"Bachelor in Paradise" star Grant Kemp officially -- and finally -- removed his ex from his life ... with the stroke of a needle.

We're told Grant Kemp on Sunday hit up Black Heart Tattoo in San Fran where artist Moses Saarni inked over the old "Grace" tattoo -- a combination of his and 'BiP' ex Lace Morris' names.

You'll recall Grant and Lace got their tattoos on the show before he proposed in the season finale ... and promptly broke up 3 months later.

Grant's new tat -- a black rose -- sends a clear message: You're dead to me. Also, I made a huge mistake. Far as we know, Lace still has her ink.

We're guessing it'll be gone before sundown.

It's the awkward moment ya just knew was going to happen -- The Weeknd and Bella Hadid in the same room again.

The former couple hit up Madison Square Garden and both were onstage for Yams Day to honor the late music exec and rapper A$AP Yams ... who died 2 years ago Wednesday.

Kendrick Lamar, A$AP Rocky, French Montana, and Kendall Jenner were also there -- and this is really a case of squads colliding. Bella and Kendall are pals with Rocky ... so they were most likely there to support him.

No word if Weeknd and Bella ever came face-to-face, but it seems inevitable.

At least Selena wasn't there.

Nick Young's a single man again ... 'cause the Lakers star and his pop singer girlfriend, Paloma Ford, have officially broken up ... TMZ Sports has learned.

Sources close to the two tell us there was no cheating or secret baby mamas (like the Iggy Azalea situation) ... this time, the two just grew apart.

The two started dating back in September and things heated up quick. But after a few months, both sides agreed that Nick's family should take priority ... especially with his new kid.

We're told Nick and Paloma are still cool and are gonna do the whole "friends" thing. He even has a cameo in her upcoming music video ... and we're told she plans on keeping him in it.

Bella Hadid believes Selena Gomez did her dirty by dating The Weeknd behind her back.

Sources close to Bella tell TMZ she feels betrayed because her ex and Selena hooked up almost immediately after Bella and Weeknd ended their relationship.

Here's where it gets interesting. We're told it was actually Bella who pulled the plug 2 months ago, because the bi-coastal relationship was just too difficult. Nevertheless, the breakup hurt.

Also interesting ... a Selena source says the thing between Selena and Bella's ex is 2 months old. They even spent the holidays together.

Bella is especially incensed because she and sister Gigi helped Selena when she was on the skids dealing with numerous emotional and other issues.

And there's more. Despite the 2-month relationship, Selena never uttered a peep about it to Gigi, who's her best friend. Gigi and Bella found out about it when they saw the pics on TMZ. And that's when Bella unfollowed Selena on Instagram.

Johnny Depp and Amber Heard will be single before dinnertime, because the judge is about to seal the deal and make their divorce final.

Amber was trying to delay the final divorce so she could take Johnny's deposition, but the judge turned thumbs down in a hearing Friday, saying enough was enough.

TMZ broke the story ... Johnny had agreed to pay Amber $7 million to settle the divorce, but Amber attempted to renegotiate the settlement. The judge ruled the $7 mil settlement stood, and that's what she'll get.

Johnny's lawyers asked the judge to impose a $100k sanction on Amber for delaying the proceeding, but the judge rejected that. She wanted Johnny to pay her attorney's fees but the judge said each should foot their own bills.

Tarek El Moussa has filed for divorce from wife Christina, but we're told they are resolved to continue their show, "Flip or Flop."

According to the divorce docs, obtained by TMZ, Tarek cites irreconcilable differences as the reason for ending his 8-year marriage.

He's asking for joint custody of their 2 kids. But this is interesting ... he wants her to pay him spousal support but wants the judge to block any move she makes to collect it from him.

A source close to the couple tells TMZ, "It's civilized, or as best it can be under the circumstances. They have to make it work if they want to continue monetizing what they do."

Music mogul Scott Storch pulled a Britney -- 24 hours after getting married, he wanted out, and is now filing for an annulment due to extreme intoxication.

The famed producer just filed the docs, attempting to get out of marriage ... which went down in 2011. Scott says he and Christina Gray said, "I do" on June 7, 2011 -- but separated the next day. He says they haven't been together since then.

In the docs, Scott admits he and Christina were hammered when they tied the knot, and were "not of sound mind." It is weird he's just now getting around to filing annulment docs ... 5 and half years later.

In this case, the hangover definitely lasted longer than the honeymoon.

Nicki Minaj and Meek Mill broke up after a nuclear fight during her birthday weekend last month ... and it wasn't about cheating.

Sources close to the ex-couple tell us ... they weren't getting along in Turks and Caicos, because he was hanging with friends and not paying attention to her. She went nuts on him, he got pissed and left the island without her, and she said, "Screw this," and ended the relationship. Just that simple.

There are reports Meek had been cheating with a woman named Sonye Rasool and that's what set off Nicki. A source close to Sonye tells us she and Meek hooked up years before Nicki, but she fully denies any cheating. We're told Sonye's about to file defamation lawsuits against several outlets.

We're also told Sonye understands truth is a defense, but she has no fear.

It's interesting ... the breakup appears to be over the small stuff, which makes you wonder if the door is still open.

Nicki Minaj won't be single for long if she gets on the horn with her ex-fiance and producer, Safaree Samuels ... who's open to rekindling things, and also throwing major shade at Meek Mill.

Nicki's ex thinks it's about time she dropped the Philly rapper because their relationship was always a better look for Meek than her ... according to a rep for Safaree.

The rep tells us Safaree thinks Meek did zilch to boost Nicki's career. Case in point, she didn't release a single new album during nearly 2 years of dating him. Or, as Janet would put it: what has Meek done for you lately?

If this sounds like a swoop in progress, there's a good reason. We're told Safaree never stopped loving Nicki, and is ready and willing to reopen communication ... if she is too.

Of course, their breakup was kinda nasty ... so it's interesting Safaree would come to her defense. Each publicly accused the other of cheating, and he even dropped several breakup raps.

Olive branch extended?

Meek Mill responded to Nicki Minaj's breakup announcement with a parting shot at her fashion sense. Burn.

The since-deleted post featured a white shoe and a caption that read: "If you walk out don't wear these they wack...."

The sneaker wedge is the same one Nicki wore in the video for Ciara's 2013 breakup anthem, "I'm Out." Gotta say, Meek firing off style jabs doesn't come off as the greatest comeback ... but we suspect there will be more coming.

Nicki beat him to the punch by announcing the couple's demise early Thursday morning. They'd been together for almost 2 years.

Nicki Minaj and Meek Mill are history ... Nicki says she's a single woman, and now we know ride or die has its limits.

Nicki just tweeted, "To confirm, yes I am single."

As for why she's confirming, TMZ's Raquel connected the dots Tuesday ... making a compelling argument the former couple had to do somersaults to avoid each other in Miami over New Year's Eve. They partied separately and on top of that, they were apart on Xmas.

Nicki stuck with Meek during his house arrest and his losing battle with Drake, but at a point, enough was enough.

To quote Drake in "Back to Back, "I don't want to hear about this ever again. Not even when she tell him that they better as friends."

It's smellin' like a breakup between Meek Mill and Nicki Minaj.

Both Nicki and Meek performed separately in Miami for New Year's Eve, and they also partied separately afterward. Nicki performed at E11even nightclub and then stayed to party. Meek performed at Dream Nightclub 2 miles from E11even and stayed there.

Nicki showed up at E11even at 11:30 PM, but Meek didn't show up at Dream until 2:40 AM, which would have given him plenty of time to ring in 2017 with Nicki.

Meek was also partying in New York on December 30 ... with Nicki nowhere in sight. On January 1st, Meek went to LIV nightclub in Miami and she wasn't there.

We're told they weren't together on Christmas Day as well.

Sources close to Tiny tell TMZ T.I. thinks Tiny has overreacted because they've always gotten through rough patches in their 15-year relationship. He waited a few weeks so she could cool down after filing divorce docs, but now he's trying talk "sense" into her ... that they're good together.

Each thinks the other was pushed by "jealous friends" into filing to end their marriage.

Halle Berry and Olivier Martinez are officially single ... and he agreed to let her say, "I left you!"

Halle and Olivier filed for divorce at almost the exact same moment back in October 2015 ... she filed as the petitioner and he also filed as the petitioner seeking a divorce.

Under California law only one person can be the petitioner, and Olivier agreed to give Halle the honor.

Under the settlement, they will share physical and legal custody of 3-year-old Maceo.

For some reason, they decided to only deal with the custody issues and the property settlement is still unresolved. It's weird, because our sources say there's a prenup so there shouldn't be big issues.

Halle and Olivier married in 2013.

"Rogue One" star Ben Mendelsohn barely had time to enjoy his movie's big premiere -- his wife filed for divorce just 7 days after it opened.

Emma Nadine Forrest filed legal docs, obtained by TMZ, which has all the makings of an uphill battle for Ben. Emma's asking for physical custody of their 3-year-old daughter.

According to the docs ... Emma -- who's a British author -- and Ben officially separated December 1 because of irreconcilable differences. They got married back in June 2012.

She's also asking for spousal support.

Ben played the role of Orson Krennic in the blockbuster film.
