Coach Prime survived bladder removal surgery to stop a cancerous spread back in July -- and Karrueche was spotted supporting him bedside ... sparking the notion they were an item.

When J. Ryan got Karrueche to cop to dating Deion, she confirmed her relationship is legit and she's very happy.

Karrueche -- who's publicly dated Chris Brown and Victor Cruz in the past -- says she wouldn't even waste her time if the love wasn't good.

The Washington Post obtained the death certificate for 65-year-old Irsay ... which listed his cause of death as cardiac arrest due to acute pneumonia, chronic atrial fibrillation and cardiac arrhythmia on May 21.

The document is signed by Dr. Harry Haroutunian, who'd been tending to Irsay at the hotel in the days before his death, and he told police the cause of death was natural.

CNN released the wild footage Thursday, which shows dozens of kids stampeding out of Annunciation Catholic School in Minneapolis the day before. Older students are seen hurrying younger ones through a set of doors, kids cowering behind pews, and others simply weeping.
