Tom Sizemore's ex-girlfriend says the actor knocked her up with twins then left the three of 'em to fend for themselves -- because he allegedly bailed on a massive amount of child support.
According to a lawsuit filed last week in L.A. Superior Court, Sizemore owes babymama Jinele McIntire $58,800 in child support for the couple's two boys, who were born in 2005.
In the first line of the lawsuit, Jinele makes her point clear, "TOM SIZEMORE is a DEAD BEAT DAD."
Jinele says because of Tom's non-payment, she's been forced to live with a friend while the kids stay with her brother in Texas. She adds Sizemore "has made no attempt to see them there."
Jinele also claims Tom last saw the boys when he was taping "Sober House" -- a gig Jinele says paid $200,000. She also claims Tom made another $200,000 for "Celebrity Rehab."
Besides the overdue child support, McIntire is also seeking health insurance coverage for the kids and $50,000 for attorney's fees.

Tom Sizemore has been charged with one count of spousal battery over an allegedly violent incident with his girlfriend earlier this month.
As TMZ first reported, the former "Saving Private Ryan" actor was arrested in L.A. on August 6th after a verbal argument allegedly turned physical and someone called 911.
If Sizemore is found guilty of the misdemeanor charge, he could do up to a year in jail and face a maximum $2,000 fine.

It's been 70 days since his last arrest -- and actor Tom Sizemore is already back behind bars!
TMZ has learned the former "Black Hawk Down" star was arrested in L.A. late last night after an argument with a woman allegedly became physical.
Sizemore was taken into custody and booked for misdemeanor domestic violence. His bail was set at $20,000.
As of now, he's still in custody.
Tom is no stranger to domestic violence -- back in 2003 he was convicted of hitting then-girlfriend Heidi Fleiss.

Much like fruit picked straight off the tree, the best "Celebrity Rehab" patients are the ones straight from jail.
TMZ has learned Tom Sizemore, who was arrested early Thursday morning on an outstanding warrant, checked into Dr. Drew's house on Friday to begin taping the third season of the show.
Making the addition of Sizemore particularly interesting is that one of his fellow castmates is Heidi Fleiss -- Sizemore's ex-girlfriend whom he was convicted of assaulting back in 2003. Awk-ward!
We're told Fleiss didn't take too kindly to Sizemore appearing on the show, and she didn't come out of her room for sometime after he showed up.
Bad for Heidi, good for ratings.

Tom Sizemore is desperately trying to beat DMX's impressive arrest record -- dude just got popped again, and this time it's for having an outstanding warrant.
Law enforcement sources tell TMZ there was a call for a possible domestic violence disturbance in Hollywood at around 12:30 this morning. When cops showed up, Tom just happened to be in the area -- and it was discovered he had a $25,000 outstanding narcotics warrant in Bakersfield.
Sizemore and a buddy -- who was in possession of narcotics -- were both arrested and booked.
UPDATE: Tom actually made bail and has been released.

Congenital convict Tom Sizemore may be heading back to his old prison cell soon, because he's the alleged ring leader of one of the most pathetic heists in history ... one that was caught on tape.
The LAPD confirms Tom -- who spent time behind bars in '07 for a probation violation -- is being investigated for grand theft after he allegedly stole multiple cell phones, a pen and a highlighter from a Verizon Wireless store in L.A. last week.
Sources tell us Verizon's security cameras clearly see a man who looks a helluva lot like Tom, along with another man, stealing a handful of phones -- including one belonging to an employee. The man believed to be Tom was also busted reaching over the counter to pocket a pen and a highlighter.
Sizemore, who has multiple movies coming out in '09, just completed the term of his parole in January from a prior drug bust.
Tom's lawyer just told us he's "innocent until proven guilty."

One of the many stars fresh out of the pokey, Tom Sizemore, was seen enjoying some fresh air in Beverly Hills yesterday -- and was eager to share his opinion on prison, and how it has affected his life.
Tom should wise up -- his ex-flame Heidi Fleiss was just nabbed for her own list of allegedly bad behavior.

Wacktor Tom Sizemore has been released from Pleasant Valley State Prison, where he served time for violating probation on a 2004 methamphetamine conviction.
Sizemore was sentenced last June to 16 months in state prison, while he was already in custody. He was awarded 213 days credit for time he spent in county jail and residing in two live-in drug programs. He also got time off for good behavior. Tom became a free man on Dec. 10. Party time!
We've learned that Tom's other court case in Bakersfield has also been resolved. Sizemore was busted last May while high on crystal meth. He's now back in a drug program designed to help him stay clean.

Troubled actor Tom Sizemore was moved from L.A. County Jail to state prison, in order to start serving time for probation violation in relation to a 2004 methamphetamine conviction.

Could things look any worse for Black Hawk Down actor Tom Sizemore? Already in prison on a 16-month bid for violating probation in relation to an L.A. methamphetamine case, the seismically screwed Sizemore is once again in court. This time, he's trying to overturn a 2003 corporal injury conviction for an assault on his then-girlfriend, "Hollywood Madam" Heidi Fleiss.

A plastic surgeon testified that when he examined the former Hollywood Ho-Manager in the days after the supposed assault, he didn't see bruises on Fleiss' face. Surgeon Michael Cardenas testified that Fleiss and Sizemore seemed to be getting along.
Sadly, Sizemore faces up to six additional years behind bars if convicted of other pending drug-related charges.
The two felonious former lovebirds dated for two years.

A weeping Tom Sizemore appeared in a Los Angeles County Courtroom today where he admitted to violating his probation in a 2004 methamphetamine conviction.
Sizemore was brought into the courtroom in a jail-issued orange jumpsuit, wearing handcuffs, where he waited for a judge to hear the case. The judge asked Sizemore, "Do you agree you are in violation of your probation?" Tom replied, "Yes, your honor." The judge made her findings clear, telling the "Black Hawk Down" star, "Probation is a privilege ... Tom is not a matter of probation anymore because he has abused that privilege."
Sizemore openly wept in court, begging the judge, "I don't want to go to jail ... give me one more chance ... I have a terrible disease ... drugs have strapped me around the neck ... I guarantee I am not going to be here again! I want to rehabilitate myself and become a good productive citizen to society."
Tom's brother Paul and girlfriend were present in court for today's hearing. Sizemore was arrested last month in Bakersfield on suspicion of various drug charges, including methamphetamine possession. He is due back in court Monday, June 25 to hear his fate.

Home video maker and alleged meth enthusiast Tom Sizemore is driving his mother toward an early grave, say family members.
The man who rose to fame in films such as "Black Hawk Down" and "Saving Private Ryan," and sunk to shame in efforts like "Shooting Sizemore" and XXXTom.com, is currently in jail awaiting his next hearing ... and it's the wait that's the problem. Family members say that Tom's latest arrest has proven to be more than his elderly mother can take, and the stress is beginning to affect her health.
Tom is facing some serious jail time, and unlike other celebs who are in and out of jail in days or weeks, some insiders are predicting Sizemore's stint in the pokey will be measured in months ... or even years.
To make matters worse, rumors have been swirling about the release of yet another Tom Sizemore sex video (did anyone ask for the first one?), with even more kinky sex and drug abuse than the first installment. Those close to the project tell us that the new video will show a "whole new side of Tom that no one has seen before." Like clean and sober?

UPDATE: TMZ has learned that Sizemore's bail has been denied, so he will remain locked up until his next hearing on June 19.
Actor Tom Sizemore appeared at a Los Angeles courtroom this morning, expecting the judge to recall the warrant for his arrest. But things didn't go exactly as planned.
TMZ was inside the courtroom when the actor was unexpectedly taken into custody by the bailiff before the judge even arrived for Sizemore's probation violation hearing.
Sizemore's attorney had filed a motion to transfer courtrooms last week, claiming that Sizemore would not get a fair shake with the current judge. A judge obliged, and granted the motion.
Sizemore will remain behind bars until the new judge assigned to the case decides when she wants the hearing to begin. We're told that Judge Cynthia Rayvis could begin the case as soon as after lunch -- or set it for another day entirely.
