EXCLUSIVE

Cory Monteith died from a lethal combination of heroin and alcohol ... this according to Canadian authorities.

The British Columbia Coroners Service says an autopsy and toxicological analysis show that the 31-year-old "Glee" star died on from a "mixed drug toxicity, involving heroin and alcohol."

The explained... "At this point there is no evidence to suggest Mr. Monteith's death was any other than a most-tragic accident."

Monteith's body was discovered by staffers at the Fairmont Pacific Rim hotel in Vancouver, Canada on July 13.

Monteith had struggled with addiction for years and had been to rehab earlier this year.

Monteith had said he began struggling with drugs and alcohol as a young teenager.

CORY MONTEITH'S DOUBLE LIFE

SOBER FOR 'GLEE'

Wild in Vancouver

0715-cory-monteith-leah-michelle_ex2We previously reported Cory Monteith was leading a double life -- a hard-working, "Glee" star in L.A. struggling to stay clean ... a hard-partying substance abuser in Vancouver.

CORY'S L.A. LIFE

Our sources say Cory's GF Lea Michele and his "Glee" cast knew his struggles all too well and spent a lot of time keeping him clean and sober.  We're told Cory never showed up on the set under the influence of anything and most of the time abstained from drugs and alcohol.

The pic above shows the last trip Cory and Lea took together -- it was taken on June 18th in Fire Island, where they visited a friend on a movie set.  We're told Cory was sober the entire time.

There were, however, lapses, which is why he checked into rehab in April.

CORY'S LIFE IN VANCOUVER

Our sources say Cory's resolve to stay clean dissipated whenever he returned to his native Canada.  That's where he became a drug addict with a serious drinking problem.

Corey went back to Vancouver many times, and hung out with many of the friends who helped fuel his addiction.  We're told alcohol and drugs were always involved.

Our sources say Cory's family was aware he reverted back whenever he went North of the border, and they tried in vain to help.  We're told as family members became alarmed recently and began making a full-court press ... Cory became distant.

Somehow, Cory was able to compartmentalize his life into a tragic tale of 2 cities ... and the city where he slipped became the city where he died.

And he was a role model for teenage boys. Laughable. I pity no fool like this. When you have the world at your feet and this is what you do, then you deserve to die.

"....friends who helped fuel his addiction". Those aren't friends, and they were probably jealous of his success. Sad.

He did not accidentally ingest heroin and alcohol. He took those things deliberately thus causing his death. Not an accident and I wish they would stop calling deaths like this accidents. It's like one bullet in the chamber and spinning it and pulling the trigger. It's not an accident. It is what it is suicide.

So he died from a heroin and alcohol od. Enough with this and move on. Stop glamorizing drug addicts like Whitney, or Michael or Janis, or Jimi or Elvis.

It makes me so angry when young people die like this. And especially this guy. He had it all; could have had just about anything he wanted and he pisses it away

People think they know it all where in fact you idiots don't know crap ! Drug addiction is a desease same as many other health problems !!! You can't tell a cancer victim to buck up and get over it nor can you tell an addict to stop it and grow up !!!! But you can work on stupid if you try...so grow up and have some mercy for a young life that was taken way to soon !! R.I.P. Corey and I wish some peace for you freinds and family

My condolences to his family and friends. His friends should have done more to keep this young man away from his addictions and he should have known to keep away from people places and things that cause him to relapse. They teach you that in rehab. I know this and those of you who are like me know this. TG I have been clean and sober now for 18 yrs.
