TMZ spoke with a rep at the venue who told us the strings that were attached to P!nk's harness were not secured properly ... which resulted in the singer falling roughly 8 feet onto the arena floor.

P!nk suffered injuries to her legs -- but got up and went back on stage, where she apologized to the crowd ... then got into an ambulance and was whisked to a local hospital.

Sheriff's spokesperson Steve Whitmore tells TMZ, "We would like to speak to Mr. Gibson," adding his department has extended an invitation "but we have not gotten a definitive answer."

As TMZ first reported ... Gibson's lawyers have said they might make their client available at some point in the future.

TMZ broke the story ... Dr. Ross Shelden will be a critical witness if criminal charges are filed against Gibson. Oksana's lawyers asked Dr. Shelden last month to write a declaration detailing information he had about the incident and aftermath.

TMZ has learned what's in Dr. Shelden's declaration. Dr. Shelden says he has known Oksana for 12 years. According to the declaration, Oksana came in the day after the incident and told him Mel had hit her, but she was not more specific. The doctor says in his declaration he saw evidence of "blunt force trauma" as well as physical damage.

Something significant -- in the declaration, Dr. Shelden says Oksana's teeth were not damaged ... just her veneers. He says one veneer was knocked completely out and the other was broken in half.

The doctor wanted to take pictures of Oksana's mouth, but not to create a record for authorities -- he wanted them for his lab for the purpose of repairing the damage.

The doctor says Oksana did not want the pictures taken, and had expressly said she wanted to protect Mel. But Dr. Shelden persisted and got the photos, although he says Oksana was so reluctant she would not look into the camera.

As we previously reported, Oksana insisted that Dr. Shelden keep the information confidential, which is why he did not contact the police.

Levi and Bristol's story is a tale of classic romance ... he knocks her up when she's 17, she has a kid (Tripp), they break up, they attack each other in the media, he poses for Playgirl, they attack each other in the media again ... and then they reconnect during custody negotiations about three months ago.

But the best part: The couple claims they didn't tell Bristol's mom -- Sarah Palin -- about the engagement first. Instead, they told Us Weekly ... and hoped that it was one of the "variety of sources" she reads.

Levi and Bristol claim they hope to get married in the next six weeks -- and due to Bristol's abstinence pledge, they won't be having sex again until after they say "I do."

As for the wedding -- they want to get married in Alaska ... and Bristol wants Levi to wear a camouflage vest.

The alleged incident occurred Sunday night, at the Crowne Plaza hotel in Seattle. Sources tell us two women showed up for a party in one of the hotel rooms. Kingston was present.

Law enforcement sources tell TMZ ... the women were both intoxicated when they arrived, with bottles of vodka and beer in hand. We're told one of the women claims Kingston had sex with her, and, as one source says, "She was very aggressive." Kingston then left the party and went to another room on the same floor.

Sources say the woman then proceeded to have sex with two other men in the room.

The second woman, we're told, also had sex with someone in the room.

A hotel manager tells TMZ "an incident" did indeed occur in the hotel Sunday night, though he was not specific. The fire department tells us an ambulance was called and transported both women -- 19 and 20 years of age -- to a hospital where rape tests were conducted. We're told the women were also tested for Rufinol, but the results are not in.

Sources say Kingston, who is scheduled to perform tonight with Justin Bieber in Portland, was initially asked by cops to come in for an interview, but police have determined the case is not credible and they now say an interview is unnecessary.

We're told one of the women is going to file a civil lawsuit, and maybe both.

The Official Michael Jackson Fans of Southern California have extended an olive branch to Forest Lawn Cemetery -- where MJ is buried -- offering to clean up the recent wave of hidden vandalism outside the singer's tomb ... before the cemetery decides to ban MJ fans altogether.

A rep for the group -- which organizes a monthly pilgrimage to MJ's burial place -- tells TMZ, they want to "fix this mess" so FLC doesn't associate MJ's upstanding fans with a bunch of sharpie-wielding hoodlums.

As TMZ first reported, Forest Lawn is currently on red alert over the illegal doodles -- hunting down the artists responsible ... and threatening to ban any potential copycats for life.

No word if Forest Lawn will take the fan group up on their offer.

As you can see from the pics, Shaq turned up the heat at PURE Nightclub inside Caesars Palace -- dropping all sorts of rhymes ... and several buckets of body moisture.

Sources tell us Alexander Dalton saw many of the blowups between Mel and his mom, both before and after the explosive January 6 showdown. Alexander -- whose dad is actor Timothy Dalton -- lived with Mel and Oksana while they were together, and is considered "a very important percipient witness" to Sheriff's investigators.

Based on the information we know, Alexander did not witness when Mel allegedly struck Oksana, but he may have been in the vicinity, heard the argument and saw the aftermath.

And we're told Alexander -- who is sometimes called Sacha (Russian for Alexander) -- is also a critical witness in the couple's custody battle.

According to the report, a woman named Nan Fowler phoned the pro shop at the Country Club at Muirfield Village around 6:53 PM and complained that a white, tall male -- sporting a blue golf shirt and khaki shorts -- was "urinating into some trees."

The report states that Nan was informed by the woman at the pro shop that the person who fit that description was Big Ben. Nan then called police and told them that the NFL star was peeing in public.

Cops eventually arrived to the scene and spoke with country club management -- and later determined that no crime had been committed. Sources connected to the investigation tell us cops never interacted with Roethlisberger -- because Ben was "long gone" by the time authorities had arrived.

TMZ spoke with reps at Muirfield who tell us, "We had a full slate of golfers that day ... it may have been anyone."

Of course, this isn't the first time Ben's penis has been at the center of controversy -- earlier this year, he was accused of raping a young woman inside a nightclub bathroom. That case was ultimately dropped after investigators determined there wasn't enough evidence to proceed.

So far, no comment from Roethlisberger's camp.

Sources familiar with the treatment tell TMZ Dr. Shelden, who practices in the San Fernando Valley, took photos of Oksana's mouth the day after the January 6 blowout, in which Mel allegedly knocked out Oksana's teeth.

We're told Dr. Shelden took photos of Oksana's mouth and turned them over to her lawyers. We're also told Dr. Shelden has made it clear ... it was not a case where Oksana lost her veneers, which Gibson's people have claimed. We're told Dr. Shelden's analysis is that Oksana lost a tooth and another was chipped.

TMZ broke the story ... Oksana told her dentist -- whom we now know is Dr. Shelden -- that Mel knocked her teeth out, yet he did not report the incident to authorities which he is required to do by law. We now know why Dr. Shelden didn't report the incident -- before telling him about Mel, Oksana insisted on confidentiality and Dr. Shelden felt he was duty-bound to honor it.

Sheriff's spokesperson Steve Whitmore tells TMZ, "We are supposed to be getting the tapes this week." Whitmore says the tapes are supposed to be turned over voluntarily, adding there have been no subpoenas issued for the tapes by his department. And the D.A.'s office has not issued a subpoena.

TMZ broke the story ... the Sheriff's Department is investigating Gibson for 3 alleged crimes -- domestic violence, assault with a deadly weapon and child endangerment.

Turns out ... Right On Programs is currently being looked at by the L.A. County Department of Public Health as part of its twice annual review. The review started this week, but a rep for the county says the timing is just a coincidence. Riiiiiiiiiight.

We're told the review will focus on whether the program turned over accurate records on all their cases. If they uncover problems, they will work with Right On to correct them -- but if the problems are really bad the county could refer the matter to the state to shut Right On down.

As for Michael Lohan ... he tells us he wants to see the program shut down entirely. He says he thinks Right On falsified records and gave preferential treatment to their celebrity clients ... much to their detriment (see: Lohan, Lindsay).

Right On had no comment.

In a harshly-worded statement, Jackson took Gilbert to task for his open letter to Cavaliers fans which slammed LeBron's decision to sign with the Miami Heat. Jackson says, "He speaks as an owner of LeBron and not the owner of the Cleveland Cavaliers. His feelings of betrayal personify a slave master mentality. He sees LeBron as a runaway slave."

He goes on, "LeBron is not a child, nor is he bound to play on Gilbert’s plantation and be demeaned. He has been a model citizen and has inspired the children of Akron, Cleveland, the State of Ohio and the United States."

Not sure if Cleveland fans would rush to LeBron's defense right now.

Sources close to Lindsay tell TMZ she would go to six different doctors for prescriptions. One source says, "When one doctor says no to refilling a prescription, she will go to the next. It’s a whole process to get what she needed.”

We're told Lindsay has doctors both in Los Angeles and New York -- even one of her past rehab facilities still prescribes her meds.

As we first reported, Lindsay has prescriptions for: Zoloft (antidepressant), Trazodone (antidepressant), Adderall (stimulant to control ADHD), Nexium (acid reflux) and the extremely powerful painkiller Dilaudid.

You'll recall Judge Marsha Revel ordered Lindsay to do 90 days in jail, followed by a 90-day inpatient rehab program. But we found out there's a program in L.A. County for inmates with drug and alcohol problems -- a program that combines jail with rehab.

It's called the Impact Program -- the brainchild of Mothers Against Drunk Driving. Here's the drill ... according to L.A. County Sheriff's Department Spokesperson Steve Whitmore, the inmates go to jail, where they attend an intense 12-step program for five hours a day, seven days a week. The program takes approximately 90 days to complete.

Judge Revel has not ordered Lindsay to attend the Impact Program, but if the psychiatrists who are evaluating LiLo believe it's important to integrate jail with rehab, the judge could modify her order.
