TMZ has learned ... Dr. Frank Ryan -- a prominent Beverly Hills plastic surgeon to the stars, famous for Heidi Montag's extreme makeover, is dead ... killed in a car crash.

Dr. Frank Ryan's Death

According to law enforcement, Dr. Ryan's Jeep went over the side of Pacific Coast Highway yesterday at around 4:30 PM.  The Jeep landed on rocks and lifeguards initially tried to help Ryan, to no avail.

Ryan, who was 50, was trapped in the vehicle and had major head injuries.  The Ventura County Fire Department tried hoisting Ryan using a helicopter, but the attempt was called off. Ryan was pronounced dead at the scene.

Minutes before the crash, Ryan Tweeted, "After 25 years of driving by, I finally hiked to the top of the giant sand dune on the pch west of Malibu.  Much harder than it looks!  Whew!"

Ryan's last Tweet before he died, "Border collie jill surveying the view from atop the sand dune."  Ryan posted the picture below with his Tweet.  Law enforcement tells us the dog was injured but survived.

Dr. Ryan performed rhinoplasty, septoplasty and minor chin lipo on me last year. I am absolutely devastated today. Not only was he a brilliant surgeon, he was an extremely kind hearted wonderful person. He will be missed!!! I was supposed to see him on September 2nd for a 10 month check up. I just got the cancellation phone call a few minutes ago. It's so surreal. I feel awful :( My heart goes out to his family and friends.

Dr. Ryan was a wonderful man who gave selflessly to underpriviledged children through his organization and at the Bony Pony Ranch. He gave 100's of ex-gang members free laser removal of their gang tattoos, and also gave free plastic surgery to children born with embarrassing birthmarks on their face. The man is dead- lets remember what he did that was GOOD- not just sensational.

WOW...poor Dr Ryan he didn't deserve to die like that at all. I used to work for him, he was a good boss. All his patients loved him, he did really good work. Who's going to run the Bony Pony Ranch now? Just so so so sad.

Frank was a caring, kind and beyond talented surgeon. He was accepted into a very very difficult fellowship for plastic surgeons that perhaps only 1 or 2% of the plastic surgeons IN THE WORLD are even asked to. He had a passion for helping children with birth defects and he repaired many many accident victims, battered women...faces of people who were ready to give up. All gratis.

To many of you reading these posts you assume that all of these words about him are canonizing him in some way... most of the time you would be right but not with Frank, he really was one of the exception individuals who instinctively made you feel better with his words and demeanor.
For those of you who picture him as a greedy surgeon, I once accompanied a mature friend of mine to see him (which is how I met him) and she wanted to do some "lifting" in a few areas along her body. A "greedy" surgeon wold have just said yes, yes Yes to her requests.. but not Frank, he sat down and told my friend that what she was asking wasn't necessary--he really did care, he held her hand and LISTENED. She chose not to do have as much surgery and she left feeling better about herself.
Frank's charity work was more important to him then anything-- his work rescuing underprivileged youth was heroic and he loved and SAVED many dogs along the way. This man GAVE most of his time away.
Frank had this way about you that made you WANT to be your best around him. I am glad he had a great weekend and happy that on the day he left us he did something that HE had been wanting to do-climb that dune. To God, thank you for giving us this talented bright man he served the world well. Frank you are deeply missed.
For those who loved him, I encourage us all to continue to support the Bony Pony Ranch www.drfrankryan.ORG this was his true passion, and his legacy.
Goodbye sweet friend,
C

He just finished climbing a sand dune which is physically exhausting. As anyone who has tried to climb those steroid fueled ant hills can attest to. Possible heart attack? There are numerous cases where someone appeared to be in perfect health, but died from an underlying medical condition that was not discovered. Condolences to his family. Many Hollywood stars are sad today. You can't tell by looking at them because they can't frown... but I'm sure they're sad. (Sorry, I couldn't resist.)

Maybe he lost consciousness from a stroke or heart attack. He was an older man that had just climbed up a steep embankment.
