Our sources say the D.A. will make filing decisions on both investigations at the same time. And, we've learned the L.A. County Sheriff's Department is still a ways away from sending the extortion case over to the D.A.

Bottom line -- don't expect to know whether Mel and Oksana will be prosecuted for a while.

The L.A. Coroner's Office just released their report on Simon Monjack's death.

In the report, the investigator documents an exchange with a person whose name has been withheld -- but whom law enforcement sources have identified as Brittany's mother, Sharon Murphy.

According to the report, the investigator and the unnamed person (Sharon) were going through the master bedroom in the Hollywood Hills home that Simon shared with Brittany ... when (Sharon) pointed to one side of Simon's bed and identified it as "her side of the bed."

Sharon also told the investigator that certain prescriptions in the nightstand -- bearing the names Sharon Murphy and Sharon Monjack -- were also hers.

Sources connected with the treatment of the dog tell us, the dog is stable and should recover from her eye and paw injuries. One person from the clinic where the dog was taken after the crash told us, "The CHP was instrumental in saving the dog, by taking her to an emergency facility that was equipped to handle her injuries."

We just received a press release from the O.C. District Attorney ... about a man who was just arrested "after ejaculating two times into female co-worker's water bottle while at work."

According to officials, the alleged wackjob waltzed into his co-worker's office in January and flogged the dolphin into her water bottle ... which she later unsuspectingly drank. Officials claim she threw the water bottle away after feeling "sick and irritated."

Three months later, officials say the alleged victim and the suspect were both transferred to another branch ... and dude pulled the schtick all over again and the lady drank the water ... again. Only this time, when the woman felt sick ... she saved the bottle and sent it to a private lab for testing.

According to officials, the tests confirmed "that the water bottle contained semen" ... and investigators later connected the DNA to the suspect. The alleged wanker was arrested early this morning.

According to law enforcement, Dr. Ryan's Jeep went over the side of Pacific Coast Highway yesterday at around 4:30 PM. The Jeep landed on rocks and lifeguards initially tried to help Ryan, to no avail.

Ryan, who was 50, was trapped in the vehicle and had major head injuries. The Ventura County Fire Department tried hoisting Ryan using a helicopter, but the attempt was called off. Ryan was pronounced dead at the scene.

Minutes before the crash, Ryan Tweeted, "After 25 years of driving by, I finally hiked to the top of the giant sand dune on the pch west of Malibu. Much harder than it looks! Whew!"

Ryan's last Tweet before he died, "Border collie jill surveying the view from atop the sand dune." Ryan posted the picture below with his Tweet. Law enforcement tells us the dog was injured but survived.

Among Ryan's patients -- Gene Simmons, Shauna Sand, Vince Neil and Adrianne Curry.

Dr. Ryan appeared on numerous TV shows and even performed plastic surgery on the air.

UPDATE: TMZ just received a statement from Janice Dickinson's rep saying, "Janice is deeply, deeply anguished! She is stunned and wants the world to know what a genius Dr. Ryan was."

The rep adds, "He was a friend and my love goes out to his mother and the rest of the family."

UPDATE: According to the CHP, Ryan's 1995 Jeep Wrangler veered off the roadway and overturned down a rocky embankment toward the ocean, coming to rest on its roof. Ryan was pronounced dead by paramedics. Law enforcement sources tell us Ryan's dog was found in the ocean and seriously injured. The dog was taken to a local veterinary hospital.

The front passenger wheel area is a total mess -- with serious cosmetic and functional tire damage ... which is why the car had to be towed from the scene.

Montana says Laurence told her she is unwelcome in his life and wants nothing to do with her ... at least for now. According to Montana, her dad said, "I'm not going to speak with you 'till you turn your life around." Laurence was referring to his daughter joining the ranks of Vivid Entertainment as a porn star.

Montana says Laurence added, "You embarrassed me."

And this parting shot. According to Montana, Laurence said, "You used your last name. No one uses their real name in porn."

According to an eyewitness, Method Man and Redman were performing at the festival Sunday night when someone in the audience threw what looked like "a full beer can" at MM's face -- resulting in a pretty nasty eye wound.

In our video, shot moments after the incident, Redman throws expletives at the crowd before telling Method, "Your face is cut, go take care of that."

But MM refuses to stop the show -- bloody eye and all -- and yells out, "Start the music we're continuing the show, let's f**k these motherf**kers up."

TMZ spoke with ICP member Violent J -- who claims the group informed Tila about a rumor circulating within the Juggalo community that Tila was going to be a target when she performed on Friday night.

Violent J tells us, "We told her about the rumor and she didn’t care" ... noting that Tila informed the group that she wouldn't back out of the performance because she's "not a bitch like that."

Violent J continued, "We paid her before the show and assured her that even if things got out of hand, she could keep the money if she had to leave the stage."

Obviously things did get out of control -- and Tila took a few shots to the face when the crowd began firing objects at the stage.

J tells us Tila's people have already informed the group that a "lawsuit is underway" -- but ICP insists, "We're not gonna sit back and give her our money.”

Still, Violent J tells us he thinks it's "f*cked up" that the crowd got violent ... saying, "I wish they didn’t throw stuff at her.”

Multiple sources pretty damn close to the couple tell us the two were seen getting all touchy-feely with each other at the Mandalay Bay hotel ... and had a romantic dinner at N9ne Steakhouse at The Palms.

But here's the rub -- if you know what we're sayin' -- they stayed at another very high end Vegas hotel.

Kat Von D's people are playing the "just friends" card -- but we're told "friends with benefits" is more like it.

His and her tats can't be far behind ... if they can find an open space.

Sources tell TMZ it happened at 8:35 PM in Malibu. Gibson, we're told, lost control of his car and ended up striking the rocky hillside on Malibu Canyon Road.

Gibson was driving a 2008 Maserati, which had to be towed from the scene.

Hours before the crash -- photogs shot footage of Gibson driving the Maserati around Malibu (see video above).

According to the CHP report, obtained by TMZ, "For unknown reasons, Mr. Gibson steered his car to the right and struck the rock hillside. He stopped his car at the scene and was contacted by officers from the West Valley CHP Area."

CHP Public Information Officer Leland Tang tells TMZ ... "Gibson told officers in the field he did not know how the car drifted out of the lane and into the hillside." Tang said his officers followed up by asking Gibson if he was on a cell phone and he said no. Tang adds officers also asked Gibson if he had dozed off "but he did not give a firm 'yes' or 'no'." Gibson just repeated several times, "I don't know how the car left the roadway."

The report goes on, "Mr. Gibson supplied all the pertinent information to the officers at the scene, gave a statement, and received a ride home from a friend."

Gibson was not hurt.

According to the report, "DUI is not suspected in the accident."

UPDATE: As for the curious statement in the report ... "For unknown reasons, Mr. Gibson steered his car to the right and struck the rock hillside." Tang tells TMZ his department was in no way trying to insinuate Gibson did it intentionally. Tang says, "It was totally an unintentional act." And Tang says no citation was issued and no charges will be filed.

Our sources say Lindsay's doctors have not written their final report, but they have already concluded her psychiatric problems are not nearly as severe as first thought. We've been told there were indications Lindsay was bipolar but that proved not to be the case.

In addition, the doctors do not believe the drug dependency issues are as severe as first thought. As we reported, Lindsay had big issues with Adderall, but the docs don't think it requires much more inpatient care.

Bottom line ... sources say it's "very possible" Lindsay is getting out this week and will begin getting outpatient care.

One obstacle -- the judge must approve early release. We're told once the doctors write their final report, Lindsay's lawyer, Shawn Chapman Holley, can go back into court and ask the judge to spring her client from her 90-day rehab sentence.

If the medical professionals think releasing her is a good idea, the judge will almost certainly go along with it.

According to well-placed industry sources ... Stone Entertainment -- a powerhouse in the reality TV world -- is going after Slater to host a show in which various disgruntled workers quit their jobs in extravagant ways.

But reality TV is a fickle thing ... if Slater's story turns out to be a big hoax -- which we're thinkin' is the case -- the offer could disappear as fast as it came.

Still, we're told Slater's lawyer has received the offer.

In the video, Tila can be heard screaming "I ain't going nowhere!" as she tries to sing on stage -- but that didn't stop the crowd from continuing to hurl objects at her. At one point, comedian Tom Green comes on stage to try and defuse the situation. It didn't work.

By the end of the performance, Tila is "singing" -- or is it rapping? -- behind five bodyguards to avoid flying objects.

Sources say Violet Kowal claims she was communicating with Mel on January 6 about his blowup with Oksana Grigorieva.

In the days following -- January 7, 8 and 9 -- Violet Kowal claims she was contacted by one of Mel's bodyguards. We're told Kowal says the bodyguard sent text messages, warning her that she couldn't talk about the incident and that she should "disappear" for a price.

And sources say Kowal -- who is now repped by Gloria Allred -- has phone records and text messages.

According to Tila, she took the stage at the Gathering of the Juggalos in Illinois -- a concert featuring such acts as Insane Clown Posse and Kottonmouth Kings.

Tila gave TMZ a very detailed account of what happened, saying: "I went onstage and immediately, before I even got on stage, DUDES were throwing HUGE STONE ROCKS in my face, beer bottles that slit my eye open, almost burnt my hair on fire cuz they threw fire crackers on stage, and they even took the sh*t out of the port-0-potty and threw sh*t and piss at me when I was onstage."
