Kate Major had to be rushed from her Florida rehab facility to a local hospital after suffering a seizure ... TMZ has learned.

According to our sources, the episode went down early Monday morning. We're told Michael Lohan's baby mama had a seizure and complained of chest pains and was taken to JFK Medical Center near West Palm Beach.

Major was ordered back to rehab by a judge earlier this year following a DUI arrest.

She had a similar incident in 2011 when she was also taken to the hospital from rehab after suffering a seizure.

Lindsay Lohan has sworn under penalty of perjury she suffered a miscarriage ... which means she better be telling the truth, or she could land in jail.

Lindsay made the declaration in new court docs in a $5 million lawsuit over her clothing line 6126.

You'll recall, Lindsay was given a reprieve from responding to the lawsuit while she was in rehab last year -- but even after getting out she failed to respond ... so she lost the case.

Lindsay wants to undo the damage and she's using her miscarriage as an excuse.

In the new court docs, Lindsay writes, "I have been overwhelmed since leaving rehab and dealing with my sobriety and a miscarriage."

There's been tons of speculation that Lindsay made up the miscarriage story to garner buzz for her Oprah show -- or maybe just to get sympathy votes -- but these are legal documents ... which means she's under oath.

If it comes out that Lindsay lied in the docs ... under penalty of perjury ... she could face jail time.

The-Dream's baby mama photographed her injuries immediately following her alleged fight with him last year and the graphic pics are now part of the police investigation.

As TMZ first reported, Lydia Nam is accusing Dream of punching, kicking and strangling her during an argument when she was pregnant with his child.

Now TMZ has obtained the photos, which show giant bruises on her arm and leg, as well as scratches on her neck and face.

According to the criminal complaint, Dream pulled and dragged her by her hair, choked her with a necklace she was wearing by twisting it tightly around her neck, kicked and punched her in the head and then grabbed her by the neck, pinning her to the wall and choking her again.

The complaint also alleged that when Nam tried to call 911, he violently twisted her arm and said he would kill her “like O.J. Simpson killed Nicole Brown."

Nam says she waited seven months to report the incident out of fear for her safety. The-Dream denies the whole thing.

"Baywatch" actress Brooke Burns MANGLED her BMW Thursday near Universal Studios ... thankfully, there was a guy in his PJs to give her a shoulder to cry on.

Witnesses tell TMZ ... Brooke was driving in North Hollywood when she slammed into the passenger side of a Toyota Matrix.

Police and fire units rushed to the scene ... but no one was taken to the hospital. Both cars were undriveable and a tow truck had to be called.

Brooke is built better than most to withstand injury ... she has a titanium rod and screws in her neck ... from a swimming pool accident in 2005 in which she broke her neck.

No word on the identity of the pajama man ... or why he was rockin' flannels in the middle of the day.

Lily Allen was rushed to a London hospital Thursday morning after a bout of "projectile vomiting" ... but the singer insists she's fine.

Lily just posted the photo of herself on a hospital bed, plugged up with needles and tubes, along with the caption, "I'm fine though guys, just need some fluids cause I can't keep anything down."

Before she was hospitalized, Lily tweeted she couldn't stop "projectile vomiting" and felt extremely dehydrated. She had just appeared on British comedy show "Chatty Man" before falling ill.

Whatever it is, the sickness definitely appears sudden -- on Monday night, Allen was seen leaving the Met Gala in NYC scarfing pizza and looking healthy.

CHAOS at the Morrissey concert last night ... (yeah, you read that right) ... when the singer was attacked by a fan in the middle of a song, crashing to the ground ... and it was all caught on video.

Morrissey was performing "One Day Goodbye will Be Farewell" in San Jose, CA Wednesday night ... when a bunch of fans broke through security to hug the singer.

At first, Morrissey seemed to be amused by the huggers ... even chuckling at the situation ... but then some ahole ruined it for everyone when he came after the singer HARD ... knocking him to the ground.

Security rushed in ... and the band immediately stopped playing.

Unclear if Morrissey was injured in the incident or if the fan was arrested.

Rich Homie Quan was hitting the lean hard before suffering a seizure during a video shoot.

TMZ broke the story ... Quan was rushed to the hospital yesterday in ATL after he collapsed, hitting his head on the ground and suffering the seizure. His rep called it heat exhaustion.

But sources close to the rapper think lean -- aka sizzurp -- may have played a part. We're told Quan was on a lean binge for days, and stopped just 3 days before the seizure.

The drug can reportedly stay in your system for up to 4 days -- even longer depending on the dose ... and we're told he was a heavy abuser.

Quan's not shy about his love for lean ... he's posted videos that show him mixing lean on an airplane and in a shopping mall.

It's unclear if he's willing -- or able -- to connect the dots.

Mirrors are usually Adriana Lima's best friend, but this afternoon in New York City ... they were her mortal enemy.

Lima was shooting a Maybelline commercial in SoHo Tuesday as she walked around her trailer to get to the door ... all while a gaggle of photogs snapped away.

As Lima turned the corner, she ran SMACK into the bus mirror and hit the ground like a ton of bricks. Or, considering Lima is a supermodel, more like 90 pounds of bricks.

8:25 PM PT -- Quan's rep tells us the rapper fainted after experiencing heat exhaustion... and then suffered a seizure. We're told he is currently in "good condition," and is recovering in the hospital.

Rich Homie Quan just got rushed to a hospital after suffering TWO seizures during a music video shoot ... TMZ has learned.

According to Quan's rep ... the rapper was shooting a video with Problem in Atlanta's Piedmont Park, and about an hour ago he suffered back-to-back seizures.

We're told when Quan hit the ground ... he smashed his head badly, paramedics were called ... and he was taken to an Atlanta hospital for emergency treatment.

Quan -- one of the hottest new rappers -- was just featured on XXL mag's 2014 Freshman class issue.

Bloody photos from the brawl allegedly involving Chris Brown, his bodyguard, and the D.C. man they're accused of brutalizing have just been released -- and they're gnarly.

A judge approved the release of the photos Tuesday -- the pics were entered into evidence by prosecutors during the recent trial of Brown's bodyguard Christopher Hollosy.

The bloody face belongs to Parker Adams, the man who says both Brown and Hollosy went ballistic on him in October outside a D.C. hotel -- punching him in the face for attempting to photobomb a pic Brown was taking with two girls.

The Ringling Bros and Barnum & Bailey circus will keep chugging along after a devastating accident Sunday involving its high-flying "Hair Hang" act -- but the ""Hair Hang" act won't be coming along ... a rep for the circus tells TMZ.

The rep tells us, 8 of the 9 acrobats who were injured during the accident -- when a rigging structure failed, dropping the acrobats 30 feet into the center ring -- are still in the hospital ... making it difficult to include the act in upcoming performances in Hartford later this week.

The rep tells us, the girls involved in the accident are the only ones trained to do the act, so the act had to be cut while they recover.

We also spoke to a spokesperson for the circus in D.C., who gave us some interesting trade secrets behind the "Hair Hang" act ... like the fact that the performers weave a metal loop into their hair to help them stay suspended. And there's more where that came from.

No one was more shocked by the return of Valerie Harper's cancer than Valerie Harper -- so she insists in new legal docs, claiming the Broadway playwright who accused her of keeping her cancer a secret is wrong beyond measure.

TMZ broke the story, Matthew Lombardo -- the playwright behind the Broadway production "Looped" -- sued Harper for $2 million, claiming she withheld the truth about her cancer's return in 2012.

Matthew claims Valerie's ruse eventually fell apart when she was forced to pull out of the play because of her medical issues ... and it cost him $500,000 to find a last minute replacement.

Harper is now firing back, insisting Matthew's talking out of his ass. Harper claims Lombardo found out about her cancer's return the same time she did, so his lawsuit is baseless.

At the time, Harper claims she believed her cancer had been successfully removed during a prior surgery, and when her health began to deteriorate in early 2013, she thought she had suffered a stroke. It was only later she discovered her lung cancer hadn't just returned, it had spread to her brain.

A "Ringling Bros. and Barnum & Bailey Circus" stunt went horribly wrong Sunday in Rhode Island when a high wire snapped ... sending multiple performers crashing to the ground ... and the frightening accident was caught on video.

The performers were about to begin an aerial stunt in front of a packed house ... when the wire snapped, and several people fell about 30 feet.

11 people were transported to the hospital -- 9 were Ringling Bros. employees -- and according to reports, one person is in critical condition.

Moments after the crash, emergency responders raced to help the injured -- and the lights were dimmed to keep the children in the audience from seeing the terrifying scene.

A dancer claims a disheveled and confused Britney Spears backhanded her in the face during a rehearsal, and Brit was so heartless ... she didn't even stop the rehearsal.

Dancer Dawn Noel will file a lawsuit Monday ... claiming she was hired to shoot the music video for "Work Bitch." Dawn says on August 19, she showed up at the rehearsal studio in Woodland Hills, CA. She and the other dancers perfected their moves for several hours, and then Britney arrived.

Dawn says Brit showed up "in a disheveled and confused state," and had considerable difficulty executing even basic moves.

Dawn claims she was standing off to Britney's side, when the singer twirled while unbalanced, extending her hands and SMACKING Dawn in the face.

Dawn says the impact was so forceful, everyone in the room heard a cracking sound. Dawn says she was in excruciating pain, but all Britney said was, "I'm sorry," and then continued the rehearsal.

Dawn claims after a half hour she couldn't take the pain and went to a doctor, who told her she had a nasal bone fracture and needed surgery.

Dawn claims Britney's people promised to pay the med bills, but after months of waiting she was told to pound sand. So Dawn's lawyer Sark Ohanian from Carpenter, Zuckerman & Rowley will file a lawsuit Monday against Britney.

We called Britney's people ... so far, no comment.

Everything's bigger in Texas ... including Bette Midler's big toe, which is now sporting a huge green bandage thanks to a pedicure gone horribly, horribly wrong.

Bette tweeted a pic of her aching piggie majorus a few days ago, adding ... "My New York toes don't love Texas cowboy boots."

But TMZ has learned the problem wasn't boots -- sources close to Bette tell us it was an ingrown toenail caused by a clueless manicurist who recently gave her a bad pedi.

Bette was in Dallas to make the keynote speech at the Jewish Family Service women's luncheon -- and we're told the pain was just too much, so she had the ingrown toenail removed ... (clap clap clap) deep in the heart of Texas.
