Cedars at Fault for Quaid Twin Drug Mix-Up

A citation has been issued against Cedars-Sinai for a massive medical mix-up that injured three newborns, including the newborn twins of actor Dennis Quaid.

The L.A. Times is reporting the California Department of Public Health found the hospital failed to "adhere to established policies and procedures for safe medication use ... These violations caused, or were likely to cause, serious injury or death to the patients who received the wrong medication."

The twins recovered and are home.

The report also indicates the hospital's practices "created a risk of harm for all hospital patients." According to the report, hospital staff and nurses failed to check labels before they administered the drug to the patients, including the Quaids. They also did not maintain proper records of the medication that was used, even though there were already policies in place to prevent this type of mistake from occurring.

Kathleen Billingsley, deputy director of the Center for Healthcare Quality, tells the Times the public health agency has not yet decided whether to fine Cedars-Sinai for the error.

The Quaid twins were twice given 10,000 units of the blood thinner heparin, when they should have received 10 units. The report indicates two pharmacy technicians mistakenly stocked the cabinet with adult dosages of the drug. The nurses involved told inspectors they could not remember whether they read the label on the heparin vials or not.

The third child who also received the overdose did not require further treatment and was released the next day.

In a written statement to the Times, Cedars chief medical officer Dr. Michael L. Langberg said, "While this is a rare event, we are pleased that the [public health department] shares our view that it is an important opportunity for the entire institution to explore any and all ways we can further improve medication safety." He also said "Each of us is personally responsible for patient safety."

16. Just today a study came out that revealed that the U.S. is DEAD LAST in healthcare in the world. France was first, Canada third, the U.S. was DEAD LAST. Worst healthcare in the world. We NEED UNIVERSAL HEALTHCARE. Then we wouldn't have MEDICAL ASSISTANTS (only trained basically to take blood p ressure) doing the job only an RN should be doing, either. The nurse that posted above is correct. These kinds of mistakes happen ALL THE TIME and no one knows until it happens to them. THE U.S. HEALTHCARE SYSTEM IS GARBAGE!!! And yet we pay for it more than any other country in the world. Go figure!

WASHINGTON (AFP) — France is tops, and the United States dead last, in providing timely and effective healthcare to its citizens, according to a survey Tuesday of preventable deaths in 19 industrialized countries.

The study by the Commonwealth Fund and published in the January/February issue of the journal Health Affairs measured developed countries' effectiveness at providing timely and effective healthcare.

The study, entitled "Measuring the Health of Nations: Updating an Earlier Analysis," was written by researchers from the London School of Hygiene and Tropical Medicine. It looked at death rates in subjects younger than 75 that could have been prevented by timely and effective medical care.

The researchers found that while most countries surveyed saw preventable deaths decline by an average of 16 percent, the United States saw only a four percent dip.

The non-profit Commonwealth Fund, which financed the study, expressed alarm at the findings.

"It is startling to see the US falling even farther behind on this crucial indicator of health system performance," said Commonwealth Fund Senior Vice President Cathy Schoen, who noted that "other countries are reducing these preventable deaths more rapidly, yet spending far less."

The 19 countries, in order of best to worst, were: France, Japan, Australia, Austria, Canada, Denmark, Finland, Germany, Greece, Ireland, Italy, Netherlands, New Zealand, Norway, Portugal, Spain, Sweden, the United Kingdom and the United States.

Some countries showed dramatic improvement in the periods studied -- 1997 and 1998 and again between 2002 and 2003 -- outpacing the United States, which showed only slight improvement.

White the United States ranked 15th of 19 between 1997-98, by 2002-03 it had fallen to last place.

"It is notable that all countries have improved substantially except the US," said Ellen Nolte, lead author of the study.

Had the United States performed as well as any of the top three industrialized countries, there would have been 101,000 fewer deaths per year, the researchers said.

18. What happens now to the Quaid's lawsuit against Baxter? It was evident from the beginning that it was Cedar's fault and not Baxter.
I was told that if the twins did bleed out, which I understand they did, they can appear to be fine now and years later something can go wrong as a result of this overdose. Anyone in the medical profession know if this is true?

21. My heart and prayers go to the Quaid family. I cant imagine how they must be feeling that this happened. I can say that if it was one of mine, hell would be happening in Cedars-Sinai Hospital.

I hope this poor judgement by that hospital will not go unnoticed. It really would be hard for me to ever trust that hospital again. I hope the children are going to recover from this and are going to be ok. My wishes to the Quaid family!

24. What do they mean the Health Department isn't sure they are going to fine the hospital for this mistake??????
My mom died because of nurse error. And what do the nurses mean they couldn't remember if they read the labels on the medication vials or not? HU?? sounds like a cover up to me.
Not only should the hospital be sued but those nurses should be fired and have their nursing license revoaked.!!!

25. It's very ubiquitous in the air of Cedars-Sinai Hospital. Grandpa Davis would roll over in his grave, he built that Hospital. Wisdom seems to skip some generations in that family, like the present one. It got replaced with Mr Bean.

Maybe in Gummi's "logic" he thinks if I eat the person with wisdom....I'll get some. Please explain to him it doesn't work that way!

That Hospital needs fresh air, way too dusty. No more Bears. No more Mr Beans. Mr. Bean is funny purely and simply because he is a total buffoon.
