Diddy received a 50-month prison sentence just a couple weeks ago ... and we now know the special conditions he'll be facing when he gets out and begins his 5 years of supervised release -- they're chock full of continued treatment for his issues.

The fallen mogul's special conditions of supervision were filed as part of the judgment in his criminal case Wednesday ... and, they include some pretty standard fare -- Diddy must submit to searches, and he can't contact the victims in this case for any reason.

DECISION MADE IN DIDDY'S SENTENCING

Diddy will have to participate in outpatient treatment programs for drug abuse and mental health issues ... where he will be required to take prescribed medicine, unless a health care provider instructs him otherwise.

Sean "Diddy" Combs had to break some tough news to his 2-year-old daughter after a judge sentenced him to 50 months in federal prison ... and he got emotional when he had to tell her that daddy wasn't coming home as soon as his family had hoped.

Sources close to the situation tell TMZ ... Diddy was talking to the mother of his youngest child, Dana Tran, on the phone after sentencing and she put their toddler daughter, Love, on the phone, and the kid asked her father when she could see him and where he was.

Diddy was at MDC Brooklyn ... and we're told he choked up when Love told him, "I miss you, Daddy." An emotional Diddy replied, "I miss you and love you very much, Love."

Diddy should serve the remainder of his 50-month sentence in a federal prison as close to the New York metropolitan area as possible ... at least according to the judge who imposed the sentence.

According to new legal docs, obtained by TMZ, Judge Arun Subramanian stops short of naming a specific prison where Diddy should do hard time and instead says Diddy should stay close to home.

Besides geography, the judge says Diddy should be shipped off to a prison with "any available substance abuse program, including the Bureau of Prisons' Residential Drug Abuse Program (RDAP), for which he qualifies."

10/7 12:41 PM PT -- Federal prosecutors just responded to Diddy's request to serve his sentence at FCI Fort Dix and they say they take "no position."

Diddy's got a preferred prison in mind for where he wants to serve out the remainder of his 50-month sentence ... and it's FCI Fort Dix.

Teny Geragos, one of the attorneys who represented Diddy in his criminal trial, fired off a letter, obtained by TMZ, Monday to Judge Arun Subramanian ... asking the court to recommend to the Bureau of Prisons that Diddy be shipped off to the low security federal correctional institution in New Jersey.

Diddy's legal team says FCI Fort Dix is the best landing spot for Diddy because there he can "address drug abuse issues" and "maximize family visitation and rehabilitative efforts."

Diddy's defense is back on the mic at his sentencing hearing, and they're claiming he almost got attacked with a makeshift knife inside the Brooklyn jail where he's been detained since his arrest.

Defense lawyer Brian Steel just said Diddy's got a target on his back at MDC Brooklyn, because there are certain people who would gain prestige if they harm him. He then claimed a guard at the jail stopped a man with a shank.

Diddy's been held at the detention center since he got busted in September 2024, and he lives in a room with 24 other men ... surrounded by drug use and violence. Steel said Diddy hasn't slept more than 2 hours in a row since being incarcerated, and only eats bags of chips.
