A Walt Disney World guest suffered a heart emergency while riding It's a Small World ... and they were later pronounced dead ... TMZ has confirmed.

The death happened in April but it's just coming to light now ... the latest theme park injury report update from the Florida Department of Agriculture & Consumer Services says a 54-year-old man with a pre-existing condition experienced a cardiac emergency on the attraction back on April 24 ... and he was transported to a local hospital, where he died.

The Mouse House doesn't like to call attention to these tragedies ... which is why it took almost three months for anyone to catch wind.

The man who allegedly stormed NBC's "TODAY" show offices, lunged at Craig Melvin and called him the N-word is now facing a slew of hate crime charges ... TMZ has learned.

Law enforcement officials tell TMZ ... Andrew Truelove was arrested Thursday at 10:39 AM and booked on hate crime burglary, hate crime menacing, hate crime criminal trespass, and harassment charges. NBC News was the first to identify Truelove as the suspect.

We broke the story ... Truelove slipped past security and stormed NBC's Rockefeller Center headquarters Thursday morning, trying to find Al Roker before going after Craig.

The Houston driver -- Miracle Herron -- told us she was in the middle of completing her first delivery of the day when she was hit by a car involved in a police pursuit. The footage quickly went viral, drawing concern from viewers who were stunned she managed to get back up and complete the delivery after impact.

The man who allegedly stormed the "TODAY" show set and lunged at host Craig Melvin has been identified as Andrew Truelove ... and TMZ has learned he has a lengthy criminal history, including a felony conviction.

We broke the story ... a man allegedly slipped past security Thursday morning at NBC's Studio 1A and confronted Craig in a restricted area, lunging at him while yelling the n-word. The man was arrested and NBC News was the first to identify him as Truelove.

Court records, obtained by TMZ, show Truelove recently pleaded guilty to felony criminal mischief in a case involving damage to multiple vehicles. He was sentenced last month to six months behind bars and five years' probation, along with $375 in fees. It's unclear how much of that sentence Truelove served before Thursday's alleged run-in at "TODAY."

Georgia Grandma 2-Year-Old Grandson's Drowning Death Leads To Criminal Charges

A grandmother in Georgia is facing manslaughter and cruelty to children charges after her 2-year-old grandson drowned in a pool, police said.

Emelisa Paz-Santos was booked Wednesday in the Fulton County Jail on those two charges, while Immigration Customs Enforcement also put a detainer on her, according to official online records.

Roswell PD says 2-year-old Axel Delgado drowned in a clubhouse pool on the night of June 21. Police also say first responders raced to the scene and performed live-saving measures before transporting the child to a hospital where he died.

Karmelo Anthony Attorney Slams Trial Judge

A new judge has been assigned to oversee the ongoing proceedings in Karmelo Anthony's murder case ... as the convicted murderer's new lawyer calls foul on his trial judge.

The new judge, Sid L. Harle, was assigned to Karmelo's case Tuesday, per court records obtained by WFAA. Harle will preside over Karmelo's motion to recuse trial Judge John Roach from handling post-trial proceedings.

Karmelo's new attorney, Russell Wilson, is blasting the trial judge ... claiming Roach made comments following Karmelo's conviction that showed he could not be fair and unbiased.

Wilson also claims "gentleman's agreements" made between attorneys about what testimony would be admissible weren't completely followed, and that impacted Karmelo's decision not to testify and therefore impacted the outcome of the trial.
