Philip Seymour Hoffman's recent and fatal path down the road of addiction was triggered by an innocent drink ... sources close to the actor tell TMZ.

Hoffman had been sober for 23 years prior to shooting "The Master" -- but during a wrap party in 2012, the actor succumbed to temptation ... and celebrated the movie with a drink ... which quickly became a couple of drinks.

Hoffman acknowledged to confidants ... the drinks opened the floodgates.  Addictions experts all say one drink can destroy an addict's life ... and that seems to be what happened here.

Sources tell us Hoffman didn't begin experimenting with heroin until 2013, after he had already fully fallen off the wagon. We're told he had long been curious about the drug.

It started with snorting -- and last May, he quickly checked into rehab because he was terrified it would escalate to IV use ... which it eventually did.

After 24+ years sober I do not take sobriety for granted, nor will I ever. As a sober friend once described it, addiction is like a jealous and patient lover. It'll wait as long as it has to. One drink IS all it would take - and that one would be too many and the thousand that followed would never be enough...

I have compassion for people who struggle with addiction! However, it defies logic or any small amount of intelligence for someone who KNOWS he is an addictive personality & has struggled with sobriety for 23 years to claim they are "curious" about a drug that is well known to be quickly addicting, grossly expensive AND dangerous because of impurities.

Philip had to have some very serious mental/emotional problems if he'd be "curious" about trying heroin.

Sad that he left 3 young children fatherless. Sad for his family & friends who are left behind to try to make sense of his sad choice.

RIP, Philip Seymour Hoffman.

There is a blind item on another website that heavily hints to PSH saying that it was all bullsh*t that he was sober for that long beforehand and that he has been off the wagon for a very long time now. Look up the site Blind Gossip, they are all over that story.

Please! They call drug use a decease when nothing is further from the truth! Cancer is a decease, drugs are by choice, period! People use them because they like them not because they have decease. It's so weird, after this celebrity dies, the police go on an immediate hunt for anyone involved when there are people dying everyday due to the over dose of heroine but no attention is paid to the average Joe. Society is so full of ****!
