Gwen Stefani was literally on pins and needles after an obsessed fan threatened to meet her at a Korea Town acupuncture clinic ... forcing her to call in cops.

Law enforcement sources tell TMZ ... the singer's been dealing with this fan for awhile, but things took a terrifying turn Monday when he sent her a slew of messages which said he'd be waiting at the clinic, waving from a nearby spot.

A rattled Stefani -- who had her 1-year-old son Apollo with her -- called police who spotted a suspicious man and questioned him. Turns out, it was just some random guy who happened to get some fresh air at the wrong time.

We're told investigators are still trying to track down whoever's sending the messages.

Something is bothering Miley Cyrus ... it's pretty clear when you put the pieces together, and there's lots of speculation it has something to do with Patrick Schwarzenegger.

We got Miley going to the Laugh Factory in Hollywood Monday, where she was stone faced when asked about the pics we posted of P.S. manhandling another girl during Spring Break in Cabo.

Patrick tweeted it was just a good friend and he would never betray Miley, but she posted some weird messages Monday, including:

-- "Don't you ever let someone treat you like a yellow Starburst. You are a pink Starburst."

-- "Surround yourself with people that reflect who you want to be and how you want to feel, energies are contagious."

And she posted a pic of a bunch of hot dogs in her mouth, suggesting silence is the way to go.

Bruce Jenner is continuing his ride to transform, because we've learned a few weeks ago he got a nose job to smooth things out.

Sources close to Bruce say the goal was to "soften" his features and make his nose look more feminine. Must say ... it worked.

It's the latest in a series of physical changes, that included a laryngeal shave to smooth out his Adam's Apple.

As we reported Bruce has also been undergoing hormone treatments and has been undergoing therapy for the transformation ... therapy that has been going on for years.

10:40 AM PT -- Patrick didn't do Miley dirty ... he says the chick is just a friend. Moments after we posted the pics, he tweeted, "Omg. It's one of my best friends girlfriend" -- and added, he would never step out on his GF.

Patrick Schwarzenegger might be taking a wrecking ball to his relationship with Miley Cyrus ... as he got very close with some OTHER chick in Cabo this weekend.

The timing of this is pretty odd ... considering Patrick and Miley just went out to dinner last week with Patrick's mother, Maria Shriver.

It's also possible this girl is just a friend ... though "friends" don't usually do body shots off one another.

Then again, when Miley is your girlfriend, something like this is probably no big deal.

Patrick seemed pretty chill about everything in this Instagram shot.

Multiple sources connected with Miley and Patrick tell us their relationship is intense. Miley is going out of her way to impress Patrick's mom, Maria Shriver, and it goes beyond dressing conservatively when Miley's around her. The conversations do not resemble what Miley puts out on social media.

The couple has partied hard, and initially people who spent time with them thought it was just a hook-up, but it's proven to be deeper.

And we're told it's not like one is pushing the other into getting serious. They're both totally into it.

If this is how Carl's Jr. babe Charlotte McKinney plans to rehearse for her upcoming gig on "Dancing With the Stars" -- there's at least a chance we might be interested in that show again.

Seems someone is going to be walking the plank ... because police in Australia have captured the guy who allegedly threatened a security guard with a knife after trying to sneak onto the set of the new "Pirates of the Caribbean" movie.

The 22-year-old was charged Wednesday with one count of "going armed as to cause fear" ... he'll appear in court on March 27, presumably without his bird.

The guy was taken into custody with the parrot still on his shoulder. Way to stay committed to the bit, pal!

As for the actual movie ... shooting is being interrupted slightly after Johnny Depp mysteriously injured his hand (not during shooting) and flew back to the U.S. for surgery. The injury is not expected to affect the release date.

The "American Idol" host was spotted out this weekend in NYC with a new blonde -- Renée Hall.

Hall (who is ... shocker!!! ... a model) met Seacrest, fresh off his breakup with model Shayna Taylor, at his 40th birthday party in Napa a few weeks ago.

We're told they met through the Marciano brothers (GUESS founders) and quickly hit it off.

Seacrest took Hall to NYC over the weekend, where they went to the Museum of Modern Art.
