3:40 PM PT -- TMZ has just learned there was a second individual who was injured during the incident. The Costa Rican Red Cross tells TMZ ... the second individual was rushed to a local clinic in critical condition. Their current condition is unknown.

3:10 PM PT -- Judicial Investigation Agency tells TMZ ... Malcolm was rescued by bystanders and taken to the shore where he received aid from the Costa Rican Red Cross. He was declared dead at the scene. The body was sent to the Judicial Morgue in San Joaquín de Flores, Heredia for an autopsy.

10:53 AM PT -- Warner died of asphyxia after he was caught by a high current in the ocean, according to ABC News. His body was reportedly found Sunday afternoon.

"The Cosby Show" alum Malcolm-Jamal Warner has died in Costa Rica at the age of 54, TMZ has learned.

TMZ has a reliable source who says Warner died as a result of an accidental drowning while he was vacationing with his daughter.

As you know, Malcolm‑Jamal Warner straight‑up owned the '80s as Theodore Huxtable on "The Cosby Show," snagging an Emmy nod and locking in his spot in TV royalty.

Law enforcement sources working the case in Memphis tell TMZ Hip Hop that U.S. Marshals recently took a 19-year-old woman named Talia Jones into custody in Minnesota for her alleged role in the killing of Patrick Houston, Jr. ...  which they believe may have been the result of a targeted robbery that turned fatal.

Jones remains in custody and is expected to be extradited to Memphis to face charges. Houston Jr. was only 21 at the time of his death.

Bill Cosby is reacting to the sudden death of "The Cosby Show" alum Malcolm-Jamal Warner ... and it's taking him back to a dark time in his life -- his son's murder.

Cosby's rep, Andrew Wyatt, tells TMZ ... when Bill got the call that Warner died on vacation in Costa Rica, it reminded him of being in the studio filming "The Cosby Show" in January 1997, when he got a call informing him that his son Ennis Cosby was shot dead in a failed robbery attempt.

Wyatt says news of Malcolm-Jamal's death hit him the same way as Ennis' murder ... the two used to play together when they were younger and "were amazing together."

Malcolm‑Jamal Warner was looking forward to some R & R in Costa Rica just one week before his tragic passing at 54, and his arrival there was captured in what could be the final public pic of him.

The actor was photographed at the airport in CR, looking relaxed and low‑key on July 13. He had just arrived on a flight from Atlanta ... exactly one week before his tragic death by drowning.

TMZ broke the story ... Malcolm-Jamal's body was found Sunday after he got caught in an ocean current, resulting in his accidental drowning.

Magic Johnson is one of countless heartbroken fans mourning the death of "Cosby Show" alum Malcolm-Jamal Warner at the age of 54 ... with the Lakers legend sharing a touching tribute to the Emmy-nominated actor -- saying "he will truly be missed."

Johnson shared the tribute to X not long after TMZ broke the story that Warner died as a result of an accidental drowning while in Costa Rica with his daughter.

"[Cookie and I] were both super fans of the hit Cosby Show and continued to follow his career on shows like Malcolm and Eddie and The Resident," Johnson said. "Every time I ran into Malcolm, we would have deep and fun conversations about basketball, life, and business."

Dog the Bounty Hunter's step-grandson's fatal gunshot wound was in his neck ... TMZ has learned.

We obtained the police report from the Collier County Sheriff's Office in Florida and deputies say 13-year-old Anthony Zecca had an entry wound on his neck, just above his sternum, and no exit wound.

Law enforcement says Anthony had lost a ton of blood by the time they arrived at the Naples apartment where he was shot ... and the teenager received CPR and other life-saving measures, including chest compressions.

Dog the Bounty Hunter's family is going through a personal tragedy ... because his stepson shot and killed his step-grandson in what we're told was an accidental shooting.

Sources with direct knowledge tell TMZ ... Gregory Zecca -- son of Dog's wife, Francie -- shot his 13-year-old son Anthony in a freak accident at their apartment in Naples, Florida Saturday night.

Law enforcement confirms to us they responded to a call about a shooting around 8 PM ET. They haven't made any arrests and call this an "isolated incident." The investigation is ongoing.

Corey Adams -- a defensive lineman on the Ole Miss football team -- has been killed in a shooting ... according to reports.

ESPN -- citing the Shelby County Sheriff's Office -- reports Adams was killed in a shooting in Cordova, Tennessee, in the greater Memphis area.

Four other men were also shot ... though they were rushed to the hospital and are not in critical condition, ESPN reports. No one has been arrested in connection to the shooting at the time.

A ferry carrying 280 people caught fire off the coast of Indonesia over the weekend, leaving at least one person dead and prompting a dramatic rescue operation.

The ferry was traveling from the Talaud Islands to Manado when it erupted in flames around 12 PM local time Sunday. Shocking videos show passengers jumping into the sea as fire engulfed the upper deck, with thick smoke billowing into the sky.

Rescue ships and local fishing boats rushed to save those in the water. One survivor said she floated for nearly an hour before being rescued. Authorities say survivors were transported to a nearby island, and search efforts for any remaining passengers are ongoing.

Eileen Fulton, the actress who became a household name through her role as Lisa Grimaldi on "As the World Turns," has died at the age of 91.

Fulton passed away on Monday, July 14, in Asheville, North Carolina, following a period of declining health, according to an obituary shared by Groce Funeral Home.

After studying drama and music at Greensboro College, she moved to New York City in 1956. Her breakout came in 1960 when she appeared in the film "Girl of the Night."

Mihaela Schwartzenberg shared the clip Friday night of Felix rising into the air ... noting in a touching tribute alongside the video that she had no idea he was beginning the final flight of his "extraordinary" life.

She went on to remember being there for nearly every one of Felix's takeoffs and landings -- from skydiving and paragliding to helicopter flights and beyond. And when she wasn't there, they had a "WLC code" -- standing for "the Wifey Landing Call." She added ... "Never missed one. Except …this one."

A friend of the late Annabelle investigator Dan Rivera is super concerned about the other staff working on the "haunted" Raggedy Ann doll's tour -- 'cause he thinks the whole damn thing is CURSED!!!

Philip Siracusa -- the former owner of the "Haunted Horsefly Chronicles Home" -- tells TMZ ... he became buds with Rivera when he stopped by to check out paranormal activity inside the alleged haunted house in 2020.

Siracusa says the ongoing tour of Annabelle should promptly close up shop because it's "cursed" following Rivera's untimely death at a hotel in Gettysburg, Pennsylvania, last week. He says he believes the tour is a bad idea and will open up a can of worms that may affect other people if it continues.

Felix Baumgartner -- an Austrian BASE jumper and skydiver who set a record after jumping from the stratosphere down to Earth back in 2012 -- has died while paragliding in Italy ... according to a local outlet.

He was 56 years old.

According to La Repubblica, he was paragliding in Porto Sant'Elpidio when he became ill and lost control of his aircraft. He ended up crashing into a hotel swimming pool ... injuring a woman, who sustained non-life-threatening injuries.

G Herbo wasn't in attendance for rapper Mello Buckzz's Chicago album release party that left four people dead, but as a guest feature on the project and close friend of hers, Swerve says he's feeling the PTSD!!!

TMZ Hip Hop linked up with Herbo in NYC on Wednesday ... a bittersweet trip considering the 4th of July Weekend shooting is still fresh in his mind.

Michael Madsen’s cause of death has been confirmed ... TMZ has learned the actor passed away from cardiac arrest.

TMZ has obtained the death certificate from the L.A. County Medical Examiner, which details his cardiac arrest was brought on by cardiomyopathy, coronary artery disease, and chronic alcoholism. Another significant condition was thromboembolic disease -- blood clots causing obstruction of vessels or arteries.

As we reported ... Michael was found unresponsive July 3 in his Malibu home and was pronounced dead at the scene. At the time, his rep told us he suffered cardiac arrest. He's since been cremated, according to the death certificate.
