"Jackass: Best and Last" almost ended up being way more literal ... 'cause Steve-O says he came inches from death during a skydiving incident that was 100% his fault!

Steve-O spilled the story to our Towanda Robinson and Deven Rall on "The TMZ Podcast," telling us he was so determined to get the perfect mid-air kiss with his GF on camera, the pair couldn't get enough separation to deploy their parachutes safely.

Steve-O definitely looked like he was cringing while reliving the ordeal ... telling us he couldn't get stable for the pic, and with his GF locked close to him, they were burning through altitude fast.

Steve-O's got a lot of love for Bam Margera ... but says a little accountability would go a long way.

He sat down with our Towanda Robinson and Deven Rall on "The TMZ Podcast" Thursday to tell us how he feels about Bam's sobriety journey ... and what he wishes his 'Jackass' costar would say now that he's healthy.

As you know, Bam skipped Wednesday night's premiere of "Jackass: Best and Last" because of another commitment ... but also because he still wasn't ready to face Johnny Knoxville and director Jeff Tremaine since getting fired in 2020.
