Scott Marshall -- champion of "Ink Master" season 4 -- died from a deadly cocktail of heroin and prescription drugs ... TMZ has learned.

According to the coroner's report ... cops found a small bag of heroin on the nightstand next to Scott's body in the Roselle, IL hotel room.

Cops say Scott was lying prone on the bed, and noted there was a small amount of blood on the mattress underneath his face.

According to the toxicology report ... Scott had Xanax, Valium, and the metabolized form of heroin in his system. The coroner concluded his death was an accident due to intoxication.

The coroner noted Scott had multiple "red crusted abrasions" on his thigh and foot, suggesting prior intravenous drug use.

TMZ broke the story, Scott was found dead in a hotel room in October near his tattoo shop.

Marque Lynche -- the former Mouseketeer found dead in his apartment -- got into a bizarre fight with an NYPD officer ... just a few months before he passed away.

The ex-Mouseketeer and former "American Idol" semi-finalist illegally jumped a subway turnstile back in August and when cops approached him ... he lashed out, punching one of them in the face and smashing him in the head with a bottle of soda.

Lynche was charged with assault. There's no mention of drugs or alcohol in the police docs.

Mark McGrath, Sugar Ray front man and close friend of the late Scott Weiland says he saw the singer just weeks before his death and he wasn't his normal self.

Mark appears on an upcoming episode of Theo Von and Matt Weiss' "Allegedly" podcast this week and talks about his run-in with Weiland. Mark says he chatted with Scott and the two had a few drinks at a friend's wedding, he says Scott was out of it.

McGrath says Scott took the stage at the ceremony too -- listen to the clip -- it's interesting to hear Mark describe his performance.

Rose Siggins, the actress who played Legless Suzi in "America Horror Story: Freak Show" has died ... TMZ has learned.

A rep for Rose tells TMZ she went in for a kidney stone surgery on Monday and never came out of the hospital alive.

We're told Rose contracted an infection, which spread through her body and ultimately became fatal.

She died Saturday morning in a Denver hospital.

Rose was born with a rare genetic disorder called sacral agenesis and we're told she suffered from kidney and pancreas issues her whole life.

Rose was 43 ... she leaves behind two children. A GoFundMe account has been set up for them.

Marjorie Lord, who starred in one of the first big TV hits and became a major philanthropist, has died.

Lord starred opposite Danny Thomas in the 1957 show, "Make Room for Daddy." The show premiered on ABC, where viewers watched Thomas court her, and then it moved to CBS when the pair married.

Lord did lots of TV shows and movies, but became extremely active in the arts in L.A. Her third husband, banking honcho Harry Volk, founded the Music Center.

Lord was 97.

Former Atlanta Braves pitcher Tommy Hanson died of cocaine and alcohol toxicity ... according to the autopsy results.

Hanson, who was only 29, died on November 9th -- he was found unresponsive at a friend's home in the Atlanta area. At the time of his death, the coroner listed "overdose" as a possible cause of death.

We spoke with the coroner who tells us the autopsy showed Hanson died from the combination of cocaine and booze.

It appears Kristin Cavallari's brother did not commit suicide, and most likely died simply from exposure to the elements ... according to law enforcement.

Michael Cavallari's body was found 3 miles from where his car wrecked in the high desert of Grand County, Utah. Law enforcement sources tell us there were no signs of major trauma and no suicide note. Cops did not find any evidence of drugs or alcohol either.

We're told temperatures in the area were frigid -- in the teens at night and 30s during the day -- and the operating theory is that's what killed Cavallari. He was missing for 2 weeks.

It's still unclear how he crashed -- his car was found about 100 yards off the road ... and he might have been walking for help. We're told he left his mobile phone in the car, but service in the area sucks due to the terrain.

Official cause of death won't be determined until toxicology testing comes back.

Kristin looked distraught Thursday afternoon in Chicago, hours after getting the news.

2:45 P.M. PST -- TMZ obtained a police report from Orange County, where Michael was arrested for criminal threats just 5 days before disappearing. According to the report, Cavallari showed up to his girlfriend's apartment with a loaded shotgun after a verbal dispute, and made threats. Cops also say they smelled a strong odor of alcohol on his breath. He was released 2 days later, and then disappeared.

1:23 P.M. PST -- Law enforcement tells TMZ they located remains they believe to be Michael at 10:30 A.M. They say the body was found in a steep and very rocky area.

Kristin Cavallari's brother and Bears QB Jay Cutler's brother-in-law has been found dead in Utah.

30-year-old Michael Cavallari's car was found abandoned November 27th. It appears he wasn't reported missing until 3 days ago.

In a statement to TMZ, Kristin says, "We have just been informed by the authorities my brother's body has been found. This is a very painful time and we are still processing it all."

It's unclear how Michael died, but he's struggled for many years with alcoholism and has had more than his share of trouble.

Scott Weiland's tour bus was a pharmacy on wheels ... police found a stash of drugs on board that ran the gamut from hardcore narcotics to prescriptions to ED meds.

TMZ obtained a copy of the search warrant Bloomington, MN cops got to search Weiland's bus after his death -- and it shows they found at least 2 bags of a white substance. They've already said that substance tested positive for cocaine.

Also on the bus -- a generic version of Xanax, 2 different brands of sleeping pills, Buprenorphine ... a synthetic opiate painkiller, and Viagra. There was also Ziprasidone ... which is used to treat bipolar disorder and schizophrenia. Scott was diagnosed as bipolar in 2001.

According to the docs, all of those drugs were found in a bedroom on the bus. We know Scott's body was found in a bedroom ... although the document does not mention his name.

Cops also found one bag of a green, leafy substance.

Darel Evans -- an anti-violence activist who was featured in the Sundance documentary series "Brick City" -- was shot to death late Monday night in Newark, NJ. He was 33.

So far, authorities are investigating the motive and have released no details about the incident so far. No arrests have been made and no suspects have been identified.

Evans, a former gang member who went by the name Creep, was profiled as part of the award-winning TV series.

Scott Weiland emotionally abandoned his 2 children and completely gave up on even trying to be their parent ... according to his ex-wife.

Mary Weiland wrote a scathing open letter to Rolling Stone about Scott's death. She pulled no punches, saying ... reports Scott had been drug-free for years and loved spending time with his kids were false. She says the truth is Scott "was a paranoid man who couldn't remember his own lyrics and who was only photographed with his children a handful of times in 15 years of fatherhood."

She also said she can't say he's in a better place now because, "He belongs with his children barbecuing in the backyard and waiting for a Notre Dame game to come on."

Mary added, Scott's kids -- Noah, 15 and Lucy, 13 -- "never sought perfection from their dad. They just kept hoping for a little effort."

One of Britney Spears and Ryan Gosling's former 'All New Mickey Mouse Club' co-stars was found dead in his NYC apartment ... TMZ has learned.

According to law enforcement sources Marque "Tate" Lynche's roommate found him on Sunday afternoon. He was only 34 years old. The medical examiner's office conducted an exam of the body on Monday, but hasn't determined a cause of death yet.

Lynche was a Mousketeer from 1993 to 1995 -- and shared the camera with Britney, Ryan, Justin Timberlake and Christina Aguilera. He later appeared on Season 3 of "American Idol" and made it to the semi-finals.

Tiffany Thornton's husband, former MTV star Chris Carney, was so wasted before Friday's fatal car crash ... he probably didn't think twice about getting in a car driven by a guy who was liquored up.

People at The Tavern in Little Rock tell us, Chris started off with beer and moved to the hard stuff. We're told he was slamming shots of Fireball Whiskey and doing Vegas Bombs to the point he couldn't stand up.

Chris was in no position to judge the sobriety of the driver, his friend Ezekiel, who we're told had been drinking beer and downing some shots.

As we reported, Carney and Ezekiel died early Friday after their SUV drifted off the road and struck a tree. We're told neither were wearing seat belts.

Chris struggled with alcohol over the years but his mom tells us she believed he had been sober for a year.

Scott Weiland had semi-strict rules in place to prevent him from falling off the wagon during his tour, but there was a gaping hole in the plan.

TMZ got hold of a backstage rider from Weiland's management team, which requires "sober stage hands" for his shows.

Members of the Wildabouts got a full stocked bar in their dressing rooms -- everyone except Scott. They got whiskey, vodka and beer. Here's what's curious ... the rider says Scott gets a bottle of Pinot and a bottle of organic vodka.

Scott was a hardcore addict and just about every respectable addiction expert out there says addicts should stay away from drugs and alcohol. It's weird Scott -- who fought a losing battle for much of his adult life -- was provided booze that could trigger a relapse.

Our sources say the no-drug policy was jettisoned once the band was on the road, and that explains how cocaine ended up in Scott's bedroom on the tour bus.

The apparent relapse was underscored by accounts we got from some big musicians, who say Scott was "drunk texting" them over the last few days of his life.

Tour merchandise for Scott Weiland's band that seemed really cool yesterday is now just really tragic ... yet still available for purchase.

The gear features Scott's picture with the slogan "Not Dead and Not For Sale" -- playing off the rumors that frequently swirled prematurely about the rock star's death. It was also the title of a memoir written about Scott.

Arguably, the clothing is more valuable now after Scott's death ... something fans clearly realize since there's a bunch of new listings on Ebay for shirts and sweatshirts with the phrase.

Scott's own Amazon store is still hawking the stuff too ... if you want to pay retail.

Actor Robert Loggia -- who played drug kingpin Frank Lopez in "Scarface" -- is dead.

Loggia, who was nominated for Best Supporting Actor for "Jagged Edge," died at his home at noon Friday.

Loggia's wife, Audrey, tells TMZ her husband was struggling with Alzheimer's for the last 5 years. She says things took a drastic turn for the worse Wednesday, he seemed to rebound and then died suddenly and peacefully at home.

Audrey says she and their daughter Cynthia were at Robert's bedside when he passed. The couple had 4 kids. They've been married for 33 years.

She says, "He was an awesome husband and an awesome father."
