The doctor who performed the endoscopy on Joan Rivers has been fired by the clinic's board.

Sources at Yorkville Endoscopy tell us Dr. Lawrence Cohen stepped down after the board asked him to do so -- however the board isn't directly blaming him for Joan's death.

A rep for the clinic confirmed he was ousted from his position as medical director.

According to our sources ... the board's issue with Cohen is that he allowed Joan's ENT doc to step in and perform an unauthorized procedure. Reportedly, that procedure was a biopsy ... and Joan went into respiratory and cardiac arrest while it was being done.

Kanye West ranted on in concert in Australia, despite the SKULL EXPLODING migraine headache he was treated for in an emergency room earlier that day.

The guy’s a real trooper, cuz the show (and the rant) must go on, even if you’re in excruciating pain.

'Sons of Guns' star Stephanie Hayden opened up to Dr. Phil about her father allegedly raping her and her sister -- and seconds after the interview she fainted and blacked out.

Stephanie sat down with Dr. Phil last week in Los Angeles -- and sources tell us Stephanie fell unconscious into her husband's arms as she walked away from the interview.

We're told someone called 911, but Stephanie had regained consciousness by the time EMTs arrived.
She had mentioned being stressed about discussing Will Hayden's rape case.

As for the interview ... we're told she divulged details for the first time ... about the abuse she claims to have suffered.

The interview airs Thursday. Her father remains behind bars.

The surgery clinic where Joan Rivers' heart stopped beating is calling BS on a report suggesting her death was caused by a biopsy that should have only been performed at a hospital.

Yorkville Endoscopy broke its silence after the New York Daily News story ran ... saying a biopsy was NOT performed during the ill-fated procedure.

The Daily News story claimed a doctor who arrived with Rivers was performing the biopsy after doctors spotted something on her vocal cords, and her vocal cords seized ... leading to a fatal chain of events.

And Yorkville went on to say they do NOT administer general anesthesia, saying they use "light to moderate sedation."

New York authorities are still investigating cause of death.

Kanye West was rushed to the hospital with a throbbing headache while on tour down under ... but made a quick turnaround and was able to perform the same night.

Yeezus and his entourage rushed to the emergency room Wednesday in Melbourne after playing basketball.

Sources tell us Kanye had a pretty intense migraine. He was released later that afternoon after doctors checked him out and determined it wasn't serious.

The Australian media is claiming Kanye suffered seizure like symptoms ... but we're told it's not true.

The Season 2 winner of "RuPaul's Drag Race" got busted for assault ... but he tells TMZ he's the one who got beat down by a security guard during an autograph session.

James Ross, who goes by Tyra Sanchez, tells TMZ he was signing autographs at the Rainbow-Cactus Company in Virginia Beach a few weeks ago ... when a guy in the crowd started heckling him.

Tyra claims he approached the man to try to calm the situation, but a security guard for the venue suddenly grabbed him, slammed him to the ground, and cuffed him ... though he eventually let him go. He claims he suffered a cut to his head, a sprained shoulder, and bruises.

On Monday, Tyra went to police to file charges against the venue ... only to discover there was a warrant out for HIS arrest. He says he turned himself in, posted bond, and was released.

Tyra says he still plans to take legal action against the guard and the Rainbow-Cactus Company. We haven't heard back from the venue.

Richard Kiel, who became famous for portraying the James Bond villain, Jaws ... died Wednesday afternoon at the age of 74 ... TMZ has learned.

Kiel starred in dozens of movies over the span of 50 years ... though his Jaws character became his most iconic role after debuting in 1977's "The Spy Who Loved Me" alongside Roger Moore. He returned in "Moonraker" two years later.

The 7-foot-2 Kiel also co-starred with Adam Sandler in the golf classic "Happy Gilmore."

Kiel had broken his leg last week and was in a hospital in Fresno, CA. A family member tells us he passed away this afternoon in the hospital -- and it's unclear if his death was connected to the leg injury.

Kiel's family tells TMZ ... "Though most people knew of him through his screen persona, those who were close to him knew what a kind and generous soul he was. His family was the most important thing in his life and we are happy that his last days were spent surrounded by family and close friends."

Paris Hilton probably gets hit on all the time ... but it's rare when a homeless lays the mack down.

Paris was leaving Tao restaurant in NYC when she stopped to give some cash to a homeless guy -- and that's when the dude saw his chance to spit some game.

He kissed her. He smooth-talked her. He asked her out on a date.

And for the record ... she didn't exactly say 'No.'

A doctor performed an outpatient procedure on Joan Rivers that should ONLY have been performed in a hospital ... and that's what led to her death ... according to a new report.

During Rivers' planned endoscopy ... doctors discovered something on her vocal cords and her doctor decided he would biopsy it ... this according to the NY Daily News.

During the biopsy Rivers' vocal cords reportedly seized -- slammed shut -- cutting off her air supply. Sources tell TMZ while they were trying to resuscitate her, her heart stopped.

Medical experts told the Daily News ... the biopsy was too risky for outpatient surgery and should have been performed in a hospital.

Rivers never recovered and died a few days later.

Conrad Hilton was driving so out of control moments before he crashed his car near Palm Springs ... at least one alarmed driver was convinced he was going "about 200 miles an hour" ... according to 911 tapes obtained by TMZ.

As we reported ... Paris' youngest brother crashed after clipping the rear of 2 other vehicles, and had to be cut from the wreckage with the jaws of life. Somehow he walked away without major injury.

So many people called 911 after seeing Hilton fly by on the freeway -- sometimes on the shoulder -- the police dispatcher seemed to get annoyed at repeat callers ... and even challenges the 200 MPH report.

The "Love & Hip Hop: Atlanta" star has a "flipper tooth" -- a removable fake tooth -- which made a hasty exit smack in the middle of her single release party at Club Sutra in ATL.

3:05 PM PT -- The fire's out, and most of the coaster is still intact. The blaze was reportedly started, accidentally, by welders who were building the new Twisted Colossus.

One of Six Flags' most famous roller coasters ... the Colossus went up in flames Monday -- just weeks after the classic ride was shut down.

The fire engulfed a big chunk of the coaster's track ... burning completely through the wood and steel. Firefighters were called to the scene, North of Los Angeles, around 1:30 PM.

The 36-year-old roller coaster had just been shut down in August, so the park could start work on a new and improved version ... to be called Twisted Colossus.

Colossus has been featured in tons of TV shows and movies like "National Lampoon's Vacation" ... "Wonder Woman" ... "Step by Step" .... "Knight Rider" and the "A-Team."

Mega-popular DJ Avicii has canceled his upcoming tour and promotional activities because of lingering effects from surgery earlier this year ... TMZ has learned.

A rep for Avicii tells TMZ he is still suffering from certain health issues related to surgeries that removed his gallbladder and appendix back in March. Avicii is resting at home in Stockholm so he can fully recover and promote an upcoming album.

As TMZ first reported, Avicii bailed on his gig at the Ultra Music Festival in Miami after he was rushed to the hospital due to severe abdominal pains, nausea and a fever.

This is what a two game suspension looks like -- Ray Rice delivering a vicious punch to his fiancee's face, knocking her out cold ... and TMZ Sports has the shocking video.

We've already shown you the aftermath outside the elevator ... Rice dragging the unconscious woman on the floor. But we've now obtained video of the punch that put her down, raising the question ... What was the NFL thinking when it wrist-slapped Rice with such feeble punishment?

The incident took place Feb. 15th at the Revel Hotel and Casino in Atlantic City -- after Ray and then-fiancee (now wife) Janay Palmer got into a heated argument on their way into the elevator.

Inside the elevator it's apparent he strikes first ... she hits back ... and then Rice delivers the knockout blow.

The punch knocks Janay off her feet -- and she smashes her head on the elevator hand rail ... knocking her out cold. Ray doesn't seem fazed ... and when the door opens, he drags her out into the hotel.

An employee of the hotel -- which just shut down for good -- tells TMZ Sports he was working there at the time and says the NFL saw the elevator footage before imposing the 2-game suspension.

UPDATE -- The NFL says it did NOT see the newly-released elevator footage before it imposed a 2 game suspension on Rice.

UPDATE 11:35 AM PT -- The Ravens have terminated Ray Rice's contract.

Garth Brooks performed a fire drill during his concert Friday in Chicago ... only problem ... he was nowhere near any flames.

Brooks came out of retirement as a touring headliner this weekend ... and the 52-year-old singer miserably failed when he attempted to jump in the air during one of his songs.

The country legend managed to avoid rolling off the stage -- but he was inches away -- and his ten gallon hat still fell into the pit.

Jake "The Snake" Roberts is out of the hospital following a scary stay for double pneumonia, but during his first public appearance Saturday night he announced he's fighting cancer again.

Roberts surprised the crowd at a Future Stars of Wrestling show in Las Vegas shortly after being released ... telling the crowd how he's feeling.

Jake says during his 3 day hospital stay ... doctors discovered some cancer inside his chest that needs to be removed. He had a cancerous tumor in his leg removed earlier this year. But he's not worried about it, Jake says he's fought worse.

His voice is weak, but it's pretty bad ass to see the Hall of Fame wrestler's positive outlook on his fight with cancer.
