Jeffrey Louis Starr -- a former child actor -- has died ... TMZ has confirmed with multiple family members.

Starr passed away last month ... with a family source telling us he took his last breath in a hospital in Carbondale, Illinois surrounded by loved ones. We're told he dealing with an illness before he died.

Jeffrey's brother, Kevin, and sister-in-law, Michele, posted on social media after he passed ... remembering Jeffrey as someone who gave great hugs, had a big heart and seemed to make and have friends at just about every place he walked into.

Tevin Hood -- a rapper from Georgia better known as T-Hood -- was shot at his home, and later died  ... TMZ has confirmed.

The Gwinnett County police officers responded to reports of a dispute that resulted in a shooting in unincorporated Snellville Friday ... and an investigation into the shooting is currently underway.

We're told first responders rendered aid at the scene and transported a man to the local area hospital where he ultimately died from his injuries.

Japanese boxer Shigetoshi Kotari has tragically passed away following a serious injury he sustained in a title fight just 1 week ago.

The World Boxing Organization (WBO) announced on X Kotari died on Friday after his August 2 match in Tokyo against Yamato Hata ... and remembered him as a "warrior in the ring" and a "fighter in spirit."

The super-featherweight underwent emergency brain surgery to treat a subdural hematoma following the fight, per a translated press release shared by the Japan Boxing Commission. A subdural hematoma is a type of bleeding that occurs between the brain and skull, per Cleveland Clinic, and can be fatal if not attended to.

Malcolm-Jamal Warner's mother is speaking out for the first time since her son drowned at a beach in Costa Rica.

Pamela Warner broke her silence Friday in a lengthy post on social media, saying ... "Needless to say our hearts are heavy with the loss of Malcolm."

Malcolm-Jamal's mom continued ... "He was a kind, loving man with a huge heart for humanity. He wanted all to be aware of their inner strength and that they could succeed, accomplish and transform. The District Six Medical Examiner in Largo, Fla., said on Friday that despite Brooke Hogan's wishes for a probe into her father's body ... it'll ultimately be Sky Daily's choice.

Savannah -- the daughter of Brandon's other ex-wife, Melissa Ashworth -- wrote she and Quentin had been "keeping a couple of sweet surprises."

Jon Miyahara -- the actor best known for playing Brett on "Superstore" -- has died, TMZ has learned.

His daughter Pennie Cappasola tells TMZ he died Wednesday with his wife, Chiyo, by his side. She says the family appreciates the outpouring of love from fans ... adding her father loved working on the NBC sitcom.

TMZ learned Jon died less than 2 weeks after paramedics responded to his Los Angeles-area home. Law enforcement sources say ... an EMS call was made around 7:15 PM on July 28 for an elderly man who had taken a fall the day prior and was feeling pain.

Brooke Hogan is now offering to pay for an autopsy for Hulk ... saying Thursday she believes there's still "speculation and uncertainty" surrounding her dad's death.

The wrestling legend's daughter extended the proposal on her Instagram page ... just a day after Hulk's wife, Sky, revealed they haven't yet cremated the icon "as our family is making sure every question about his medical care is answered first."

Brooke said she'd personally cut the check for any invoice a medical examiner required ... writing, "if that's what it takes to get it done."

Kelley Mack was feeling much better amid her cancer battle months ago ... but everything suddenly took a turn for the worse right before her death, TMZ has learned.

A family source tells us ... in February, "The Walking Dead'" star underwent treatment for glioma -- which is cancer of the nervous system. After her treatment, the source says Kelley experienced a "nice recovery."

The source says Kelley's cancer then relapsed out of nowhere, spreading through her body in recent weeks ... adding she had a "quick end."
