We spoke to people very familiar with the NYC facility where Wendy is living ... and they say residents on the 5th floor, where Wendy stays, are mostly in their 80s and up with severe cases of dementia or cognitive impairments.

That account matches Wendy's own description of her surroundings, which she shared in our new documentary, 'Saving Wendy.'

Teddi Mellencamp received some shocking health news Wednesday ... but, despite finding out her brain is covered in numerous tumors, it hasn't changed anything about her pending divorce.

Sources with direct knowledge tell TMZ ... the reality television star and her estranged husband, Edwin Arroyave, are still going through with their divorce despite her diagnosis.

The famous TV physician is "gravely" concerned about Diplo's strong affinity for LSD, and during a Wednesday interview with TMZ, he gave his analysis on how Diplo could land in a mental institution. Drew says, "It can happen. I have seen that from higher doses of LSD for sure," but on lower doses typically it can be managed on an outpatient basis.

And, Drew didn't stop there. The doctor says if Diplo's LSD intake is not managed by a professional, he might end up with a slow onset of mania or other types of neurotoxicity -- especially if he already has a mental health issue. The DJ has previously said he's struggled with depression.

Sources with direct knowledge tell TMZ ... Wendy signed an affidavit Wednesday afternoon asking the judge to set her free. According to the affidavit, Wendy says she has "regained capacity" to function without a guardian.

Wendy also says in the affidavit ... she does NOT have Frontotemporal Dementia, despite what doctors had claimed. Teddi Mellencamp's brain is covered in tumors ... and, doctors are working quickly to remove the biggest ones, she revealed Wednesday.

The reality television star shared the news in a simple black-and-white text post ... telling her millions of followers she's been out of commission the last few weeks due to a series of debilitating headaches.

Mellencamp was fighting through them until Tuesday, she says ... when the pain forced her to go to the hospital. Doctors then administered a CT scan and an MRI -- and, Teddi says they found multiple tumors on her brain, which her doctors think have been growing for months.

Wendy Williams is going to Miami this weekend for her dad's 94th birthday -- it's the first time she's been able to socialize in many many months -- and she'll be spending time with her son, whom she believes took advantage of her financially and triggered her guardianship.

R. Kelly lost his appeal across the board on his sex trafficking conviction ... but one judge thinks he was ordered to pay too much to one of his victims for name-brand herpes medication.

The disgraced R&B singer argued in his appeal to the U.S. Court of Appeals for the Second Circuit in New York that there was insufficient evidence, certain jurors should not have been empaneled, and counsel was ineffective while questioning potential jurors ... among other things, the docs state.

But after a full review, the appeals court affirmed the lower court's judgment, including an order requiring Kelly to pay restitution to a female victim for her herpes medication.
