The Times Union of Albany cites information from "several cooperating witnesses" in the investigation, saying, "Blige and other stars were shipped prescribed human growth hormone or steroids -- sometimes under fictitious names -- at hotels, productions studios, private residences, an upscale Manhattan fitness club and through the Long Island office of Michael Diamond, a chiropractor affiliated with the celebrities." Pump it up!

But the article also states, "Law enforcement officials have said they have no evidence in their sprawling multi-state probe that customers, including Blige or other entertainers, violated any laws."

Reps for Tyler Perry had no comment. No other reps have responded.

TMZ has just received the final coroner's report on the death of Donda West. The L.A. County Coroner has concluded that Kanye's mom died of "coronary artery disease and multiple post-operative factors due to or as a consequence of liposuction and mammoplasty." Plainly put, she had a heart attack after the surgery.

The report indicates her final manner of death "could not be determined."

The coroner says several things could have played a role in her death, but they could not say for sure what exactly killed her, adding there was no evidence that anesthesia -- or even the surgery -- caused or contributed to her death.

We've reported extensively that Donda West had cosmetic surgery the day before her death -- performed by Dr. Jan Adams, who was not board certified in plastic surgery. We know another doctor refused to operate on West before Adams came into the picture. The other doc had said she had a medical condition that made her a candidate for a heart attack.

The Coroner's report narrative states that Stephen Scoggins (Donda's nephew), a nurse with an advanced degree in Public Health, was called by a friend of Donda to go to the surgery center where the procedures were performed, because Ms. West was not "waking up soon enough."

West died on November 10. We're told her family has been told of the results, but her family declined to release a statement.

A citation has been issued against Cedars-Sinai for a massive medical mix-up that injured three newborns, including the newborn twins of actor Dennis Quaid.

The L.A. Times is reporting the California Department of Public Health found the hospital failed to "adhere to established policies and procedures for safe medication use ... These violations caused, or were likely to cause, serious injury or death to the patients who received the wrong medication."

The twins recovered and are home.

TMZ has learned family members and professionals in Britney's life are working as a team to get her mental help -- either voluntarily or involuntarily -- for treatment of what appears to be a severe bipolar disorder.

We're told it's going to happen one way or the other, and fast. So far, Britney refuses to commit herself voluntarily, but the team is persisting.

Sources say they are considering "a number of options," some of which are "creative."

The most extreme option -- going to court and getting an order forcing Britney into in-patient treatment. That would be the last ditch effort.

If involuntary commitment is the only option, Brit must be picked up by cops -- again -- on a "5150," meaning she's a danger to herself or others. Then, while she's in the hospital, health care professionals can go to the County Public Guardian, who can then go to court and ask for what's called a "conservatorship." The conservator would then basically assume control of Brit's life.

TMZ has learned Eminem was rushed to the hospital over the holidays.

Hospital sources tell TMZ Em was taken to a Detroit-area hospital for a serious heart condition and severe pneumonia. Sources tell TMZ Em's weight has ballooned to 212 lbs.

After a nasty fall during a Vegas concert Tuesday night, Emma Bunton sprained her ankle and has been walking around all week on crutches (and heels, natch) -- not exactly the perfect on-stage accessory for the Girls' killer Roberto Cavalli costumes.

We're told she is still expected to perform at the next show this Saturday in London. Say you'll be there!

Liza Minnelli collapsed during a gig in Sweden today, and has quickly been flown back to the States for treatment, according to reports.

Minnelli was singing in a Christmas show in Goteborg, says the AP, when she collapsed while walking offstage. "As she walked down the steps, she passed out," said a rep for the Swedish concert organizers. "She was immediately taken to her hotel, where a doctor was waiting."

The 61-year-old songstress was then flown back to the U.S. No cause for the collapse was immediately given.

Rapper Bow Wow (he's not lil' anymore, people!) was rushed to a Cincinnati hospital Thursday night after a performance there.

According to a statement issued by his record label, the 20-year-old is suffering from "stress and exhaustion." They also said that although his tour with Chris Brown, Sean Kingston and Soulja Boy is heading to Chicago tonight, Bow Wow won't be on stage.

A chartered bus packed with Dr. Phil fans crashed in Hollywood today, after the driver says the brakes failed on the runaway vehicle. Get out of the way!

A spokesman for the L.A. City Fire Department says 19 people were taken to the hospital as a precaution, but there were no serious injuries.

A rep for "Dr. Phil" says the show chartered the bus to take fans to a taping of the show -- they never got there. All is not lost, however, as the rep said the show's producers planned to invite those on the bus back for lunch and to watch the taping of their 1,000th episode. "We're grateful that people are all right, and we hope that their next trip back to Dr. Phil will be less eventful and much more fun," the rep said. "His thoughts are with them."

The couple's newborn twins Zoe Grace and Thomas Boone were mistakenly given a massive overdose of the anti-coagulant drug last month while hospitalized at Cedars-Sinai Medical Center in L.A. The Quaid's attorney says the twins "were very critical for a while," but appear to have recovered and "everything looks good." They filed suit because they want to prevent this from happening to any other children, Loggans said. The suit claims the Baxter is liable and negligent because the packaging of the 10 unit vial of Heparin looks almost exactly as the 10,000-units-per-milliliter vial of the drug.

Three children died in Indiana from a similar mix-up with the drug.

We're told that the Quaids have not sued Cedars ... yet. Just yesterday, Baxter issued a news release outlining the company's new "drug safety initiative" that promises to change the labeling of Heparin vials.

Cedars-Sinai released the following statement:

What started off as an awkward hug devolved into a brawl outside the hotspot Mood. Security eventually stepped in, but not until after one shirtless guy took several kicks to the back.

In the safety alert dated February 6, 2007, the FDA and Baxter Healthcare Corporation, one of the pharmaceutical companies that manufacture heparin, warned of the dangers of switching 10 and 10,000 unit vials of the drug. The alert was issued after three infants died in Indiana after they were mistakenly given adult doses. The memo tells hospitals to double-check their inventory to make sure errors in dispensing do not occur. We're told a memo with this information was also circulated around that time at Cedars-Sinai Medical Center. Despite the memo, the dispensing mistake happened last Sunday and Dennis Quaid's twins and several other patients were accidentally overdosed.

The California Department of Public Health says they are investigating the incident.

Thomas Boone and Zoe Grace are in stable condition. But a well-placed source at Cedars tells us they are "still very concerned because of the bleed out," adding they won't know for another week if the mistake will cause "longterm effects."

Sources tell TMZ that pharmacy technicians stock the drug Heparin, used to prevent clots and flush out IVs. The drug comes in vials -- 10 units for babies, up to 10,000 units for adults. Protocol at the hospital is to keep the different units separated, but a technician accidentally put 10,000 units in the drawer where the 10 units were stored.

Last Sunday, both infants -- born November 8 by surrogate -- were each given two, 10,000-unit dosages. They began to bleed out just before midnight and were transferred to the neo-natal intensive care unit.

TMZ caught up with Dr. Jan Adams as he arrived to chat -- for about a minute -- with Larry King last night, and he told us nothing, nor did he have anything to say on his way out, despite being peppered with questions.

Larry King appeared later, saying that this was "the first time" a guest had come on and then bailed. Another dubious distinction for Adams.
