Philip Seymour Hoffman Famous Friends Flock to Funeral Spike Lee, Cate Blanchett, Louis C.K., Ethan Hawke

Philip Seymour Hoffman's funeral has just begun -- and the church is jam-packed with the late actor's famous friends ... including Cate Blanchett, Spike Lee, Louis C.K., Ethan Hawke, and Ellen Burstyn.

The private funeral service for family and friends is now underway at the Church of St. Ignatius Loyola in NYC -- the same church where Jackie Kennedy Onassis and Aaliyah's funerals were held.

All told, roughly 400 people were expected to attend the ceremony.

As we reported, 46-year-old Hoffman was found dead Sunday from an apparent heroin overdose. He is survived by his longtime partner Mimi O'Donnell and 3 young children.

A larger memorial service is in the works for later this month.

Justin Bieber Selena's Not Lovesick ... She's a Drunk

Justin Bieber is scoffing at Selena Gomez, telling friends she has no one to blame but herself for her addictions ... TMZ has learned.

Sources with daily Bieber contact tell TMZ ... Bieber and his friends are laughing at Selena's "bad influence" claims, because they say she's always the drunk at the party. Bieber acknowledges he gets wasted, but whenever he had parties Selena was downright competitive ... smoking as much weed as the guys and sometimes outdoing them. They say she was also HEAVY into booze.

We're told Justin believes Selena's parents are behind the blame game, because they HATE him. He thinks they are fueling Selena's anger and making him a scapegoat.

Justin says he's happy he's no longer with Selena because she's just too high maintenance ... and out of control.

Philip Seymour Hoffman A Single Drink Ultimately Led to Death

Philip Seymour Hoffman's recent and fatal path down the road of addiction was triggered by an innocent drink ... sources close to the actor tell TMZ.

Hoffman had been sober for 23 years prior to shooting "The Master" -- but during a wrap party in 2012, the actor succumbed to temptation ... and celebrated the movie with a drink ... which quickly became a couple of drinks.

Hoffman acknowledged to confidants ... the drinks opened the floodgates. Addictions experts all say one drink can destroy an addict's life ... and that seems to be what happened here.

Sources tell us Hoffman didn't begin experimenting with heroin until 2013, after he had already fully fallen off the wagon. We're told he had long been curious about the drug.

It started with snorting -- and last May, he quickly checked into rehab because he was terrified it would escalate to IV use ... which it eventually did.

Selena Gomez Bailed On Rehab

Selena Gomez ignored the advice of medical professionals and waltzed out of rehab just 14 days into a 6-week program ... over the strenuous objections of the rehab staff ... TMZ has learned.

Sources close to Selena tell us just after New Year's she checked into a program at The Meadows called DAWN -- for alcohol, pot and Rx Ambien -- in addition to what her people believe is an unhealthy union with Justin Bieber.

But 2 weeks in, Selena bailed so she could go the Sundance Film Festival for her upcoming movie. She told her people she would return to the rehab facility after Sundance, but when it was over Selena decided she was cured and there was no reason to return.

Problem is ... that's not the way the medical staff at The Meadows sees it ... the treatment program is 45-days and Selena bailed four weeks early. We're told the staff feels it's essential in any recovery to complete the therapy.

Selena is telling her people ... she completed the toughest week, which the facility calls "Survivor's week" so everything is fine. Thing is ... Selena's not the doctor -- she's the one with the problem.

Chris Brown Took Rehab Break ... To Party Face Off

Chris Brown made a little pit stop on his way back to rehab Sunday -- after visiting his lawyer -- he dropped by Trey Songz' blowout Super Bowl party ... filled with girls and booze ... and it's possible the shindig could land Chris in some hot water.

Brown was given a pass out of rehab Sunday specifically to meet with his attorney, Mark Geragos, for the singer's court hearing the next day.

After the meeting, Chris decided it would be fun to enjoy a few Super Bowl moments with his pal Trey Songz. There was nothing in the rehab pass indicating partying with chicks and booze was ok.

Sources close to Chris tell TMZ, he was only there for around 10 minutes ... and only because he thought the party was for charity. We can't help but call BS. There's no evidence there was any fundraising going on, and even if there was ... it's irrelevant.

Our Chris sources say he didn't drink and was monitored by a rehab worker at the party.

Calls to Chris' reps weren't returned.

Selena Gomez Blames Her Rehab Stint On Justin Bieber

Selena Gomez went to rehab last month ... and sources connected with the singer say she blames Justin Bieber for her predicament.

We've learned Gomez went to The Meadows in Arizona for 2 weeks of treatment.

Sources say Selena went for a combination of problems, including alcohol, pot and prescription Ambien. She's also addicted to Justin Bieber.

We're told Selena's decision was largely based on "that crazy boy" -- aka Bieber. Selena blames a lot of her problems on the excesses she was exposed to by being around Bieber and his buddies.

Sources close to Gomez tell us ... people in Selena's camp went ballistic when they found out she was hanging out again with Justin ... and saw photos of her joyriding with him on Segways. We're told that was the final straw -- she checked into rehab immediately after that.

The 2-week treatment was part of a program called DAWN at The Meadows ... specifically designed for people between 18 and 26. DAWN specializes in substance dependence, trauma, family dynamics, mental health and relapse prevention.

We're told Selena is not getting outpatient help now nor is she in therapy.

Philip Seymour Hoffman Long-time Partner Makes Final Plans

Philip Seymour Hoffman's longtime partner came out of seclusion Tuesday ... to make plans for the actor's funeral.

Mimi O'Donnell went to the funeral home on New York's Upper East Side to make arrangements for Friday's service. She was accompanied by Hoffman's personal assistant.

Hoffman and O'Donnell were together 15 years and had three kids. There are unconfirmed reports the two split recently.

We're told the funeral will be small and private -- for family and close friends.

Ke$ha Touring Can Wait ... Gotta Finish Rehab First

Ke$ha is taking her rehab so seriously ... she's decided to postpone her March and April tour dates because she needs more time to deal with her eating disorder.

Ke$ha's people tell us she decided to push back her shows so she can "remain committed" to her ongoing treatment.

We broke the story ... Ke$ha checked herself into Timberline Knolls treatment facility near Chicago ... the same place that treated Demi Lovato.

Ke$ha originally said she was undergoing a 30-day treatment program. She's now been in for 34 days.

Philip Seymour Hoffman 'Ace of Hearts' Heroin Traced to Near Fatal OD

Several bags of heroin found in Philip Seymour Hoffman's apartment were potentially lethal ... so says a woman who almost died from using the brand.

We got this photo from a woman who is very familiar with the NYC heroin scene. She asked us not to ID her. The woman gave us this picture, showing the "Ace of Hearts" heroin brand.

She tells us she injected herself with a "tiny bit" of "Ace of Hearts" back in Sept., and says she stopped breathing almost instantly. She was revived with CPR and landed in the hospital.

The woman says when she got out of the hospital she went to cops and told them about what she called a lethal strain of heroin going around NYC.

The woman says she saw Hoffman on the streets in the weeks before he died and it was apparent to her he was strung out.

We do not know if Hoffman injected himself with "Ace of Hearts" the day he died, since there were dozens of heroin bags in his apartment. It appears Hoffman may have died quickly after an injection, since a needle was sticking out of his left arm when he was found.

Philip Seymour Hoffman 'I Know I'm Gonna Die'

Philip Seymour Hoffman was on a heroin binge 6 weeks before he died, and told friends he feared he was destined to fatally OD ... TMZ has learned.

Sources Hoffman confided in tell us ... back in December Hoffman told them he had started injecting himself with heroin and couldn't kick it. Hoffman said he would kick it for a few days and then fall off the wagon.

We're told he went back to AA in a desperate attempt to clean up ... but much to his great frustration it didn't work.

During the last 6 weeks he often looked disheveled and dirty. At one point someone asked him how bad his problem was, and he responded, "If I don't stop I know I'm gonna die."

We're told Hoffman was also drinking excessively ... which opened the door to many bad decisions when it came to heroin use.

Ironically and sadly, we're told Hoffman went to AA meetings over the last 4 years and was considered a "guru," because he had been sober for so long. Hoffman would give inspirational talks to people who attended.

Philip Seymour Hoffman Apartment Littered with Rx Pills

Besides the heroin ... cops found a laundry list of prescription meds in Philip Seymour Hoffman's apartment ... including pills to treat addiction, ADHD, and anxiety.

According to law enforcement sources ... investigators found methocarbamol (muscle relaxer), hydroxyzine (anti-anxiety), Vyvanse (ADHD), and buprenorphine to treat addiction ... and clonidine hydrochloride (blood pressure).

It's unclear at this point if Hoffman had prescriptions for any of the drugs.

Of course the pills were just a small part of the stash cops found. As we've reported ... there were 50 bags of heroin as well as used and unused hypodermic needles.

Ke$ha Producer Dr. Luke Target Of Twitter Death Threats

Ke$ha's music producer Dr. Luke is in serious danger, according to the LAPD, after some of the singer's fans went nuclear -- blaming Luke for her eating disorder.

LAPD's Threat Management Unit filed legal docs -- obtained by TMZ -- asking for a search warrant to hunt down the identity of people who are threatening Luke's life on Twitter.

The vengeful tweets started after TMZ ran a story that Ke$ha entered rehab for an eating disorder because Luke taunted her about her weight. Ke$ha's friend told us ... Luke told her she "looks like a f**king refrigerator."

In the search warrant docs, cops point out some of the tweets, including:

-- "You need to be worried because IMA slit your throat before January is over."
-- "You ugly fat ass bum bitch. Im gonna carve u like a pumpkin with a long sharp knife."
-- "Im gonna gut you like a fish then skin you and glue ur wrinkly raisin skin to a fridge!"
-- "I'm gonna slit your F**KIN throat open then gouge your eyeballs out."

Cops are taking the threats seriously, particularly because one tweet gave the home address and phone number of Luke and then invited followers to kill him.

Chris Brown, CeeLo Make Full Court Press

Chris Brown and CeeLo had a gig at the same place at the same time Monday ... the criminal court building in downtown L.A.

Chris was there for a hearing in the Rihanna beating case ... the judge had some questions about the D.C. assault case as well as his anger management rehab.

As for CeeLo ... he's there for a routine hearing in his felony drug case.

Chris got top billing. His hearing was on the 13th floor and CeeLo's was on the 5th.

Chris Brown D.A. Wants Him Jailed ... Now

Chris Brown would have gone to jail today if L.A. prosecutors had their way ... because they asked a judge to send him directly to the pokey.

Chris appeared for a hearing on the D.C. assault case, where he stands accused of beating up a guy last October. The hearing was held by the judge in the Rihanna beating case, who will decide if Chris violated his probation by allegedly committing the assault.

Prosecutors asked the judge to throw Chris in jail immediately and set a probation violation hearing. Prosecutors claim "Brown's violent behavior continues to increase in severity and frequency" and he poses "an increasing danger to society."

The D.A. claims they have now reviewed the D.C. case -- interviewing witnesses and examining evidence -- and they say there is probably cause to believe Chris is guilty as charged.

The judge, however, ruled Chris was better off completing his 90-day rehab for anger management than being thrown in jail, so he shut down prosecutors.

Philip Seymour Hoffman Nods Off On Plane After Drinking Binge

Philip Seymour Hoffman was out like a light minutes after boarding a plane in Georgia last week ... after he was seen hitting the bottle at an Atlanta bar ... TMZ has learned.

According to a passenger ... Hoffman was out almost as quickly as he took his seat for the NY-bound jet Thursday ... his head falling forward into his chest.

We're told sometime after the plane took off ... PSH came to ... groggy and disheveled. After the flight touched down, we're told he was quickly put on a waiting airport cart and whisked away.

As we first reported ... Hoffman had been drinking in a downtown Atlanta bar earlier that day -- just 3 days before his death -- witnesses said he appeared drunk and made repeated trips to the bathroom.

Trace Adkins Father Dies After Adkins Rushed To His Bedside

Trace Adkins' father has died ... just days after Adkins hastily ditched rehab in Florida to be by his father's bedside ... TMZ has learned.

TMZ broke the story ... Trace cut short his treatment for alcoholism last Wednesday ... saying he wanted to be with his dad who was seriously ill.

We've now learned Aaron Adkins has passed away in Louisiana ... and Trace was able to get there in time for his dad's final hours.

We're told Trace wanted to take care of his dad ...before deciding whether he'll return to an inpatient rehab center.

Adkins checked himself into a treatment facility last month ... after getting into an explosive fight with a Trace impersonator on board a cruise ship.
