Kim, Kris, Kourtney and Kylie are heading back to L.A. after being at Lamar Odom's bedside since Tuesday ... but Khloe refuses to leave her estranged husband.

We're told Kim has an appointment with a pregnancy specialist who flew to LA just to meet with her. She's had an ongoing condition in which her placenta is growing too deep into her uterus.

They all left on a private jet.

Kendall, who's in London, is on her way to Vegas to be with Khloe.

The entire family plans to head back to Vegas this weekend to be with Lamar.

Lamar Odom's condition has deteriorated, and doctors are now telling the family the longer he remains in the condition he's in the less likely it is that he will recover.

Sources tell TMZ, Lamar did indeed squeeze Khloe's hand Wednesday night, which gave everyone hope, but Thursday morning doctors are saying it well could have been an involuntary movement.

As of Thursday morning, our sources say Lamar has shown no signs of consciousness or improvement. We're told 4 organs, including his kidneys, continue to fail. Our sources say doctors have been clear ... the fact that Lamar is not getting better significantly reduces his chances of survival.

As we reported, on Wednesday doctors gave Lamar a 50/50 chance of pulling through.

Scott Disick is back in rehab ... partly in hopes of winning a victory in his child custody battle.

Sources connected to Disick tell us he checked in last weekend to a facility in Malibu, and has not only admitted he has a problem ... but cut ties with some of his usual party pals.

We're told Disick took the leap because of his kids -- he's gunning for unsupervised visits. He's told friends Kourtney Kardashian won't let him have unsupervised visitation until she knows he's clean. Us Weekly first reported Scott's effort to get clean.

Disick made an abbreviated run at rehab earlier this year when he went to a center in Costa Rica that used hallucinogenic plants for treatment. He lasted a week before bailing.

Lamar Odom's father -- who's had numerous battles with Lamar -- just arrived at Sunrise Hospital in Las Vegas.

Joe Odom lives in the L.A. area, but arrived in Vegas Wednesday morning. He joins Khloe, Kim, and Kris Jenner on the short list of people who've been allowed access to see Lamar since he arrived at the hospital.

Lamar and Joe have been at odds for years -- over the Kardashians and Joe's own addiction issues -- but they eventually squashed their beef.

As TMZ reported ... Lamar has a 50/50 shot at surviving, we're told he's suffered several strokes and has multiple organs failing. Brain damage is a distinct possibility.

Lamar Odom's survival is right on the line.

Sources at the hospital tell us doctors are now saying Lamar has a 50/50 chance of surviving after enduring multiple medical problems ... the apparent result of complications from drug use.

Our sources say doctors are saying 4 of Lamar's organs, including his kidneys, are failing. As we reported, he's had a series of strokes in the hospital and has heart problems as well.

The doctors are saying with each failing organ the chances of survival go down significantly.

We're also told doctors are having trouble determining the extent of any brain damage because the organ issues make it difficult to test and get an accurate reading on brain function.

Lamar Odom had multiple strokes as a result of his medical emergency ... this according to witnesses at the hospital.

We're told an MRI revealed since being admitted to the hospital in Las Vegas Tuesday night, Lamar suffered multiple strokes. It's unclear if brain damage resulted from the strokes.

We're also told Lamar is on a 3-5 hour dialysis for kidney failure.

Our sources say Lamar remains unconscious and unresponsive, which is in contrast to what Jesse Jackson just told the media.

Jackson walked out of the hospital and said he spent 30 minutes with Lamar and his condition was improving and he had "signs of responsiveness."

Our sources inside the hospital say Jackson was allowed in for 3 minutes to pray with him, and that was it. The sources say Jackson is flat-out wrong about Lamar's condition.

Khloe, Kim and Kris are by Lamar Odom's hospital bedside but reality show cameras are not ... according to sources close to production and at the hospital.

TMZ has learned that "Keeping Up with the Kardashians" is on a hiatus, so there's no staff to shoot anything even if the family wanted it. We're told none of the Kardashians want their hospital vigil public ... in no small part because Khloe is devastated.

There are reports the family walked in with cameras rolling -- not true ... cameras did not accompany them.

To the contrary, we're told the Kardashians have banned any photography in eyeshot of Lamar.

Lamar Odom lost it Sunday ... and the trigger was an episode of "Keeping Up with the Kardashians."

Sources at Dennis Hof's Love Ranch South tell us, Lamar seemed normal during his stay until he got a call from someone about the show. Some of the women who were around Lamar say he became irate, complaining about the way he had just been portrayed on the show.

As for what was on the show ... Khloe got 2 calls from Lamar. When she got off the phone Kim criticized Khloe for being flirty with Lamar. In another clip, Lamar called Khloe to tell her that his good friend Jamie had died. When she got off the phone she worried Lamar might spin out of control. And in another scene Khloe fled a club after she heard Lamar was trying to get in.

We're told Lamar remained upset until he was found unconscious.

Up until getting the call, people at the Ranch say Lamar was "chill." There was no evidence he had been high on drugs or alcohol. We're told when he arrived Saturday, he ordered a single bottle of cognac and when he was found unconscious a third of the bottle was left.

As we reported, people at the Ranch say Lamar had been using an herbal substitute for Viagra. Someone at the Ranch who was around Lamar tells us he was popping 8 to 10 herbal substitutes for Viagra at a time. There are perils to the drug -- it can cause dangerously low blood pressure and it can restrict blood flow to vital organs.

Another source at the ranch says Lamar was taking the actual Viagra drug from a prescription bottle.

A visibly upset Kris Jenner tried wrapping her head around Lamar Odom's fight for life.

Kris and family hopped on a private jet 2 hours after TMZ Sports broke the news that Lamar was in dire shape.

We're told Khloe is distraught, sitting by Lamar's bedside. The Kardashians are the closest Lamar has to an actual family.

Kourtney Kardashian had a serious mom moment -- she looked away for a split second, and one of her kids took a car door to the face!

Kourt was actually holding 3-year-old Penelope's hand as they walked past a parked car in Malibu on Sunday -- and at that very moment, one of the car doors flung open.

Poor Penelope went down in a heap, and, naturally burst into tears. Mom and the nanny quickly comforted her ... and we're told she's okay.

Khloe Kardashian was head to toe in Adidas over the weekend, which makes sense given boyfriend James Harden's $200 million deal with the company ... but now it seems the entire clan is in on the act.

Kylie was out with Tyga Sunday in Calabasas, decked out in an Adidas tracksuit, and this isn't the first time they've been spotted giving the company prized promotion.

It would sure make sense. Fact is, they might be better pitch people than Harden.

Kanye West, the same guy who went on an EPIC anti-award show rant thinks the entire Kardashian clan deserves multiple Emmys for being pioneers in the reality show world.

In an interview with SHOWstudio, Yeezy relates Caitlyn's public transition and how new it was to society to rappers Run-D.M.C. and Will Smith. He says rappers weren't accepted into high class society at first because it was a new concept -- just like Caitlyn's transition.

He then goes on about "Keeping up with the Kardashians." The good stuff starts around 1:02:21 ... Kanye says the reality show was such a ground-breaking "art form" that it deserves multiple Emmys for starting a whole new genre.

Kylie Jenner is all about the family biz, which apparently now requires all the sisters to rep HARD for Adidas ... at all times.

Ever since big sis Khloe Kardashian's BF James Harden signed a $200 million dollar deal with Adidas -- Khloe AND Kylie have been sporting the gear constantly. It also can't hurt that the company cranks out Kanye West's Yeezys too.

Sooo ... Adidas: outfitting hoops superstars, NCAA teams, rappers and Kardashian asses everywhere.

The Kardashian family did everything right during the midnight fireworks show in August that pissed off tons of LA locals ... and according to the D.A., the people to blame are in law enforcement.

The D.A. says the family will not be prosecuted for disturbing the peace, they had been given permission to do it from the Coast Guard and cops. There was even an LA fire captain there that night to make sure everything went according to plan.

While the D.A. does say the fireworks caused a "community inconvenience," they say all the right parties in law enforcement were notified before the event and still chose to do nothing.

Bust out the M80s -- Kris Jenner and Khloe Kardashian can celebrate dodging criminal charges for that massive fireworks display they set off in the middle of the night ... terrifying beach residents.

Law enforcement sources tell us the L.A. County Sheriff's Department wrapped its investigation into the explosive yacht party Khloe threw for James Harden's birthday early in the morning of August 26th.

We're told the Sheriff's Dept. presented its findings to the LA. County District Attorney, and recommended a misdemeanor charge of disturbing the peace -- but the D.A. rejected it because the Kardashians had "passive permission."

We broke the story ... the Coast Guard gave the Sheriff a heads up that Kris' production co. would be lighting up the sky that night, and didn't need a permit since the boat was outside the marina. Still, angry residents wanted the Sheriff to bust them for violating noise ordinances.

Had they prosecuted, it's unclear who would take the fall -- Kris, Khloe, or the fireworks vendor -- but it's a moot point now ... criminally, anyway.

We're told a group of residents is still pushing forward with a small claims lawsuit.

The Kardashian matriarch is all over Fashion Week ... although it's hard to spot her in the considerable shadow of her mammoth new bodyguard, who is a dead ringer for a modern day Thor.

Sure, Kris is only 5'6" ... but her slab of beef-for-hire looks to be at least THREE heads taller than her. Warning: Approach the momager with extreme care.
