Former "Celebrity Apprentice" contestant Claudia Jordan told police ... she was attacked by a crazed woman who's currently involved with her ex-husband ... and it all went down at Drake's Grammy after party.

TMZ has learned ... the actress/former "Deal or No Deal Model" claims she was at Drake's party at Greystone Manor in L.A. when the woman assaulted her out of nowhere -- taking a swing at her and connecting with the top of Claudia's head ... in the middle of the nightclub.

According to Claudia, her ex recently broke up with the woman in question -- and for some reason, she blamed Claudia for things going sour. Claudia says she hasn't spoken to her ex in two years.

Claudia tells TMZ ... she didn't say a word to the woman the entire night -- but the unidentified female was staring daggers at her the entire time ... before eventually losing it.

Security at the club was notified and the woman was tossed out.

Claudia tells us, she filed a police report with the LAPD -- and went to the hospital Monday to have her head and neck checked out.

Claudia Jordan is terrified for her life after months of super-creepy run-ins with a man who claims he "loves her" -- and after his latest threatening phone call she's turning to police.

The actress and former "Deal or No Deal" model filed a stalking report with the L.A. County Sheriff's Department this week.

Claudia tells TMZ the man started showing up outside events she was hosting last November -- and then sent letters with frightening messages like, "How can you not want me?" She says he also wrote that he was attracted by "the aroma of [Claudia's] blood."

Claudia says the final straw was a recent voice mail in which he angrily says, "Since you won't come and talk to me on a real level. You wanna go f**kin' snitch to police."



The message continues, "Write me a check for $25,000 and I'll leave you the f**k alone. You playing with the wrong ni**er."

Law enforcement sources confirm they are seriously investigating the matter. Claudia tells us she's also considering going to the FBI ... since she feels like the man is trying to extort her.

It might be the sexiest DUI arrest ever -- Claudia Jordan's smokeshow friend BUSTED after sideswiping a parked car ... while driving a freaking DeLorean ... and it was all caught on TAPE.


Law enforcement tells TMZ, model SueLyn Medeiros -- we strongly recommend Googling her -- was pulled over last night in Hollywood after colliding with a parked car, and when she opened her DeLorean's gull-wing door, cops caught a "strong odor" of alcohol.

Police administered a field sobriety test -- and Medeiros, who had been driving with Claudia in the car, laughs the entire way through, prancing around barefoot ... needless to say, cops weren't impressed.

Medeiros then blew a .14 -- nearly twice the legal limit in California -- and was placed under arrest.

According to law enforcement, Medeiros bragged incessantly about being a rich model, and was extremely flirtatious with the officers.

Medeiros is still in custody on $45,000 bail. As for the high bail amount, we're told she had an unrelated warrant out for her arrest ... but it's unclear why.
