ABC says the network DID have a 5-second delay in place on "Good Morning America" this morning ... but insists Nicki Minaj's nip slip just happened to fall through the cracks.

A spokesperson for the network tells TMZ, "Although we had a five-second delay in place for the Nicki Minaj concert on GMA , the live East Coast feed of the concert regrettably included certain fleeting images of the performer that were taken out of later feeds of the broadcast in other time zones."

It's her first photo shoot since the actress got out of house arrest two weeks ago -- taken in the middle of the night inside the famous 2nd Street tunnel in Downtown L.A.

If you're wondering who the dudes are ... it's Lindsay's photog BFF Tyler Shields and Virgin Mobile "Sparah"-half Spencer Falls ... who Lindsay was reportedly dating.

UPDATE: Jennifer's people tell us they've seen the unblurred photo and insist Lopez is wearing "clothing tape."

Khloe was wearing a black shirt with see-thru material around the sleeves and part of her chest area -- and during the interview, her boob slipped into the see-thru section for all to see.

Nobody on the set seemed to notice -- but tons of viewers did ... and they bombarded Khloe's Twitter account with messages about the nip slip.

The 26-year-old looks hot through thick and thin.

Katy shot a segment for the show -- which was supposed to air on New Year's Eve -- in which she sings a cleaned-up version of her song, "Hot N Cold" with Elmo.

The song made its way to YouTube Monday and racked up nearly a million views. But some parents started complaining her cleavage was too front and center. We've learned producers have decided not to air the song on the show.

Among the parental comments:

Fact is ... what looks like Katy's bare chest is actually covered in flesh-colored mesh that goes all the way to her neck.

OK, now to the facts -- It was all part of Marc Ecko's secret "Muse" campaign directed by Markus Klinko and Indrani -- a secret he's had to keep under wraps for several months ... hence the mask.

TMZ has learned Lindsay posed for the shoot back in May -- right around the time she was getting into trouble with Judge Marsha Revel.

We're told Marc chose Lindsay because he wanted someone "whom fascinated us -- for better or worse."

Well, mission accomplished.

Smith participated in something called the Pimpit.com "Boxing for Boobs" event back in 2007. She lost in the second round.

Sandy would have at least made the semifinals.
