Simon Monjack -- Cause of Death Still a Mystery
We just obtained the death certificate of Brittany Murphy's husband, Simon Monjack ... and the cause of death has not been determined.
According to the document, the cause of death is "deferred." The certificate lists Monjack as "film maker" ... in "entertainment." It also notes his marital status -- "widowed."
The death certificate lists Monjack as 40 years old.

Lindsay Lohan Mum On Drug Use
Lindsay Lohan refused to say under oath whether she was under the influence of drugs when she went on a wild chase down Pacific Coast Highway in 2007 ... TMZ has learned. And we've now obtained the police report from the incident showing Lindsay's urine tested positive for cocaine.
The lawyer for Tracie Rice, a passenger in the car Lindsay was chasing, claims during her deposition last month, LiLo refused to answer questions "regarding her drug use and/or possession on the day of the incident."
In legal papers obtained by TMZ, Rice's lawyer, Paul Hoffman, claims Lindsay invoked her Fifth Amendment right against self-incrimination and refused to answer the question.
As TMZ previously reported, when police pulled Lindsay over they allegedly found traces of cocaine in her pants pocket. She was booked for possession of cocaine but never charged for that crime. Lindsay eventually pled no contest to two counts of DUI. She also pled guilty to two counts of being under the influence of a controlled substance.
Hoffman claims there's no problem with self-incrimination because the criminal case is over.

Hoffman wants a judge to force Lindsay to answer the question. He also wants the judge to force Lindsay to answer whether she feels remorse over the incident.

Gary Coleman Police Report -- 'Nothing Suspicious'
The police officer who responded to Gary Coleman's home on the day he was hospitalized claims he observed "nothing suspicious" about the scene at the actor's home, according to the police report.
In the report, obtained by TMZ, Officer K Johnson from the Santaquin PD in Utah writes that when he first arrived to the home, Gary's wife, Shannon Price, met him at the front door.
The officer claims Shannon directed him to Gary -- but wouldn't accompany him to her injured husband because she didn't want to "see the blood on the floor."
The officer also notes when he spoke to Gary -- who was sitting on the ground with a towel on the back of his head -- the actor couldn't remember how he injured his head.
According to the report, Shannon told officers that the two had been in bed ... when she asked Gary to go downstairs and fix her some food. Shannon claimed Gary fell shortly after leaving the room.
Eventually, the officer claims Gary was able to walk -- with assistance -- to a waiting ambulance which transported him to a nearby hospital.
Gary died two days later.

Restraining Order Against 'Survivor' Producer's Dad
Bruce Beresford-Redman's father just got slapped with a temporary restraining order -- filed by the general manager of the restaurant Bruce's murdered wife owned ... and she believes Bruce's dad wanted to kill her because she knew secrets about Bruce and Monica.
Norma Paula De Ahlf says David Beresford-Redman entered Zabumba restaurant late Saturday night with his right hand inside of his shirt and asked an employee "in a very threatening way, 'Where is Norma? I want to talk to Norma."
Norma says, "The way he was looking at me with his hand in his shirt as if he had a weapon ... so menacingly, I honestly believed he wanted to kill me."
Bruce's lawyer, Richard Hirsch, previously called the allegation "ridiculous."
But Norma says in legal docs, "I am afraid of him and fear that he will harm me because of all the information I have regarding his son and Monica."
Norma says, several weeks before the fatal trip to Mexico, Monica told her she was going to withdraw "a lot of money from their joint account. In the following several days, she withdrew the money, she changed the locks on their house and threw Bruce's clothes out in trash bags and out into the street."
Norma claims "Bruce knew that I was aware about the extramarital affair ..."
And there's this ... Norma says in her declaration ... Monica told her Bruce and David had taken around $25,000 out of Monica and Bruce's joint account to pay a debt David owed. Norma also says Bruce and David took additional money out of the joint account to buy either stocks or bonds in Bruce's name only.
In Norma's declaration, she claims she was suspicious of the trip to Cancun because it was the only trip Bruce planned during his 11 years of marriage to Monica.
A judge in Santa Monica granted the temporary restraining order, which prohibits David from going within 100 yards of Norma. The order expires on June 22.

Gary Coleman 911 Call -- 'Blood Everywhere'
TMZ has obtained the 911 call made when Gary Coleman fell last week at his home in Utah ... and you hear Gary's wife say, "I'm gagging. I've got blood on myself. I can't deal."
During the call, Gary's wife Shannon Price tells the dispatcher there's "blood everywhere" and that she doesn't know what happened.
Gary is conscious during the call, but Shannon says he's "like, not with it" -- and tries repeatedly to tell him to put pressure on a cut on the back of his head ... but he's too out of it to follow her instructions.
During the call, Shannon tells the dispatcher she didn't drive Gary to the hospital because she's sick and is afraid she'll have a seizure.
Gary -- who ended up suffering a brain hemorrhage -- passed away on Friday after Shannon decided to take him off life support.
He was 42.

Charlie Sheen Will Serve Jail Time for Assault
Charlie Sheen will plead no contest to misdemeanor assault and serve 30 days in jail ... TMZ has learned.
TMZ broke the story Charlie had a choice to either serve jail time or get straight probation -- but probation was just too risky. Charlie will go to Aspen next Monday, enter a no contest plea to 3rd degree assault and immediately begin serving time. The felony menacing charge against Charlie will be dismissed.
Charlie will be sentenced to 30 days but with good time credit it will be whittled down to 17 days, and two of the 17 days will be minimal. Once he completes his jail stint, he'll be a free man -- no probation.
Charlie will also be required to complete 36 hours of anger management before Monday's hearing. He's already completed all but 10 hours, sources say.

Grand Theft at 'The Tonight Show'
How's this for news ... Jay Leno is the victim! Law enforcement sources tell TMZ someone allegedly committed grand theft against "The Tonight Show."
Someone allegedly stole thousands of dollars of wardrobe at an office facility for the show. We're told the clothes were for the band. We're told band members were supposed to go to the facility -- in Hollywood -- for a fitting, but before they arrived someone noticed lots of stuff was missing.
A "Tonight Show" insider told us the threads were for the new band, fronted by Rickey Minor.
The LAPD is investigating. No word if it's an inside job. So far, no suspects.

MJ Mausoleum -- Fans No Longer Welcome
The cemetery where Michael Jackson is entombed is launching a new crackdown on fans -- actively removing people hanging out in front of MJ's mausoleum.
According to sources at Forest Lawn Cemetery in Glendale, security has been informed that MJ fans are no longer permitted to gather on the patio of MJ's mausoleum. Those who loiter will be asked to leave.
It's unclear why Forest Lawn is making the move -- especially because fans have been visiting the burial site regularly since MJ moved in last year.
And now -- with the one-year anniversary of Michael's death fast approaching -- the Justice4MJ fan group is concerned about their plans to gather at the mausoleum on June 25.
A rep for the group tells us they hope Forest Lawn will "take into consideration" the hundreds of fans traveling from all over the world to pay their respects.

Dennis Hopper Dies
Dennis Hopper died at his home in Venice, CA today. He was 74.
We're told his family was by his side when he passed.
Hopper had recently been suffering from prostate cancer and going through a difficult divorce from his wife Victoria.
Hopper is best known for starring in the 1969 classic "Easy Rider."

Bret Michaels -- From Deathbed to 'Idol' Judge?
Bret Michaels returned to the stage last night in Biloxi, MS where Bret dropped a huge piece of info on the crowd -- he might be taking Simon Cowell's seat on "American Idol."

Heidi Montag Calls It Quits with Spencer Pratt
Heidi Montag has separated from husband Spencer Pratt, TMZ has learned.
Heidi's rep tells TMZ, "Heidi is looking to move out due to all the fake bad press that Spencer controls. She's tired of it and is looking for a place and wants to focus on her acting career."
We're told Heidi -- pictured above with Spencer in Malibu last Friday -- is looking for a new place to live ... in Malibu.
As TMZ first reported, Heidi and Spencer called the police on Heidi's mom a few weeks back ... so there's been plenty of turmoil.
A source close to Heidi and Spencer tells TMZ the tension in the relationship has been building for a while now ... "It wasn't just a sudden thing."
Now it makes sense ... On Tuesday, Heidi tweeted, "I am not Heidi Pratt, I am Heidi Montag."
UPDATE: Spencer's sister, Stephanie Pratt, tells TMZ, "I hope whether this statement is true or not that they are both happy in whatever life decisions they make."

Gary Coleman Dies
Gary Coleman has died as the result of injuries he suffered earlier this week, the hospital tells TMZ.
We've learned Gary's wife, Shannon, made the decision to pull life support early this AM.
We're told Coleman died at 12:05 PM MST. He died of a intracranial hemorrhage.
Family members and close friends were at Coleman's side when life support was pulled.
He was 42.
As TMZ first reported, Coleman was rushed to a hospital in Utah after he fell and hurt his head. He was on life support since Thursday.
There will be a press conference at the hospital at 3:00 PM PT.
A rep for Coleman released this statement: "Thanks to everyone for their well wishing and support during this tragic time. Now that Gary has passed, we know he will be missed because of all the love and support shown in the past couple of days. Gary is now at peace and his memory will be kept in the hearts of those who were entertained by him throughout the years."

'Survivor' Producer's Wife - No Custody for My Sister
TMZ has obtained a copy of a will left by the murdered wife of Bruce Beresford-Redman -- it was written just 2 years ago and gives a former "Survivor" crew member "full and complete custody" of the couple's kids if both Monica and Bruce die -- cutting out Monica's own sister.
The will -- signed in 2008 -- supersedes the prior will of 2004. The new will says if both Bruce and Monica die, she asks custody be vested in Steve M. Smalley.
The 2004 will gave custody to Carla Ferreira-Burgos, Monica's sister. But the 2008 will would invalidate the 2004 version.
A man with the name Steve Smalley is listed as a camera assistant for "Survivor: All-Stars." Bruce also worked on the All-Stars show.
Here's the complication. One parent cannot will a couple's children to someone. But it is something a judge would consider, especially if Bruce is out of the picture for any reason.
And it's very interesting that Monica specifically left Carla out of the 2008 will.
The 2008 will was filed in court two days ago by Bruce's father. Carla is challenging the will, calling it "extremely suspect" and claiming Monica's signature does not seem authentic.
