Dan Rivera -- the paranormal investigator who worked for the Annabelle haunted doll tour -- told people around him he wasn't feeling well just hours before his untimely death in Pennsylvania ... TMZ has learned.

Adams County Coroner Francis Dutrow tells us ... Dan was surrounded by coworkers Sunday morning -- but, they parted ways when he told them he was sick and needed to go back to his room to rest.

We're told these coworkers hadn't heard from him in a while, so they went to check on him ... which is when they found him dead.

NFL legend Jim Kelly's family is celebrating a much-needed win on Wednesday ... with his grandson undergoing a successful procedure to address "unexpected medical complications" following his birth.

The legendary Buffalo Bills signal-caller shared the news on Instagram ... saying the Kellys are "overwhelmed with gratitude for God's hand in every detail today."

"For highly skilled and compassionate doctors and nurses ... and for every single prayer that's been lifted on our behalf."

Famous bodybuilder Victor Martinez -- who won the 2007 Arnold Classic -- underwent emergency surgery to combat an undisclosed medical issue, it was announced this week.

The shocking news regarding the 51-year-old fitness star was revealed to Generation Iron, the same outlet that hosts Martinez's podcast.

The type of procedure was not disclosed ... but it is apparently so serious he required immediate attention.

The "haunted" doll Annabelle will continue touring the United States following the sudden death of paranormal investigator Dan Rivera ... who was on tour with the infamous doll when he was found dead in his Pennsylvania hotel room Sunday night.

A press release from the New England Society for Psychic Research, given to TMZ, says although they "do not know what the future of NESPR will look like without Dan, we do plan to move forward with the events previously scheduled for this year."

Dan died in Gettysburg after bringing Annabelle -- who Lorraine and Ed Warren have said has a "demonic" presence attached to her -- to a sold-out crowd at the ongoing "Devils on the Run" national tour over the weekend.

Taylor Swift's dad had to go under the knife for a heart procedure ... and she was by his side in the hospital ... but now he's feeling fantastic, TMZ has learned.

The singer's spokesperson tells TMZ ... Scott Swift had quintuple bypass surgery over a month ago and his wife, Andrea, and their kids, Taylor and Austin, were with him through the entire surgery and recovery process. Taylor's rep also told us the procedure was NOT the result of a heart attack, and he's feeling just great.

Scott -- who's been married to Taylor's mom Andrea for 37 years -- originally went in for a check-up and his doctor saw something, which led to the surgery.

Randy Couture is on the mend ... TMZ Sports has learned he's been discharged from the hospital following his scary crash last week and is now at home recovering.

The UFC legend sustained serious injuries after he wrecked his car during practice runs at a racetrack in Kansas City ... and actually needed to be airlifted to a medical facility to get treatment.

We're told he suffered first and second-degree burns in the accident ... and had trauma injuries as well as smoke inhalation.

Ronnie Coleman believes he probably would have died if it wasn't for his 13-year-old daughter ... opening up on his near-death sepsis battle in a positive update for his fans.

The legendary bodybuilder posted the clip on Tuesday ... weeks after he was hospitalized with the serious medical condition that required emergency surgery and treatment.

"Man, let me tell you, these past few weeks have been some of the toughest of my life," Coleman said. "Ya boy was hit with sepsis, and it nearly killed me."

Diddy is already attempting to rehabilitate himself -- he's enrolling in self-improvement programs while in federal lockup to address his drug use and violence against women.

Sources close to the situation tell TMZ ... Diddy is in the STOP Program, a course the feds offer inmates focused on preventing sexual assault, domestic and dating violence.

We're also told he is undergoing therapy, and is in the Dr. Harry K. Wexler drug abuse program.

Seth Rollins' knee injury might be as bad as it looked ... as Triple H provided an update on the WWE Superstar's status on Sunday -- and it's not giving much hope for fans.

Rollins suffered the ailment Saturday night in Atlanta during his match with LA Knight ... and roughly 24 hours later, the promotion's COO admitted it "doesn't look good."

Triple H opened up on the incident while meeting with the media after Sunday's all-women's "Evolution" event ... and he said early tests were inconclusive about the full extent of the injury.

"Empire" star Bryshere Gray needs mental help more than a jail sentence -- this, according to his pal Dustin "Chosen" Wilkins ... who filed a criminal complaint against him in June after the pair got into a physical altercation at a Virginia hotel.

Chosen tells TMZ he only went forward with the report because the actor is suffering from several mental health conditions that were exacerbated when he stopped taking his medications -- and was drinking on top of that -- and believed pressing charges could jumpstart getting him into treatment.

Production sources confirm to TMZ the pair were spending time together in Virginia as they prepared to work on "The Starboys" ... but Bryshere is no longer associated with the project. quite responsible when it comes to taking his meds and monitoring his alcohol intake.

However, Chosen adds Bryshere is easily sidetracked by people who don't have his best interest in mind.

Chosen describes Bryshere -- who we learned was dropped by his pubic relations agency this week -- as his "little brother" ... which is why he cares so much about what happens with him over the assault and battery charge.

Chosen tells us he would gladly have the charge dropped if Bryshere would commit to a mental health treatment plan and focus on his wellbeing.

He says he worries about the possibility of BG heading to the clink -- and even admits he regrets filing the complaint.

We broke the news -- Chosen filed a misdemeanor complaint against the actor and rapper on June 13 by bypassing police and directly working with the Chesapeake magistrate's office. A warrant for his arrest was issued ... and Bryshere was taken into custody on June 15.

Plot twist -- Chosen is the one who bonded him out within hours.

Oh -- and he wants to make it crystal clear that their quarrel did not erupt over a "payment dispute" as he wrote in his complaint. He says the situation quickly got ugly due to the state of Bryshere's mental health, and that's that.

Production sources confirm to TMZ the pair were spending time together in Virginia as they prepared to work on "The Starboy" ... but Bryshere is no longer associated with the project.

It's not clear if the "Canal Street" actor will return to the small screen anytime soon, as Chosen says he previously confessed to him he wants out of the entertainment industry because it's been so hard on him.

Chosen says he and BG do not currently aren't in contact due to the "no contact" order in place. Court documents show Chosen appeared at the Chesapeake General District Court on Monday to amend the bond provision so Bryshere could have contact with him ... but was denied.

Bryshere is due back in court at the end of the month.

We reached out to Bryshere's reps for comment ... and they say they're no longer working with him due to the state of his mental health and concerns over this legal representation.

They explain ... "Our company integrity prevents us from participating in any activity that places Mr. Gray at risk. We are not familiar with the legal representation handling Mr. Gray's criminal case in Virginia. Their representation of Mr. Gray makes us very uncomfortable, and we hope for the best outcome for Mr. Gray."
