Halle had no visible injuries or bandages as she walked to her trailer on the LA set of "The Hive" -- her first day back at work since she was rushed to a hospital on Tuesday night.

As TMZ reported, Halle fell and hit her head while shooting a fight scene -- and people on set were concerned because she started throwing up after the fall ... a common sign of a concussion.

UPDATE 6:45 AM PT -- Sources on the set tell TMZ ... after Halle fell, she started throwing up, and that's when people around her became alarmed and called for help. FYI, throwing up is a sign of a concussion.

UPDATE 5:40 AM PT -- A rep for Halle tells TMZ "Halle Berry suffered a minor head injury while shooting a fight sequence on the set of her film, 'The Hive.' She was taken to the hospital as a precaution, but she checked out healthy and was released. She'll continue production as planned."

Halle Berry was rushed to the hospital Tuesday night after suffering an injury while shooting a movie.

Sources tell TMZ ... Halle hit her head on concrete after taking a nasty fall.

According to a photo agency, Halle was taken to Cedars Sinai Medical Center in L.A. We're told the injury occurred at around 10 PM. There was an ambulance on scene, and it appears that's how Halle was transported.

Halle was shooting the movie, "The Hive." You'll recall she suffered an ankle injury during another movie shoot in Spain.

Her condition is unclear.

DMX is sportin' a couple of stitches on his melon after his brutal ATV crash last weekend ... but the rapper tells TMZ he's planning to "thug it out and get well quick."

TMZ spoke with X ... who says he was released from a South Carolina hospital Sunday night ... one day after flipping his 4-wheeler while riding down a hill.

X says he's still in a lot of pain ... "My head still hurts bad from my concussion and I am having really bad headaches."

X suffered multiple lacerations in the wreck -- and required 3 stitches to close a gash on the top of his head.

Still, the rapper tells us, "It's all good. I am going to thug it out and get well quick."

Tony Parker is still feeling the impact of the glass bottle that hit him in the eye during the Chris Brown vs. Drake nightclub brawl ... 'cause a month later, he's wearing protective goggles to keep the injury from getting worse.

Parker -- who says he was merely an innocent bystander in club W.i.P. on June 14 -- was all Kareem'd out during an Olympic warm-up game with the French national team yesterday.

Tony recently underwent surgery to repair his left eye after a shard of glass penetrated "99 percent" of his eyeball during the melee.

DMX had quite the rough ride early this morning ... when he lost control of the four-wheeler he was riding and ended up in the hospital with a concussion.

X tells TMZ he was riding outside his home in South Carolina when he crashed while riding down a hill. He says he flipped over and, lucky for him, landed right on his ass.

The rapper says he doesn't remember much, other than the lights from the ambulance, and he doesn't know when he will be released.

B2K singer Raz B is healing nicely from the gruesome hand injury he suffered in China this week, but he tells TMZ it's going to be AT LEAST three months until his hand is functional again.

As TMZ first reported, Raz B accidentally fell through a glass door in his hotel room in Shanghai, slicing his wrist wide open and losing a scary amount of blood.

Raz B tells TMZ he lost count of how many stitches it took to sew him up ... but says doctors will remove his patch work sometime next week.

Raz says he's still experiencing numbness in his fingers ... and doctors have told him it could take at least three months until he regains full functionality.

Once he recovers, Raz tells us he wants to get back to his tour as soon as possible.

Just stay away from the glass doors.

Michael Clarke Duncan just got a hospital visit from his former TV co-star Geoff Stults ... who tells us MCD appears to be recovering nicely after going into cardiac arrest early this morning.

On his way out of the hospital, Stults -- who appeared on "The Finder" with Duncan -- told us, "I saw him ... he's looking good ... thats all we know right now."

Michael Clarke Duncan is in the intensive care unit at an L.A. hospital after going into cardiac arrest this morning ... and might have died if not for the swift action of his girlfriend Omarosa Manigault.

TMZ has learned ... Omarosa discovered the "Green Mile" actor in a state of cardiac arrest just before 2:00 AM.

We're told Omarosa performed CPR and was able to resuscitate the 54-year-old.

MCD was transported to a nearby hospital where he was immediately admitted to the ICU.

Sources tell us ... Duncan's heart rate is now stable and Omarosa is by his side. Doctors are performing tests to figure out why the actor's heart stopped in the first place.

MCD's publicist tells us, "According to doctors, Michael Clarke Duncan suffered a myocardial infarction early this morning. He is now stable and we look forward to his full recovery."

Kristin Chenoweth has been released from the hospital ... after sustaining a head injury on the New York set of the CBS legal drama "The Good Wife" yesterday ... TMZ has learned.

A rep for the actress released a statement saying, "Kristin has been released from the hospital and is home recovering. She thanks everyone for their prayers and well wishes."

TMZ broke the story ... Kristin was rushed to the hospital at 5:07 PM ET after she was struck by a piece of lighting equipment.

Kristin Chenoweth was rushed to a New York hospital after she was injured on the Brooklyn set of "The Good Wife" Wednesday afternoon ... and TMZ has obtained dramatic photos of the actress being treated on the scene.

A source connected to the production of the CBS show tells us ... Kristin was struck by a piece of lighting equipment that collapsed because of a large gust of wind.

We're told Kristin immediately fell to the ground ... and smacked her head on the concrete during the fall.

Someone on the set called 911 around 5:07 PM ET to report a woman who had been hit in the head by a scaffold.

TMZ spoke with a witness who was on the scene who says, "That area is a vortex for wind ... and a big gust of wind blew the lighting off the scaffolding."

The witness tells us Chenoweth was hit "square in the head" and appeared to be "knocked out cold." The witness says, "I thought she was dead at first."

Paramedics treated Kristin on the set ... fitting her with a neck brace and putting her onto a stretcher. She was quickly taken away from the set in an FDNY ambulance and was admitted to a nearby hospital.

A rep for the New York Fire Dept. tells us ... Kristin is being treated for "minor injuries" connected to the accident.

Kristin seems to be accident prone ... she injured her neck during the pre-Broadway engagement of "Wicked" in 2003.

Then, in 2006 she fell off an elevated section of the stage during a rehearsal of the Broadway revival “The Apple Tree.”

Chumlee from "Pawn Stars" was involved in a crazy -- and bloody -- fight right in the middle of Hollywood Blvd. this weekend ... but the reality star tells TMZ it was all in self-defense.

A video has surfaced showing Chum and his pals hanging out in front of Chum's Rolls Royce ... when they are approached by an unknown man.

Chum tells us ... the guy was harassing the group and demanding a ride in the Rolls. When Chum's group refused, Chum says the man threatened to pull a gun.

Chum says the man then reached for something in his pocket ... so his boys took action, socking the guy in the face HARD ... dropping the man to the pavement.

Once the guy hit the floor, Chum and his crew scrambled to get back in the car and quickly fled the scene ... while the man tried to pick himself up, blood pouring out of his face.

Chum tells TMZ, "Instinct just took over. [The man] didn’t even know I was on TV or anything … just wanted to ride in our car and start problems.”

Sublime's summer tour nearly came to a screeching halt last weekend -- when the band's bassist dislocated his shoulder during a freak longboard accident ... and his rottweiler's to blame.

It all went down Sunday at Summerfest in Milwaukee -- sources close to the band tell TMZ, Eric Wilson's rott was pulling him around backstage on his longboard ... when the bassist took a nasty spill.

We're told Wilson's hand had somehow gotten tangled in his dog's leash -- and the strong animal dragged him 10 feet, dislocating his right shoulder.

Paramedics responded to the scene and rushed Wilson to a nearby hospital, where doctors popped the shoulder back into its socket. Eric's now back on the road, but still recovering.

Lucky for Eric, he didn't injure his left shoulder, which supports his bass strap and does most of the moving when he plays. Eric's rep tells us, "If he had injured his left shoulder, we would have had to cancel the rest of the tour."

Famed L.A. Lakers owner Jerry Buss was rushed to an L.A. hospital last night, where he was treated for severe dehydration -- Buss family sources tell TMZ.

According to sources, 78-year-old Buss fell ill at his home around 9pm and someone inside the house called 911. The L.A. City Fire Department responded and paramedics rushed Jerry to a local L.A. hospital.

Jerry is still being treated, but a rep for the Lakers says, "He is recovering and is expected to be released soon."

Buss was most recently hospitalized in December for blood clots in his legs due to excessive travel.

B2K singer Raz B nearly died Sunday in China -- after accidentally falling through a glass door in his hotel room ... slicing his wrist wide open ... and losing a SCARY amount of blood.

Raz -- who had been performing in Shanghai before the accident -- tells TMZ, he tripped hard inside his room ... and when he tried to break his fall, he crashed through a sliding glass door.

Raz says the broken glass caught his wrist and ripped it to pieces, tearing through ligaments, nerves, tendons ... right down to the bone. (We've seen a graphic close-up of the injury, and decided to spare you the visuals. You're welcome.)

Raz says he immediately began gushing blood and quickly passed out. Someone else in the room then alerted the hotel, and an ambulance was called.

Raz was rushed to Huashan Hospital Fudan University, where doctors operated to repair his mutilated wrist -- a grueling procedure that took roughly 5 hours.

Raz was released from the hospital Monday. He has since canceled his remaining tour dates -- for now.

Rapper Lil Scrappy believes gay men like Frank Ocean are the "doorway to AIDS" ... and wants more closeted homosexuals to "come out" so it can't be spread to straight people.

Scrappy -- who now stars on VH1's "Love & Hip Hop: Atlanta" -- was on "TMZ Live" this week to talk about Ocean's revelation ... when he explained why he completely SUPPORTS the R&B singer.

"I'm glad that he came out ... so all the real women that love to mess with real men, straight men, we can keep the AIDS situation down, you feel me?"

Scrappy continued, "[homosexuality] is a doorway to AIDS, scientifically."

As for the facts, there is evidence that shows men who have sex with men are at an increased risk of contracting HIV. According to the Center for Disease Control and Prevention, men who have sex with men accounted for 61% of all new HIV infections in 2009.

George Clooney and his girlfriend Stacy Keibler got to enjoy a meal they ate in Italy this week ... twice -- because hours after eating it ... they both got a bad case of food poisoning ... this according to sources connected with the couple.

George, Stacy, and some friends dropped by a restaurant in Cernobbio July 4th -- in the same province where Clooney owns a fancy lake house -- but they quickly regretted it.

The next day, Stacy tweeted, "Finally had a bad meal in Italy, our whole dinner party got food poisoning ‪#ohwell‬ at least I'm still in Italy."

Our sources -- who are in a position to know -- tell us the entire party who dined with George and Stacy that night got food poisoning.

But the manager of the restaurant in question told us ... George and Co. must have gotten food poisoning elsewhere ... because it didn't happen at his restaurant.
