Kevin Hart's car crash was caused by the driver operating the vehicle in a reckless fashion ... this according to the California Highway Patrol, which has just completed its report on the accident.

Law enforcement sources tell us ... Kevin's 1970 Plymouth Barracuda crashed because driver Jared Black gunned the engine and lost control. The car had incredible power with a 720-HP engine. We're told the sudden acceleration and loss of control caused the vehicle to go into a spin and it ended up crashing through a fence and into an embankment.

According to our sources, the report says none of the 3 people in the car were wearing seat belts. As we reported, the Barracuda did not have safety harnesses, but now it's not an issue since they weren't wearing any safety device.

Our sources say the vehicle did not have any mechanical issues. It was purely driver error -- willful or wanton disregard for people or property.

Boxing superstar Errol Spence -- one of the top pound-for-pound fighters in the world -- is badly injured and being treated in an Intensive Care Unit in Dallas after crashing a white Ferrari early Thursday morning.

The 29-year-old was driving on Riverfront Blvd. around 3 AM at a high rate of speed, according to cops, when the car went over the center median and flipped at least 5 times.

Spence, who was not wearing a seatbelt, was ejected from the car and suffered serious injuries, officials say. You can see in the photos, obtained by FOX 4, it doesn't appear the vehicle has a roof.

Spence was transported to a nearby hospital for treatment. Police say he's expected to survive.

The Dallas Police Dept. raced to the scene. Several officers were seen investigating the crash. It's unclear at this point if they believe alcohol was a factor.

Kevin Hart is getting back to his famous "Hustle Hart" mode after his horrific car accident -- but he's not rushing into anything intense -- like full action scenes -- just yet.

Sources connected to Kevin tell TMZ ... he's reported to work this week to promote the upcoming 'Jumanji' sequel in a new marketing campaign, which clearly required his presence, because he's nowhere near 100 percent yet.

Ever the trooper though, we're told Kevin is committed to fulfilling his commitments as best as he can. For example, our sources say Kevin did 2 hours of shoots on set Monday, another 2 hours Tuesday, and will be back for another couple on Thursday.

We're told Kevin has not been in full costume for these shoots and he's not walking much for them either -- sitting through most of the day -- even though he can walk on his own these days.

Everyone else on set -- including The Rock, Jack Black, Nick Jonas and others -- have been doing full days themselves. Of course, they're not recovering from a near-fatal wreck.

Madness at the weigh-in for the highly anticipated Claressa Shields vs. Ivana Habazin fight -- when Habazin's trainer was hospitalized after being sucker punched at the event.

James Ali Bashir was laying in a pool of his own blood after someone snuck up and socked him in the face HARD at the event in Flint, Michigan.

Emergency personnel raced to the Dort Federal Event Center to treat Bashir -- who was wheeled out of the facility on a stretcher while wearing a neck brace. Video was captured by FOX 66's Jonathan Deutsch.

Habazin was seen crying while standing over Bashir -- clearly very emotional.

Bashir is well known in the boxing world -- he worked under legendary trainer Emanuel Steward for 17 years and worked with guys like Wladimir Klitschko.

Cops have taken the suspect into custody but so far, that person's identity has not been revealed. There are rumblings the attacker was someone connected with Shields but so far, that has NOT been confirmed.

Shields was set to fight Habazin on Saturday night in the main event fight on Showtime. No word on the current status of that fight.

Shields issued a statement on social media ... saying, "I wanna thank everyone who came to weigh-in to support me. Because of the incident that took place, they had to clear the Dort federal for safety issues."

Bernie Sanders has put his campaign on hold ... this after he underwent emergency heart surgery.

Sanders was at a campaign event on Tuesday in Vegas when he began experiencing chest pains. He was taken either to a doctor's office or a hospital -- although it would seem a hospital is far more likely -- and tests revealed he had a blockage in one of his arteries.

Surgeons inserted 2 stents to clear the blockage. The 78-year-old Senator is resting in the hospital and campaign events over the next few days have been canceled.

Sander's senior adviser, Jeff Weaver, said, "We are canceling his events and appearances until further notice, and we will continue to provide appropriate updates."

Sanders is the oldest of the remaining Democratic candidates ... Joe Biden is 76. BTW ... Donald Trump is 73.

Sanders has been struggling in the polls ... overtaken in a big way by fellow Senator Elizabeth Warren.

Gene Simmons is on a break from rockin' and rollin' all night to deal with some pesky kidney stones -- and he underwent a procedure this week to nip it in the bud.

Sources close to the KISS singer tell TMZ ... Gene was admitted into Cedars-Sinai Hospital early Tuesday morning to undergo a medical procedure that should help get rid of the stones. It's non-invasive, we're told, and only took about an hour to complete.

Our sources say a stent was inserted in his urethra, which helps urine flow from the kidney to the bladder ... ultimately forcing the stones to pass through. Sounds like he'll be peeing 'em out soon.

We're told the stent will be removed in about a week -- and any remaining stones will be removed too. Gene should be good to go at that point.

Ray Lewis is OUT of "Dancing with the Stars" -- the NFL Hall of Famer suffered an injury while practicing a Cha-Cha .. and his castmates are already sending their love.

The 44-year-old Baltimore Ravens legend and his partner Cheryl Burke were rehearsing for their tribute to "Ferris Bueller's Day Off" when Ray tore a tendon in his foot ... just like he did during his playing days.

Of course, the whole thing played out during the broadcast of the show Monday night -- when Lewis revealed he tore 3 tendons in his foot and would need surgery.

"In a game in 2010, I tore a tendon in my foot," Lewis said on the show ... "And, now, all of this dancing, all of this on your toes, I’m reliving a problem that I thought I’d never have to live again."

Phil Collins is living proof his Not Dead Yet tour is an accurate measure of his physical well-being, because he's still kicking after taking a nasty backward fall.

Phil was performing at his Not Dead Yet Tour Sunday night at the Spectrum Center in Charlotte, NC. In between songs, the 68-year-old singer walked over to a chair with an assist from a cane. He was eager to enjoy a front-row seat to his son's sick drum solo.

Problem was ... Phil's momentum took him backward. As you can see and hear in the video ... the audience gasped in shock while stage crew members ran over to help him up.

Dog makes the stunning revelation on an upcoming episode of "The Dr. Oz Show" ... revealing he's been diagnosed with a pulmonary embolism.

In the clip, Dr. Oz tells Dog he's a "ticking time bomb" before offering this sobering message ... "You're not going to be here with the heart the way it is right now."

Dog says he learned of the scary diagnosis after he was rushed to the hospital a week and a half ago with chest pains.

Christie Brinkley is out of a cast after injuring her right arm in a nasty fall during "Dancing With The Stars" practice ... and it's clear why she was forced to bow out of the competition.

Christie's surgically repaired right wrist looks badly bruised and mangled ... and she even shared a few images from before and after she went under the knife, including her X-ray.

"Kings of Pain" star Adam Thorn served up his arm as a snack for a massive python ... all in the name of measuring pain.

Adam -- a wildlife biologist and cohost of the new series on the History channel -- was extremely apprehensive about the ordeal ... as he and his cohost, professional animal handler Rob "Caveman" Alleva, traveled to a jungle and volunteered to get bitten by a 16-foot reticulated python.

How apprehensive were they? The duo brought out protective cups and face shields. Gotta protect the family jewels and faces that feed the fams. Anyway, watch the python lock on to Adam's arm. Warning: this is NOT for the squeamish. The snake's teeth got stuck in Adam's arm and the bite created an absolute bloody mess.

Obvious question ... WHY?!? The guys say they're following in the footsteps of Dr. Justin Schmidt -- an entomologist (insect scientist) who, in the '80s, put himself in harm's way to create a sting pain index. Adam and Rob wanna do the same but aim to take it beyond insects.

Porn star Bridget Powers wasted little time going on the attack when she caught her boyfriend with another woman -- she immediately woke him with a butter knife to the calf!!!

According to the police report -- obtained by TMZ -- Bridget showed up to her BF Jesse's place in the wee hours of the morning and found him asleep with another woman. She told cops she screamed at them, grabbed a knife ... and stabbed Jesse in the leg.

The other woman says she was awoken by Bridget knocking at the bedroom door before she busted in screaming ... "You sl*t, I knew you didn't love me," at Jesse.

The woman claims she saw Bridget stab him in the leg, and when she tried to restrain Bridget ... she tried to stab her too but missed. According to the report, the other woman -- who's 5'8" -- picked up the porn star and threw her out of Jesse's home.

"Gypsy Sisters" star Kayla Williams' 1-year-old grandson is on a ventilator in the hospital after a scalding hot bath left his skin covered in second-degree burns.

Kayla tells TMZ ... the terrifying incident went down Sunday morning at her daughter Sissy's home in West Virginia when Sissy's son, Henry, was given a bath by his 7-year-old stepsister. Kayla says Henry's stepsister put him in the tub after letting hot water run without turning on any cold water, and the results were horrendous.

According to Kayla, Henry's stepsister quickly realized what she did and turned on the cold water, but it sent Henry into shock ... plus he was badly burned.

NFL sensation Gardner Minshew II is admitting his master plan to break his hand with a hammer in college was a "pretty dumb" idea ... and says he's grateful he didn't succeed.

Of course, the Jacksonville Jaguars QB revealed the INSANE story of smashing his hand with a hammer on Barstool Sports' "Pardon My Take" last week ... saying he was trying to get a medical red shirt so he could play another year of ball during his first year at East Carolina University.

Minshew has clearly given some thought about his failed attempt at shattering his paw ... saying in hindsight, it probably wasn't the best way to go about things.

"It didn't work, I guess, fortunately. I mean... looking back, it was pretty dumb," Minshew told reporters on Wednesday. "Believe it or not, breaking your hand is pretty dumb."
