Denise: I Want My Name Back Bitch!

TMZ has obtained paperwork filed Friday by Denise Richards asking that her name be officially changed back to that -- Denise Richards.

Richards had changed her name legally to Denise Sheen when she married Charlie.

A judge approved the change. No Sheen in her game!

31. And..... who really cares??? Charlie and his family are probably glad she didn't disgrace the Sheen name that she has done a good job for herself as Denise Richards. She's just like Heather Mills - the more she opens her mouth the worse she makes herself look in the public eye.

32. Good riddens!!! who want that tramps name attached to Sheen. Can't make it on your own so keep hubby last name until now, get a back bone sister and leave the children out of you side show you want to make about yourself. Embarass yourself but leave the poor children out of it.

33. Big deal. I took back my mainden name when I got divorced, but they didn't write a story about it. Jeez.......
