Catherine O'Hara Cause of Death Released

Catherine O'Hara passed away from a blood clot in the lungs ... TMZ has learned.

The L.A. County Department of Public Health released the actress' death certificate Monday, which lists her cause of death … O'Hara's immediate cause of death is listed as pulmonary embolism, with rectal cancer as the underlying cause.

The death certificate says she was cremated and her remains were given to her husband, Robert "Bo" Welch.

We broke the story ... O'Hara passed away at the end of January, two sources with direct knowledge told us at the time. She had been rushed to the hospital in serious condition. She was reported by dispatch personnel as having trouble breathing.

Blake Garrett Former Child Actor Dead at 33

Blake Garrett -- best known for his role in "How to Eat Fried Worms" -- has died at age 33 ... TMZ has learned.

Blake's mom, Carol Garrett, tells TMZ ... the former child star passed away Sunday. She says the family is still awaiting the autopsy results from the medical examiner to determine an official cause of death.

She tells us Blake went to the emergency room last week in Oklahoma after experiencing intense pain and was later diagnosed with shingles. Carol adds Blake may have self-medicated to cope with the pain from the viral infection, and she believes his death could have been a tragic accident.

Puppy Bowl Beloved Dog Teigan Dead ... Months Before Show Aired

Teigan -- the adorable pooch from Puppy Bowl XXII -- tragically passed away months before the annual event aired on TVs across America ... with the Connecticut animal sanctuary Perfect Imperfections calling it "bittersweet to [see] her on TV."

The Team Fluff member contracted a severe case of pneumonia weeks after filming the 2026 edition of the Puppy Bowl, with the sanctuary saying Teigan was placed on oxygen and given IV fluids after X-Rays revealed the extent of the illness.

"I took her home and set everything up for her so she could continue getting her fluids and oxygen," the rescue center worker said. "I checked on her every two hours to change her position and flush her catheter. I started to get tail wags when I talked to her and for a moment I was hopeful she would beat this again, but knowing her little body had already suffered through this once."

A night of electronic music turned tragic in Australia after one man died and six others were hospitalized following medical emergencies at a major music festival.

New South Wales Police say officers were patrolling the Dreamstate music festival at Sydney Olympic Park Saturday when several attendees suddenly began suffering what authorities described as “medical episodes.”

A man in his 40s was pronounced dead at the scene, while six other festivalgoers were rushed to nearby hospitals. Police confirmed one person remains in critical condition.

MLB Star Terrance Gore Dead at 34

Terrance Gore -- the former MLB star who played for multiple World Series-winning teams -- has died.

A shared Instagram post between Gore's account and the Gap to Gap Baseball Academy confirmed the news Saturday ... with the academy noting the world "lost a tremendous man last night, but more importantly a family lost a dad, a husband, a brother."

The announcement revealed he died Friday evening. A cause of death has not been shared.

Multiple tributes have been posted since the news broke ... with the Kansas City Royals -- the team for which Gore played a majority of his games at the major league level -- and MLB also sharing their condolences.

Check out his March 2025 interview with the nonprofit organization 360 Baseline -- he looked thrilled to confirm Lil Jon was "always showing up" for him, even turning down gigs to be present for his birthday when he was a kid. He added that he never had to worry about his father falling into the stereotype of being an absent black father "because he was always there."

Nathan was pronounced dead Friday afternoon ... just days after he went missing from his home in Georgia. Authorities began the search for him Tuesday, and unfortunately, found him in a pond near his residence.

6:53 PM PT -- The Fulton County Medical Examiner tells TMZ ... Nathan's cause and manner of death are currently undetermined.

DJ Young Slade -- Lil Jon's son -- has died ... his father announced in a statement Friday.

Lil Jon tells TMZ ... "I am extremely heartbroken for the tragic loss of our son, Nathan Smith. His mother, Nicole Smith, and I are devastated. Nathan was the kindest human being you would ever meet. He was immensely caring, thoughtful, polite, passionate, and warmhearted -- he loved his family and the friends in his life to the fullest."

Jon said, "He was an amazingly talented young man; a music producer, an artist and engineer, and graduate of NYU. We loved Nathan with all of our hearts and are incredibly proud of him. He was loved and appreciated, and in our last times together we're comforted in knowing that we expressed that very sentiment to him."

Jon thanked the Milton Police Department for their help searching for his son.

Nathan was reported missing on Tuesday this week ... earlier today, searchers pulled a body from a pond in the area. Police said there was no indication of foul play. Milton, Georgia is about 30 miles north of Atlanta.

Nathan Smith was 27.

Police announced a body has been located in the search for DJ Young Slade -- Lil Jon's son who was reported missing in Georgia this week -- and they believe it's him ... TMZ has learned.

Milton Police said Cherokee County Fire Dept. personnel pulled a body from a pond where they'd been searching for Slade -- legal name Nathan Murray Smith -- Friday morning. That person is believed to be Nathan Smith, police said. The Fulton County Medical Examiner is working to confirm identification and cause of death. Police said there was no indication of foul play, but the investigation is ongoing.

As we told you ... the Milton Police Department put out a missing person alert for Nathan Tuesday after the department said he dashed from his home barefoot. Milton is about 30 miles north of Atlanta.

Cristina Pérez Galcenco Global Runway Model Dead At 21

Cristina Pérez Galcenco -- an international runway model -- has died at 21, according to media reports.

Galcenco's body was discovered at her house in Malaga, Spain on Tuesday, February 3 ... her cause of death appeared to be natural, several outlets reported.

Spanish newspaper ABC reports Galcenco had moved to the city of Malaga to enroll in a class at a school.

In the past few years, Galcenco had been strutting down catwalks from Madrid to Milan to Paris to London -- even locations in China.

Charles C. Stevenson Jr. 'Will & Grace' Bartender Dead at 95

Charles C. Stevenson Jr. -- the actor most famous for playing Smitty the bartender on "Will & Grace" -- has died.

He died of natural causes on Jan. 19 in Camarillo, California, his son Scott confirmed to TMZ. Variety was first to report the news.

Aside from playing the beloved barman, Stevenson was known for portraying preachers, priests, and pastors in supporting roles throughout his decades-long career.

'The Thing' Actor T.K. Carter Died From Heart Failure

T.K. Carter heart gave out ... according to the Los Angeles County medical examiner.

The M.E. lists the actor's cause of death as systolic heart failure. Atrial fibrillation and pulmonary hypertension were contributing factors.

We broke the story ... Carter -- best known for playing Nauls in the 1982 sci-fi horror flick "The Thing" -- was found dead in his Duarte, California residence last week after a call for service was made. No foul play is suspected.

Greg Olsen is mourning a heartbreaking loss … the former NFL star revealed his older brother, Chris, has died after a battle with brain cancer.

The longtime Panthers tight end and current FOX broadcaster shared the devastating news in an emotional Instagram tribute ... explaining Chris lost his fight with glioblastoma -- one of the most aggressive forms of cancer -- after putting up a relentless fight.

"Love you brother," Olsen wrote with a heart emoji.

His wife Mieko tells TMZ ... the icon died peacefully yesterday in his Las Vegas home, surrounded by family and friends. His cause of death was not released.

Gospel Artist Ron Kenoly Dead At 81

Ron Kenoly, a worship leader who maintained a multi-decade career as a gospel singer, died Tuesday morning.

The news about Kenoly's death was confirmed by his friend, musician Bruno Miranda, who shared a post about the late singer on Instagram not long after he died. Kenoly's cause of death wasn't publicly revealed.

Kenoly  was born and raised in Kansas, and he told the Orlando Sentinel he "grew up with an absentee father."

Kenoly briefly spent time in the Air Force before he moved to Los Angeles and began recording with an R&B group in the 1970s, although he eventually left the group, according to The Washington Post.

He eventually enrolled in a Bible college and began focusing on worship music. Kenoly cut his first album, "Jesus Is Alive," in 1991, and he went on to record numerous other albums over the next few decades.

Kenoly later founded Ron Kenoly Ministries, which was based in Orlando, Florida.

Kenoly was 81 years old.
