talents and skills that the good Lord has BLESSED you with!!

Do you know how many people would kill to be able to afford to go to school to become a medical PROFESSIONAL? You sound like a jaded, pissed off ole fart that has cleaned one too many bedpans.

Where's your bedside manner? Oh wait, maybe it was in the sentence you wrote of thanking you for SAVING LIVES when it's in your Job Discription anyways! That is after the profession you CHOOSE to go into.

So quit Whining about wanting a medal!!!

105. I feel really awful for the Quaids'. The same thing happened a few months ago at a hospital in my area. The drug company said they were going to change the packaging to make the drugs more identifiable, but I guess they haven't yet. As a nursing student it really frightens me but makes me very aware to always check medications and never take anything for granted.
