Danny Dias 'Road Rules' Star Dead Possible Suicide, OD
9:00 AM PT -- According to cops, Danny was found with 2 vertical lacerations on his forearm. We're told police are now treating it as a possible suicide.
Danny Dias, who starred in season 13 of MTV's 'Road Rules,' has died and it appears drugs were involved ... TMZ has learned.
We're told Dias was at his Brooklyn apartment Saturday with a friend. Law enforcement sources say the friend told them he saw Dias take what he believed was LSD and mushrooms. The friend says Dias freaked out, ran in his room and that was the last the friend saw of him.
We're told the friend left the apartment and over the next 2 days called Dias, getting no response. The friend became concerned and on Monday called 911 for to get a welfare check on Dias. First responders had to force their way in, and found Danny dead in the room.
Dias appeared on the 2004 season.

NFL Hall of Famer Cortez Kennedy was hospitalized for swollen legs just days before he died ... and the information could hold the clue as to why he passed away.
TMZ Sports obtained the police report in the 48-year-old's death -- which says Kennedy had called a friend the night before he died because he was concerned about his health.
During the call, Kennedy said he was feeling dizzy and asked the friend to check on him in the morning to make sure he was okay.
Sadly, by the time the friend went to Kennedy's Orlando, FL home on May 23, he was already dead.
Investigators say medical paperwork found in Kennedy's home confirm he was hospitalized from May 10 to May 15.
Though a cause of death has not been revealed, it wouldn't be far-fetched to link his death to his hospital stay the week before. Swollen legs can sometimes be a symptom of anything from a blood clot to diabetes to heart failure.

Ghost Ship Fire Landlord Arrested for Fatal Blaze
Derick Almena, the landlord at the Oakland warehouse that went up in flames, killing 36 people, has been arrested for the fire.
Alameda County prosecutors announced Almena's arrest -- along with Max Harris, who was reportedly a tenant at the art commune known as the Ghost Ship.
Both men are being charged with 36 counts of involuntary manslaughter. The District Attorney says they "knowingly created a fire trap." His bond is set at $1,080,000.
The warehouse burned down in December during an EDM concert. Based on photos and videos, the building was packed with tons of old art pieces, furniture and wooden cargo pallets -- all of which fueled the flames.
Almena had angrily claimed the building was "up to code" during an interview on NBC's 'Today.'

Chris Cornell Toxicology Report Rx Drugs in System
12:55 PM PT -- Vicky tells us, "Many of us who know Chris well, noticed that he wasn’t himself during his final hours and that something was very off. We have learned from this report that several substances were found in his system. After so many years of sobriety, this moment of terrible judgement seems to have completely impaired and altered his state of mind."
She added, "Something clearly went terribly wrong and my children and I are heartbroken and are devastated that this moment can never be taken back. We very much appreciate all of the love we have received during this extremely difficult time and are dedicated to helping others in preventing this type of tragedy.”
Chris Cornell had several prescription drugs in his system when he hanged himself in a Detroit hotel room ... according to the toxicology report.
According to the report, obtained by TMZ, the singer had Naloxone (Narcan), Butalbital (sedative), Lorazepam (Ativan), Pseudoephedrine (decongestant) and barbiturates in his system at the time of death. It appears Chris had taken 4 (1 mg) Lorazepam tablets.
We broke the story, Vicky claims she was on the phone with Chris a half hour before his death ... when he began speaking in tones she hadn't heard since he was addicted to Oxycontin 14 years ago. She was so alarmed by the slurring and tone she asked Chris' bodyguard to break his hotel door down.
As we reported, cops observed fresh track marks on Chris' arm. According to the tox report, he had 4 needle puncture wounds on his left arm, but it appears the marks were from EMTs administering Narcan to counteract an opiate OD.
Family sources tell us Vicky and others are angry the medical examiner "jumped to conclusions" Chris committed suicide. They believe he was so high on drugs he did not really form an intent to kill himself ... he was out of his mind when he put the exercise belt around his neck.
In essence, Chris' family believes this was an accidental death.

Chris Cornell Family Not Ready to Buy Suicide Story
Chris Cornell's family just got shut down by the City of Detroit, which refused to release information surrounding the singer's death, because the City claims it's still "an open investigation."
We've learned the family's lawyer, Kirk Pasich, sent a Freedom of Information request to the City asking for the police and toxicology reports. The City nixed the request, saying releasing the info would "compromise and/or interfere with their investigation."
The family says it's "mystified" the Medical Examiner announced to the world Chris' death was a suicide when the full autopsy hasn't been completed.
Chris' wife, Vicky, says the family still has several unanswered questions about what led to her husband's death and she thinks the toxicology reports will answer those questions. The intimation ... maybe Chris was so high on drugs he was not in his right mind and did not consciously try and kill himself.

Paul O'Neill -- the founder of the Trans-Siberian Orchestra -- died from a combination of prescription drugs ... TMZ has learned.
TMZ obtained a copy of O'Neill's autopsy report, and it says he died of intoxication from a mix of methadone, codeine, diazepam (generic Valium) and doxylamine (an antihistamine).
According to the doc, the cause of death is intoxication ... but it also mentions other ailments O'Neill was battling ... such as mild heart disease, hypertension and moderate hardening of the arteries. The medical examiner determined the death to be accidental.
When O'Neill died last month, the band released a statement saying he'd died from a "chronic illness."
Paul was 61.

Michael Nance -- a contestant on season 8 of "The Bachelorette" -- was found dead early Monday morning in Texas.
According to the Austin PD ... the death is not suspicious, but the manner will be determined later by the medical examiner. Officers were called to a residence at 2:10 AM Monday to check on an unresponsive male. They say Nance was pronounced dead less than an hour later.
Michael competed for Emily Maynard back in 2012, before she eventually sent him home mid-season.
Michael was 31.

Gregg performed with Billy Gibbons at the Laid Back Festival in Atlanta on October 29, 2016. He then cancelled a series of concerts, telling his fans, "This is the hardest thing I've had to do in a long, long time. I've been on the road for 45 years because I live to play music with my friends and for my fans."
He went on, "It's time for me to take some real time off to heal."
Gregg died Saturday of liver cancer.

Of course, the members of Cornell's bands, Audioslave and Soundgarden, were also there.
As we first reported ... Chris was cremated Tuesday.
Fans were allowed entry at Hollywood Forever later Friday afternoon.

Chris Cornell's final resting place is right next to another rock 'n' roll legend ... his dear friend, Johnny Ramone.
Chris' ashes were placed in a gravesite Friday ... just 12 feet away from the Ramones' lead guitarist's monument in Hollywood Forever Cemetery. Johnny and Chris were very close, and Chris even referred to his pal as "one of rock's most indestructible icons.
We're told Cornell's family just recently purchased the adjacent plot for around $120k. Hundreds of Chris' dear friends and family members flocked around the site, which only has flowers on it now.
Sources at Hollywood Forever tell us a monument, much like Johnny's but smaller, will eventually be placed there.
