'Ned's Declassified' alum Tylor Chase has begun the process of getting into a rehab facility after his former co-stars begged him to get mental health help and off the streets.

Jacob Harris -- the owner of Shipwreck Barbershop in Riverside, California, who has been a major advocate for Tylor -- tells TMZ ... he visited Tylor for 3 hours on Christmas day, a visit he calls a "tipping point." He adds ... "I had to do something. I finally found a crisis center that would come out and do an on-site evaluation."

Jacob explains the former child star was evaluated by the crisis center, and workers decided he needed to be taken in to receive "medical attention" at a Riverside-area hospital. We're told Tylor is now on a 72-hour hold and is being treated before being taken into a rehab facility. Jacob also spoke to Daily Mail, who first reported the news of his hospitalization.

"Ned's Declassified" alum Daniel Curtis Lee thought he was doing the best thing for his homeless fellow former child star Tylor Chase by getting him a motel room to get him off the streets ... but it sounds like Tylor tore the place up.

Daniel hopped on social media on Christmas Day and gave a heartbreaking update on Tylor ... he says motel management called him within hours of Tylor checking in to inform him the door to the room was wide open, the fridge was flipped over, and the microwave was in the tub.

As we reported, Daniel drove about 50 miles from Los Angeles to Riverside to get Tylor a motel ... ahead of a heavy rain storm moving through the area.

But Daniel, who starred on the Nickelodeon series with Tylor, says Tylor's family warned him that their experiences with putting Tylor in hotels were bad ones ... and he admits he was wrong to think this would be any different.

"Today I had a planned cervical spine surgery to relieve pressure on nerves in my neck that have been causing pain and neurological symptoms for a while," he wrote. "The procedure went exactly as expected, no surprises, no drama (for once 😅), just very precise work by an incredible medical team."

He added, "They removed bone and tissue that were compressing the nerves, giving my spinal cord, disks, and radiating nerves the space it needs to heal and function properly. Now comes 4 weeks of healing, rest, physical therapy, and being nicer to my body than I usually am. MERRY CHRISTMAS! 😜"

Jenna Haze is getting emergency medical care after posting alarming and bloody images of herself, which prompted an early morning visit from cops ... TMZ has learned.

Early Wednesday, the retired pornstar's IG stories were filled with photos and videos showing blood dripping from a wound on her hand. She also showed herself coughing and spitting up in a bathtub.

Jenna really set off alarms for her followers when, in one video, she accused an ex, saying, "Before I die, I want every single person to know the abuse I suffered."

L.A. County Sheriff's Dept. tells TMZ they got several calls to do a welfare check on Jenna, and deputies did go to her home just before 5 AM. We're told an ambulance took a female patient to the hospital.

Taylor Swift is honoring her father after his heart surgery by making a seven-figure gift to the American Heart Association.

The AHA announced Taylor's $1 million donation Tuesday, saying she "gave her gift in honor of her father, Scott Swift, whose heart condition she spoke about earlier this year."

TMZ broke the story ... Scott underwent quintuple bypass surgery this summer, and Taylor and her brother, Austin, were right by their dad's side throughout the surgery and recovery.

Daniel Curtis Lee, who starred on the Nickelodeon series with Tylor, shared with TMZ an emotional video of him checking Tylor into a Riverside, California motel Tuesday ... after driving about 50 miles from L.A. to meet up with his former costar, who's been living on the streets.

Daniel tells TMZ ... he spent part of the day with Tylor walking around and catching up over pizza before helping him settle into the motel so he could stay dry ahead of a major Southern California rainstorm this week.

Ryan Railsback -- Public Information Officer for Riverside Police Department -- tells TMZ ... officers are very familiar with Chase and interact with him at least once a week.

RPD has a specialized outreach division that regularly engages with unhoused individuals, offering mental health services, drug and alcohol treatment, and temporary shelter.

Dr. Jeffrey Lieberman -- formerly the president of the American Psychiatric Association -- stopped by "TMZ Live," sharing some disturbing insight into the multiple rehab facilities ... which ultimately failed Rob and Michele Reiner's son Nick.

Before his parents' tragic murders, Nick was in and out of rehab at least 18 times. While Nick struggled with substance abuse, Dr. Lieberman suggested the care he received was palliative at best ... and after-care at rehabilitation programs is generally inadequate, resulting in a revolving-door repetition of treatment and release.

Lieberman said, "[Rehab centers] don't necessarily provide mental healthcare for a primary diagnosis such as schizophrenia, which may have been the reason that impelled Nick to use illegal medications, which he became addicted to."

Barry Manilow shared a recent cancer diagnosis Monday, but he says not to worry ... doctors caught it so early, he won't even need chemo ... just a month of "chicken soup and 'I Love Lucy' reruns."

The 82-year-old Grammy winner posted ... "As many of you know I recently went through six weeks of bronchitis followed by a relapse of another five weeks. Even though I was over the bronchitis and back on stage at the Westgate Las Vegas, my wonderful doctor ordered an MRI just to make sure that everything was OK. The MRI discovered a cancerous spot on my left lung that needs to be removed. It’s pure luck (and a great doctor) that it was found so early. That’s the good news.”

According to Barry, the "bad news" is that his surgery to remove the spot will sideline him from performing in scheduled concerts for January.

Nick Reiner is expected to claim insanity in his parents' murder case ... and we have a resurfaced interview where he admitted to using the same tactic in the past as a ploy to get drugs.

While promoting his 2015 film "Being Charlie," Nick spoke on the addiction and recovery podcast "Dopey," spilling about how he got prescribed the antidepressant Wellbutrin.

Nick told host Dave Manheim that while he was a patient at Alina Lodge -- an addiction treatment center in New Jersey -- the staff refused to give him drugs. This is, of course, standard procedure for a sober clinic, but Nick wasn't taking "no" for an answer.

Jourdin Pauline says she's stepping back from social media because of relentless cyberbullying ... posting a concerning message that also accuses people online of talking disrespectfully about her late sister.

Here's the deal ... the singer and influencer has recently made waves online after she accused Tylil James -- a member of the content creation collective Clover House -- of sexual assault. James categorically denied the claims ... and suggested he has evidence which will disprove Pauline's allegations. Fans of his immediately jumped to his defense ... and Pauline says they've taken to attacking her online.

Pauline wrote in a since-deleted tweet that the online hate has impacted her mental health ... and she's getting off social platforms to get away from the "lies, threats, and dehumanizing comments" about her.
