Five Finger Death Punch singer Ivan Moody is running out of time -- his estranged wife says he's having bizarre Jekyll & Hyde-like episodes and needs to get help for alcohol abuse, stat.

Moody's estranged wife, Holly Smith, filed an affidavit with the Adams County Court in Colorado ... saying Ivan can no longer tell the difference between his real persona and his onstage one ... thanks to his alcohol abuse.

In the docs, Smith says Ivan's taking on the personality of the band's song, "Jekyll and Hyde." She says, "Ivan has talked to me in third person in a voice different from his own as if he were a demonic presence inhabiting a body."

Ivan's sister, Sandra Dykes, filed an affidavit of her own as well backing up many of Holly's claims.

Holly and Sandra think he needs to get into rehab immediately. In the meantime, the judge granted Holly a restraining order against Ivan after she showed photos of bruises he allegedly gave her.

A woman suffered the heinous blow during the band's recent show in Walla Walla, Washington, during the encore. The band was jamming pretty hard (as hard as they can) to "Ditmas" when ... for some reason ... Marcus Mumford chucked the metal rod about 10-15 rows deep

According to witnesses, she was clearly hurt and the crowd tried to get the band to stop playing so she could get medical attention. But M&S kept going, and so did she ... sticking it out for the rest of the show.

It's unclear how badly she was injured. Reps for the band haven't gotten back to us yet.

Ironically ... the tour's called Gentlemen of the Road.

Things have gone from bad to worse for Italian cyclist Vittorio Brumotti ... TMZ Sports has learned the brutal roadside attack he suffered might cost him one of his eyes.

As we previously reported ... Brumotti was the victim of a heinous roadside ambush while on a ride in Northern Italy earlier this week.

Doctors are set to operate Monday to repair injuries from the attack ... and doctors have told Brumotti the damage is so severe, they may have to remove his right eye.

Brumotti posted a graphic picture of his face shortly after the incident which clearly showed the extent of the damage.

Former Chicago Bears quarterback Erik Kramer shot himself Tuesday night in an attempted suicide ... but survived ... and is currently recovering in an L.A. hospital.

Kramer -- who played 13 seasons in the NFL -- was discovered by law enforcement in a motel in Calabasas around 8 PM, according to NBC. The extent of his injury is unclear.

The 50-year-old's ex-wife Marshawn told NBC News Kramer has suffered from depression for years ... which she believes is a direct result of the hits he took during his time in the league.

"He is a very amazing man, a beautiful soul, but he has suffered depression since he was with the Bears. I can promise you he is not the same man I married."

1:55 PM PST: A rep for the firm tells TMZ: "Tanner Mainstain is saddened to learn that Ms. Doherty is suffering from cancer and wishes her a full recovery. However, the claim that Tanner Mainstain caused her to be uninsured, prevented her from seeking medical care, or somehow contributed to her cancer is patently false."

They also say they will "aggressively defend" all of Shannen's claims in court.

Shannen Doherty says her business manager is responsible for the severity of her breast cancer.

Shannen has filed a lawsuit, obtained by TMZ, in which she claims Tanner, Mainstain, Glynn & Johnson took 5% of her earnings in return for managing her finances, including payment of her medical insurance to SAG.

She says the firm ignored an invoice for payment of the premium in 2014. The insurance was cancelled and apparently she couldn't re-enroll until 2015. Shannen says during the lapse period she didn't regularly visit doctors, which had been her practice.

In March 2015, after getting her insurance back, she went to a doctor and discovered she had "invasive breast cancer metastatic to at least one lymph node." She says doctors told her the cancer spread during 2014, when she was uninsured, and she blames her business manager.

She indicates in the lawsuit she now will likely have to take measures -- including a mastectomy and chemotherapy -- that could have been avoided if the cancer had been caught earlier.

The docs say she parted ways with the business manager sometime in 2014.

Famous Italian cyclist Vittorio Brumotti was viciously attacked by 3 men during a ride -- and although the suspects escaped, he's trying to downplay the whole brutal incident.

Brumotti suffered nasty facial injuries during the attack which went down while he was cycling in Northern Italy. According to reports, the attackers were identified as Albanians ... something Brumotti considers a sensitive issue.

After posting a pic of his battered and bruised face, and made a plea for fans to avoid turning this into a reason to hate foreigners.

One of Vittorio's friends shot video of the aftermath, and you can see he's totally out of it. Vittorio says he's going to need surgery.

Brumotti is a serious badass in the cycling world ... who holds several Guinness Book records.

Republican presidential candidate Marco Rubio is losing ground in the polls -- and this video won't help ... not with the diehard football fans.

Rubio was tossing the pigskin with some kids in Iowa -- where every other candidate is hanging this week -- and let's just say he needs to work on his spiral ... or get Odell Beckham Jr. as his running mate.

He made it near impossible for the poor kid going out for the pass -- although, in Rubio's defense ... it did hit him in the hands. Awkwardly.

Cameron Dallas was greeted Tuesday at LAX by an absolutely INSANE mob of his fans and one of them got a little more than she bargained for ... when his driver clipped her foot as he drove away.

The girl was just fine and probably thought the pain was worth it ... since Cameron hopped out of the car to check if she was okay and gave her a huge hug.

The Vine star even reached out to her later on social media. We're told Dallas also sent her flowers and offered to pay any medical expenses.

"Deadliest Catch" star Tony Lara didn't suffer when he died ... authorities believe he suffered a heart attack in his sleep.

Meade County Sheriff, Ron Merwin, tells TMZ ... the pathologist who performed the autopsy observed scar tissue on Lara's heart, which is consistent with the effects of a heart attack.

Merwin says they'll know for sure when the toxicology report comes back, but there were no signs drugs or alcohol played a role. We're told Lara's body is being transferred back to his hometown in Alaska.

Joe Jackson has finally returned to the U.S. after his birthday trip to Brazil was sidetracked with 3 heart attacks and a stroke ... and another incident right before he was supposed to leave.

Jackson was spotted Tuesday morning at LAX where he was being wheeled around.

The 87-year-old was released from the hospital last week, but suffered a fall, delaying his return.

Wendy Williams took the name of her concert -- "The Sit Down Tour" -- to heart, when she took a big fall, but recovered like a pro.

Wendy was at The Hobby Center in Houston Saturday night when she apparently tried getting off stage but misjudged the stairs.

The full name of the tour "The Sit Down Tour ... Too Real for Standup."

Wendy brushed it off, and the show continued.

Annie Lennox's daughter is ok after a kayaking accident in upstate New York, but her long-term partner is still missing.

The 22-year-old model turned artist Tali Lennox and her boyfriend, 32-year-old Ian Jones, apparently fell off their kayak Saturday morning. Authorities say Tali was rescued by a boat that happened to be nearby, but there was no sign of Ian.

Ian reportedly was not wearing a life preserver. Capt. John Watterson from the Dutchess County Sheriff's Office reportedly said, "We believe that alcohol may have played a role."

Tali was a very successful model, fronting products for Topshop and Prada, before becoming a portrait painter last year.

Yolanda Foster thinks her leaky breast implants were making her preexisting medical condition debilitating, so she's had them removed from her body ... and it changed her life.

According to our sources, the 'Real Housewives of Beverly Hills' star had both implants removed this week. Foster says her doctors discovered one of them was leaking silicone, and they immediately realized it was exacerbating the symptoms of her Lyme disease.

Foster was diagnosed with Lyme disease in 2012 and her condition became so serious earlier this year, she lost the ability to read, write or even watch TV.

But doctors think Yolanda's condition will become way more manageable ... it won't cure her, but it will help keep the symptoms in check.

We're told she's already feeling a lot better.

Kelly Ripa and Mark Consuelos are now sporting matching footwear ... unfortunately that footwear is a walking boot.

Ripa previously broke her foot in a gym class and now Consuelos has joined her on the IR ... after tearing a calf muscle.

Jose Canseco's gonna be a daddy again -- if he can just sort out some fertility issues that might be tied to his past steroid use ... according to his wife, Leila Knight.

Leila was leaving Katsuya in Hollywood and decided to break the news -- she and Jose are trying to get knocked up ... with TWINS! Leila says they're going through in vitro right now, and they're even removing those pesky 'Y' chromosomes to ensure they get girls, 'cause she doesn't want carbon copies of Jose and his twin bro. Gunfire rang out Monday night outside a concert featuring J. Cole and Big Sean in Holmdel, New Jersey, and it may have been triggered by the victims trash talking a woman.

According to the NY Daily News, a witness claims the two victims had been talking to a woman in the venue's parking lot, "talking about her body," when a guy came over and began firing.

The men are both in a hospital in critical condition. The shooter got away.
