Usher has denied every allegation his ex-wife has made in her motion to wrestle custody away from the singer in the wake of Usher Raymond V's nearly fatal pool accident -- he's even denied she's a good mom.

In the docs -- obtained by TMZ -- Usher is not specific about what he's denying ... he just denies, denies, denies.

Tameka Raymond made various allegations, including that Usher was an absentee parent for 85% of the month, that his aunt failed to properly supervise the boy at the swimming pool, and that he has said he'd prefer that the nanny raise his kids. Usher denied everything.

And there's this ... Tameka claims she's a capable mother. Usher denied that, too.

The judge in the Amanda Bynes case just granted a temporary conservatorship to her parents.

The judge based his decision in no small part on the findings of an investigator he appointed to evaluate Amanda's mental health.

As we first reported ... Amanda was not in court, because doctors at the psychiatric hospital got a 1-month extension on her psych hold ... because she's "gravely disabled" with mental illness.

Amanda's lawyer told the judge ... she's spoken with the actress several times and Amanda opposes the conservatorship, believing it's not necessary.

The conservatorship not only covers Amanda's personal well-being, it gives her mom the ability to manage her finances. Amanda's $4 million savings have been rapidly depleted, though it's unclear what she's been spending money on. The judge cautioned the parents not to make any significant deals without consulting with Amanda's lawyer.

The judge said he'd like to have some face-to-face time with Amanda, so he can make a firsthand evaluation.

7:54 AM PT -- Kourtney's attorney, Todd Wilson, tells TMZ ..."This individual has been selling false and fabricated stories to the tabloids for years about Kourtney Kardashian and her son, Mason. Scott Disick is Mason’s father."

Wilson continues, "We have not seen the reported court filing, but we are informed that this individual has been seeking to get paid to publish his ridiculous pleading. His claims are absurd and this is simply another shameful attempt to gain publicity and profit from fictional stories about my client."

A male model who claims he had unprotected sex with Kourtney Kardashian in 2009 claims 3-year-old Mason is HIS son and he's now filed a paternity lawsuit asking for joint custody ... TMZ has learned.

Michael Girgenti claims in legal docs -- filed Thursday and obtained by TMZ -- he met Kourtney during a photo shoot in August, 2008 (above). Girgenti says they connected and began a texting relationship for 3 months.

Girgenti says in legal doc things heated up by March, 2009, when Kourtney allegedly told him she and then BF Scott Disick were "on the outs" and "she wanted to hang."

Girgenti's suit then describes an alleged sexual encounter in Glendale, where he says they had "unprotected vaginal sex." He adds, "I ejaculated inside [Kourtney's] vagina."

Girgenti says 9 months later Kourtney gave birth to Mason. He says he immediately saw a striking resemblance and tried contacting her, to no avail, so he began a media blitz to get her attention.

Girgenti's docs, filed in L.A. County Superior Court, request joint custody of Mason. He also wants the judge to order DNA tests for Kourtney, Scott, Mason and him. The judge set a hearing for next month.

We also know Girgenti received a letter from Kourtney's lawyer, Shawn Holley, saying his allegations are "false and defamatory."

Usher's ex-wife has one serious question before her emergency custody hearing today -- how the hell is it possible to let a 5-year-old nearly drown if you're supposed to be babysitting him? -- and only Usher's aunt can answer it ... but she's nowhere to be found.

Sources close to the situation tell TMZ, Tameka Raymond attempted to serve Usher's aunt Rena with a subpoena to testify at the custody hearing today -- where Tameka will argue she deserves primary custody of her and Usher's two sons -- but Rena couldn't be tracked down.

We're told Tameka wanted Rena at the hearing so her lawyers could ask about what led up to the near-drowning of her 5-year-old son on Monday ... when Rena was supposed to be watching him ... but without the subpoena, Rena doesn't have to show up.

Even without Rena, however, Tameka has plenty of ammo to fight for custody -- and we're told her biggest weapon is that there was no drain cover on the pool the day Usher Raymond V nearly drowned.

As we reported, the 5-year-old got his arm caught in the drain and nearby contractors had to dive in to save him. Rena called 911.

Usher currently has primary custody of the ex-couple's two sons ... but Tameka hopes to change that today.

Heidi Klum and her rebound guy are getting super serious ... like, buying a house together serious ... and one of the places they're checking out is Katy Perry's L.A. mansion, TMZ has learned.

Heidi and her boyfriend Martin Kristen have been house hunting all over L.A. recently -- and sources tell us they're looking for a "family home" that has enough space for the couple and Heidi's 4 kids.

We're told Klum knows exactly what she wants -- she wants to be east of the 405 Freeway and needs 4 bedrooms for the kids that are ALL located on the same floor as the master.

Enter Katy Perry -- who still owns that incredible mansion in the Hollywood Hills that she purchased in 2012 with then-husband Russell Brand ... which has everything Klum is looking for.

Klum and Kristen biked over to the place last week to check out the 7 bedroom, 11 bathroom estate ... and we're told they were impressed.

It could be a perfect situation for Katy -- who recently reduced the asking price of her sprawling dream palace to $6.495 million hoping to motivate buyers.

Whoever snatches up the place is gonna get a helluva property -- it's like a real-life castle, complete with a lagoon-style pool and views to die for.

Tito Ortiz is scrambling to protect his reputation in the face of a public smear campaign ... helmed by his baby mama Jenna Jameson -- telling TMZ, whatever she's about to say on TV is bound to be filled with lies ... because she's a dirty liar.

FYI, Jenna recently sat for an interview with "Entertainment Tonight." It's unclear what the interview is about, but Tito's guessing he's a main talking point ... and given their sordid history, he probably isn't portrayed in the best light.

So he's coming out to clear his name preemptively -- telling us, "I want to make it very clear that any and all allegations she has made about our personal life and my character are completely false. Jenna continues to paint a picture of distorted reality, which has been hurtful to both myself and my family. I hold sole custody over our boys, which speaks for itself."

He adds, "I am moving on with our life and with our boys, and I pray this is all over soon. I wish Jenna would get help to not only save herself, but for the children she brought into this world."

Amanda Bynes will be in court Friday --TMZ has learned -- and we're told she will mount a ferocious fight for her freedom.

Sources involved in the case tell TMZ ... Amanda will be transported from the psychiatric hospital -- where she's been staying against her will for the last 2 weeks -- to a courthouse where her parents will be asking a judge for a temporary conservatorship.

We're told Amanda will be transported by a hospital vehicle and accompanied by her lawyer. The only snag would be if Amanda goes off the rails before the hearing.

Sources tell us Amanda will be speaking with the judge -- possibly in chambers -- so he can get a read on her mental competency.

If the parents prevail, the temporary conservatorship will last until Sept. 30, when a full-blown conservatorship hearing will take place. If the parents get their way, Amanda will go back to the psych ward for further treatment.

If Amanda prevails, she will walk out of the courthouse a free woman.

Usher's ex-wife is clearly trying to prove a point ... she's a caring mother who deserves custody of their sons -- after the eldest nearly drowned this week -- and this photo tells the story.

Tameka Raymond posted the photo today, the first pic of 5-year-old Usher Raymond V since the horrific accident on Monday ... and it paints a picture of motherly love and protection. Tameka's arms are wrapped around her son, and he's surrounded by get-well-soon balloons.

It's a subtle dig at Usher ... who is noticeably absent.

8:18 AM PT Rihanna's rep tells us the singer paid the bill in full, and the allegation that there's an outstanding balance is "completely false."

Rihanna sent her grandma out in grand style ... and now the funeral company has sent her a lawsuit.

Clara Brathwaite died in June, 2012. Her body was sent to Barbados for burial, and Rihanna wanted a party, complete with tents, multi-media large screens, huge exotic floral arrangements -- the whole kit and caboodle.

But parties/funerals come at a price ... and the company that threw grandma's final shindig claims the grand total was more than $150,000, and Rihanna paid only about a quarter of that and refused to pay more.

The lawsuit claims Rihanna claimed the bill was exorbitant and she refused to pay the balance. The funeral home claims Rihanna's demands came at a hefty price.

So far no comment from Rihanna's camp.

Kim Kardashian's ex-stepmom Ellen Kardashian has just sued Kris Jenner and the Kardashian sisters for defamation, claiming the K-fam is on a mission to make her look like a bitter gold digger -- when they're the ones who should be ashamed of themselves.

Ellen filed the suit against Kris, Kim, Khloe, Kourtney, Rob, and even Ryan Seacrest Productions -- insisting the girls and "Keeping Up with the Kardashians" producers attempted to ensnare her in a recent plot line on the show, involving a private investigator attempting to hawk damaging information about the Kardashians.

According to the lawsuit, the investigator attempted to sell the information to Ellen -- in order to make her look like some kind of black mailing scumbag -- but Ellen refused to take the bait.

Ellen says the Kardashians trashed her on the show anyway, making up lies about her to fit their anti-Ellen agenda, such as accusing Ellen of marrying Kim's dad Robert Kardashian for his money -- and Ellen's lawsuit gets downright nasty.

Ellen -- who was married to Robert Kardashian before he died -- calls Kris Jenner a "manipulative and devious mother and ex-wife who simply used and exploited her children." She also calls the family's careers meaningless.

FYI, the feud between Ellen and the Kardashians has been raging for years ... it's nothing new.

Ellen is suing for unspecified damages. Calls to the Kardashians weren't returned.

Lindsay Lohan is true to her word -- going out for some SOBER fun with everyone in her family ... except Dina Lohan.

Sources close to LiLo tell TMZ ... the recently rehabbed actress went to see "The Conjuring" in NYC with dad Michael Lohan, sister Ali, brother Michael Jr. and her sober companion Monday night. Afterwards they chowed down on Japanese food.

We're told everyone was stone-cold sober the entire night and they still managed to have fun. It's unclear why Dina wasn't a part of the shindig -- but we have a good guess.

Our sources say Lindsay's trying hard to keep her nose clean -- she's relying on little sis Ali for support, having already moved into her NYC apartment.

Usher's ex-wife has just filed legal docs asking for an emergency hearing later this week ... to wrestle child custody away from him, on grounds he's creating a dangerous environment ... TMZ has learned.

Tameka Raymond filed the docs in Fulton County, Georgia, claiming Usher is abdicating his parental responsibilities by letting other people care for their 2 kids -- people who she claims are not on the ball.

We know Tameka has specific beef with the Usher's aunt -- who was watching 5-year-old Usher Raymond V when his arm got stuck in the pool drain on Monday. Tameka has complained about the aunt in the past -- alleging she once let a stalker in the house where Usher and the kids were staying. Tameka also says the aunt is too physical in disciplining the kids.

Currently Usher has primary custody of both kids and Tameka has limited custodial rights. She now seeks to reverse that.

TMZ has obtained the 911 call placed by Usher's aunt moments after the singer's 5-year-old son nearly drowned in a pool ... in which she begs the operator to send help ASAP.

Usher's aunt Rena Oden is in hysterics during the call, telling the 911 dispatcher, "My nephew was in the pool. He went and I couldn't get him. I tried to get him. They did CPR on him."

During the call, she says the boy is breathing after receiving CPR.

At one point during the call, you can hear a child -- presumably the boy's 4-year-old brother -- saying the kid is faking his injury. Rena sternly corrects him, saying he is NOT faking.

N-word hurling Philadelphia Eagles player Riley Cooper isn't getting any sympathy from his own grandma -- who tells TMZ, Cooper screwed up big time ... and it's his mess to clean up.

Riley's Grandma Betty didn't make excuses when we spoke to her ... saying, "[Riley] brought it upon himself and he has to deal with the consequences."

As we reported, Cooper has already taken leave from the Eagles to pursue counseling after his n-word threat at a recent Kenny Chesney concert -- when he said he would fight "every n***er" at the show.

Grandma says Riley was raised in a good household that never condoned racism of any kind -- so she's surprised by his behavior, especially because she says he's never gotten into any kind of trouble before.

That said, Grandma Betty is optimistic about Riley's future, telling us it's good he's nipping this problem in the bud now before it gets any worse.

This is surprising ... Lindsay Lohan does NOT consider her dad to be a toxic influence -- in fact, she actually wants him to be a big part of her life while she focuses on rebuilding herself after rehab.

As we reported, Lindsay went on a friend-slashing massacre a few weeks ago while in treatment, putting together a list of the bad people she wanted to cut out of her life once she got out -- and Michael was NOT on the list.

We're told Lindsay actually wants to keep Michael around because she believes -- despite his glaring flaws -- that he's actually a positive influence on her ... and family is really important.

Even crazier ... we're told Michael was in the therapy session with Lindsay when she discussed the "bad" list. Her brother Michael Jr. was there too.

Britney Spears LOOOOVES herself some waterslides and lazy rivers ... 'cause the pop star threw on a couple of bikinis this week and hit up a Southern California waterpark -- 2 days in a row!

TMZ obtained pics of Brit -- along with her kids -- at Six Flags Hurricane Harbor in Valencia, CA on Wednesday where Ms. Spears showed off her hardbody physique in a bright pink bikini.

Brit was also spotted at the park on Tuesday ... 'cause as everyone knows, ya can't hit up all the rides in just one day!
