We caught this former nightclub manager back in 2010 when she was stepping out on Sunset Boulevard -- but not for too long, because she tripped and fell like a house of cards right in front of our cameras!
Good thing she had a friend to help her out after her tumble ... although maybe an extra hand from someone like Tiger Woods -- whom she was briefly involved with -- would have helped her up a little faster! Watch the clip for the reveal!
