Dr. Jan Adams physically assaulted his ex-wife, according to court documents obtained by TMZ.

TMZ has obtained a restraining order filed by Susie Field, former wife of music mogul turned movie producer Ted Field. According to the document, Field claims on Thanksgiving Day, November 23, 2006, Dr. Adams became upset with Susie for taking a phone call during dinner. Susie claims, "He wanted to speak to whomever was calling me."

They began arguing and went up to the bedroom. According to the document, Dr. Adams was yelling, "I'm going to f**king kill you. I'm going to beat you. You need to be beaten." Susie claims Dr. Adams then "swung his right closed fist at my face...he attacked me," adding, [Dr. Adams] grabbed me with one arm and threw me toward the bed. I struck the bed and fell onto the floor."

Susie claims there was another incident as well. In September, 2005, she says they were at a New York nightclub, where he became angry because another man was looking at her. According to the document, when they returned to the hotel, he became "angry, threw me on the bed and began to strangle me. I thought I was going to die as he strangled me to the point of almost blacking out."

The judge granted the restraining order on November 28, 2006. Two days later, Susie filed for divorce. They were married in early 2006 and the divorce was final in February, 2007.

BSC would not confirm that Donda West had her surgery there, however, TMZ spoke with an employee who told us that Dr. Adams was not in today (surprise) and she didn't know when he'd be back. Wait -- there's something wrong with the neon sign!

Dr. Jan Adams told TMZ Monday night he had done nothing wrong and that Kanye West's mother likely succumbed to a heart attack, pulmonary embolism, or from massive vomiting after returning home from surgery.

In a lawsuit filed in Los Angeles County Superior Court, Teana Lucas claims she was a bartender and Dr. Adams was a customer. She says he agreed to perform liposuction on her upper arms and abdomen. The plaintiff, an aspiring model and actress, alleged as the procedure was about to begin, Dr. Adams "whispered in plaintiff's ear that he 'loved' her, that he was 'going to marry her."

According to the suit, after the procedure, he started making house calls to her home. On November 24, 2004, the suit alleges Dr. Adams told her she should get dressed and that he was taking her out for dinner. After three glasses of wine, the heavily-medicated plaintiff claims she was "heavily intoxicated." That's when, according to the suit, Dr. Adams took her to his house and had sexual intercourse with her.

Lucas claims she became pregnant and Dr. Adams wanted her to keep the baby but she didn't. Eventually, she says Dr. Adams referred her to an OB/GYN who could "take care of the problem."

Lucas also claims he botched her two surgeries. She sued for sexual battery and malpractice.

Adams denied all the allegations, but settled the case.

As we all now know, Kanye West's mother, Dr. Donda West, died a day after going under Adams' knife for a tummy tuck and breast reduction.

TMZ has obtained yet another malpractice lawsuit against Dr. Jan Adams, in which a patient claims she was deceived by Dr. Adams and Euclid Outpatient Surgery Center, because no one told her about all the malpractice claims against the doctor.

In a suit filed in Orange County Superior Court, Jana Beighle claims she went in for "a left breast capsulectomy." She says Adams was her doctor, and she was never told he "had an undisclosed history of medical malpractice claims and judgments."

The suit claims after the surgery in August, 2005, Beighle suffered from fluid collection and had to have her implants removed.

Tracy Morgan didn't wear his alcohol monitoring bracelet for the full 90 days, and now he claims that a medical issue is the reason.

TMZ has learned that Morgan's attorney, Blair Berk, appeared in a Los Angeles County courtroom today and explained that Morgan took off his SCRAM bracelet early because of an undisclosed medical condition.

Berk will present specifics to the court on October 30.

TMZ has learned David Hasselhoff was admitted to the hospital for alcohol poisoning.

Sources tell us David checked himself into Cedars-Sinai Hospital in L.A. after a binge. It was a stark contrast from what we saw last Thursday, when Hoff had a night out with daughter Hayley at Koi restaurant. Hasselhoff looked great. But things changed dramatically afterward.

No word on how the latest lapse will affect custody. A judge awarded physical custody of the kids to Hoff, but we're told for the last five weeks they've been living pretty much full-time with David's ex, Pamela Bach, while he was in Europe shooting "Anaconda 3." We're told he had "infrequent contact" with his daughters, something Hoff's people dispute. We're told Hoff has tried to keep contact at a minimum with Pam while the parties try to hammer out a property settlement.

As for how Hoff is doing, his rep says he checked out of the hospital and he's fine.

David and Pam are due back in mediation tomorrow.

TMZ has confirmed that David Hasselhoff has been hospitalized, as first reported by the National Enquirer.

Rep Judy Katz tells us that the Hoff did have a relapse and is in the hospital, but is doing fine. In an exclusive statement, Katz tells us "David had a brief relapse and immediately recognized the importance of addressing it with the assistance of his doctors. He is doing fine and will be back home in the morning".

As for how this will affect his custody case, sources say the children had been in ex-wife Pamela Bach's care for the last six weeks as he's been out of the country in Romania shooting "Anaconda 3."

We're told mediation is scheduled for Thursday.

We last saw the Hoff last Thursday night, outside of Koi with one of his daughters.

TMZ has learned Lindsay Lohan has checked out of Cirque Lodge, the residential rehab facility she has been in since August, but rehab is still very much a part of her life.

Sources connected to the actress tell us she has moved to a non-residential program to continue her treatment at the Utah facility, which will include drug testing and various meetings with professionals.

While several "Dancing With the Stars" contestants are limited in one way or another (Mel B. just had a baby, Wayne Newton has been stretched to the limit for some time), it's doubtful any of them have endured what Mark Cuban has.

The outspoken Dallas Mavericks owner had his hip replaced less than seven weeks before he started practicing for "Dancing With the Stars." Ay caramba!

"Hell's Kitchen" host Gordon Ramsay apparently got his twig and berries a little too close to an open flame and burned his nether region, several British tabs are reporting.

Gordon was quoted by The Sun as explaining, "The other day I was standing too close to the stove when I was cooking. I was wearing these cotton trousers and underneath I was going commando, suddenly, I felt this searing heat. I went, 'F**k me, bollocks to that.'" Bollocks, indeed. Ramsay painted an even clearer horrible mental picture by saying, "I burnt my right bollock and I'm in absolute agony ... You wouldn't believe how much this hurts." Not hard to imagine it, really.

Ramsay's rep confirmed that the chef did in fact burn himself, but wouldn't confirm where on his body the burn occurred.

A "call to service" report from the Santa Monica Police Department, obtained by the television show "EXTRA," shows that the 911 call from Owen Wilson's home was made because the actor tried to commit suicide.

The call was received shortly after noon on Sunday and was "cleared" some five hours later. "EXTRA" is reporting that it was one of Owen's brothers who found him. Owen was rushed to St. John's Hospital in Santa Monica and later moved to Cedars-Sinai Medical Center in Los Angeles.

TMZ was outside of Cedars Sinai this afternoon as Samuel walked in to visit Wilson, and he was clearly taken aback by paparazzi presence.

TMZ has obtained the following statement from Owen Wilson:

"I respectfully ask that the media allow me to receive care and heal in private during this difficult time."

Andrew Wilson, Owen and Luke's older brother, was spotted by TMZ cameras going into Cedars Sinai Hospital in Los Angeles late this morning. Andrew ignored questions about his brother's condition.
