Honey Boo Boo's family dun caused the car crash that left several members of the reality bunch injured ... law enforcement tells TMZ.

The Milledgeville Police Department tells us ... investigators have determined Sugar Bear is to blame for the January 6th accident, because Sugar was behind the wheel and made an unsafe left turn in front of a truck when the two collided.

We're told ... the Boo Boo family admitted to seeing lights on the other vehicle, which means the burden to make a safe turn was on Mr. Bear.

Despite being placed with the blame, Sugar Bear won't be charged with a crime -- it's now a civil issue that the insurance companies will handle.

As for the Boo Boo family, everyone in the SUV -- Honey, Pumpkin, Mama June and Sugar Bear -- have been released from the hospital.

The SUV was towed from the scene ... still unclear if it'll ever run again.

A former MTV reality star says she's the victim of a botched boob job ... and it left her left nipple so messed up, it "oozed" for several years ... this according to a lawsuit obtained by TMZ.

The woman behind the suit is Tiffany Rowe-Storms, who starred on "There and Back" in 2006 with then fiance Ashley Parker Angel from O-Town (they've since broken up).

In her suit, Tiffany claims she went under the knife for a routine breast augmentation surgery back in 2008 ... but something went horribly wrong, and "open sores developed" around her left nipple.

Tiffany claims the problems got worse ... and she was hospitalized for pain in her left nipple in 2010 -- where emergency room personnel discovered signs of staph infection.

In her suit, Tif claims she went to another surgeon in 2012 who found "chronic oozing and infection in her left breast."

Tif says the other surgeon also discovered a strain of staph in her left breast that was so severe, she was "at risk of losing her arm or worse, had it spread."

Honey Boo Boo is mysteriously bailing on public appearances ... and we know the reason -- a smashed Pumpkin.

Boo Boo was a no-show this morning on "GMA" and we're told she won't show up at Katie Couric's show either.

Turns out the kid's family ties are more important than mugging for a camera. She's been spending time in the hospital with big sis Pumpkin, who was badly hurt in the family car accident last week. Pumpkin suffered severe chest bruises, a concussion and weakness in her legs.

She can be annoying, but she's a good sister.

Kanye West just attacked an 18-year-old guy inside the waiting room of a chiropractor's office ... after the guy allegedly called Kanye the N-word ... TMZ has learned.

Kim was at a Beverly Hills medical building when she was swarmed by photogs. We're told the guy allegedly tried to help Kim get in the building, as he started screaming, "F**k these fa**ot-ass n****rs"" -- referring to the paparazzi. Kim then told him it was not appropriate to use the N-word. We're told he then screamed at her, "F**k you bitch. Just trying to help you. Shut up n****r lover, stupid slut."

Apparently that got Kim hot as fish grease ... she watched the guy walk into an office. As they were walking we're told the guy continued berating Kim. She dialed Kanye on his cell, and when the guy realized Kanye was on the phone he allegedly said, "F**k you N****r." Kanye -- who had just arrived at the building -- then met up with Kim.

Sources say Kanye and Kim rushed into the waiting room of chiropractor Richard Hill and found the 18-year-old sitting there. Witnesses say Kanye punched the guy and Kim screamed, "We have it all on tape."

Sources say the massage therapist in the office separated the 2 men. Beverly Hills cops were called and the 18-year-old said he wanted to press charges.

Law enforcement sources tell TMZ ... Kanye is now officially a suspect in a battery investigation and they want to interview him. We're also told the matter will be referred to the L.A. County D.A. for possible prosecution.

Remember -- Kanye is currently facing assault charges involving a photog attack at LAX.

James Avery -- who played Uncle Phil on "Fresh Prince of Bel Air -- was suffering from several major medical problems for years before he died from complications following an open heart surgery, TMZ has learned.

We've obtained the death certificate, filed in L.A., which shows the 68-year-old suffered from severe coronary heart disease -- along with end-stage kidney disease and type-2 diabetes.

Avery's official cause of death is listed as cardiorespiratory arrest -- which basically means his heart stopped.

TMZ broke the story, Avery passed away in a Glendale, CA hospital on Dec. 31 -- roughly 7 weeks after undergoing a bypass surgery in the wake of a heart attack.

The death certificate also says Avery was cremated and his remains were scattered at sea off the coast of L.A.

Former WWE Champ Jeff Hardy's precious little dachshund was RUSHED TO AN EMERGENCY VET LAST WEEK ... but don't worry, the courageous pooch made it out alive.

The dog (named Sophie) injured herself jumping off the back of their couch while playing with the wrestler and his wife ... but the couple assumed she had just sprained an ankle.

After a few days of limping, they decided to take Sophie to an emergency vet ... where doctors said the dog had cracked a bone in her foot -- so they hooked her up with a splint.

In the meantime ... Jeff's wife Beth says they're just focusing on keeping Sophie off the back of the couch.

It's pretty ironic ... "Bold and the Beautiful" star Victor Alfieri -- famous for his flowing locks -- claims he was attacked with a hairbrush ... and now he wants protection from the perp.

Alfieri has just got a restraining order against 47-year-old Christopher Veech, who lives with Alfieri's neighbor. According to docs, Veech beat him in the face last month with a hairbrush ... and has relentlessly harassed him for more than a year and threatened to put him in the hospital.

According to Alfieri, Veech was arrested for battery but released a day later ... and the harassment never stopped.

Alfieri doesn't explain why there's such animosity, but he does say the right side of his face still hurts like a bitch and his right eye now twitches.

Here's the problem. The judge granted the restraining order, but can't order Veech to stay far away because he lives in Alfieri's building. So the order is that Veech must stay 3 YARDS away.

Paula Abdul is seeing red -- that's because she looked down and saw her leg burned to a crisp, and now she's suing the tanning salon that she claims scorched her to a faretheewell.

Paula claims in her lawsuit -- obtained by TMZ -- she went to UVASUN in L.A. back in 2012. She bought a SlimStar infrared body wrap treatment and then apparently went into the tanning bed.

When she came out ... she claims she suffered gross burns over her body, an "especially severe second or third degree burn on a portion of [her] left thigh."

Paula uses her legs to make a living ... come on, she's a dancer.

WWE Hall of Fame wrestler Mae Young is gravely ill ... BUT NOT DEAD ... this according to local reports.

Young -- the only wrestler to compete in NINE DIFFERENT DECADES (her first match in 1939; her last in 2010) -- was hospitalized and placed on life support in South Carolina last month.

The Post and Courier initially reported 90-year-old Young passed away, but later updated the story ... saying the wrestling legend is alive but very sick.

Mae and fellow pioneer female wrestler The Fabulous Moolah returned to the WWE back in 1999 -- when they were both in their 70s -- and were involved in numerous iconic moments like being powerbombed through a table by the Dudley Boyz ... and the birth of "hand."

Mama June appears to be feeling the pain bad after Monday night's car accident -- limping across her front lawn this morning in Georgia.

As bad as June looks, she says Pumpkin got the worst of it.

TMZ broke the story, Pumpkin has severe chest bruises from the accident -- when the family Suburban slammed headfirst into another car -- and has been having panic attacks since.

Mama June claims she also suffered back and neck sprains and Honey Boo Boo hit her head.

We had the driver on TMZ Live today and he said The Boo Boos are not telling the truth when they say he didn't have his headlights on. He claims Sugar Bear was just inattentive when he made his left turn.

The Honey Boo Boo car crash 911 tape is laced with Southern tongue ... a woman urgently calling the dispatcher giving her the coordinates of the crash -- a Lowe's hardware store.

TMZ broke the story ... Boo Boo's fam hit an oncoming truck in Milledgeville, Georgia, injuring Mama June, Sugar Bear and Pumpkin. HBB was banged up but not seriously hurt.

TMZ broke the story ... Mama June, Alana, Sugar Bear and Pumpkin were all injured Monday night when their Chevy Suburban was making a left turn and smashed into a truck that was allegedly driving with its headlights off.

After the crash, June told us the family has only had the car for 6 months and hadn't paid it off yet.

The girl who sued Kris Humphries for giving her herpes has slept with so many men ... she can't say for sure whether the NBA star is actually the one responsible for transmitting it -- this according to new court docs.

TMZ broke the story, Kayla Goldberg sued Humphries in 2012, claiming she had unprotected sex with Humphries two years prior in 2010 and he gave her genital herpes.

But apparently suing Kris isn't the only thing that's easy -- according to new docs filed by Humphries' attorneys, Kayla admitted during a recent deposition that she has no hard evidence he's the one who gave her the herp ... because she's had multiple sexual partners.

Humphries wants her lawsuit dismissed.

Honey Boo Boo and her family were in a bad car crash Monday night ... TMZ has learned.

Honey Boo Boo, Mama June, Pumpkin and Jessica were all passengers in the car driven by Sugar Bear.

Sources tell us ... they were at a red light, waiting to turn left. The light turned green and Sugar Bear began the turn. A guy driving a truck was allegedly speeding in the opposite direction with his lights off and crashed into them.

Members of the family were taken to a local hospital ... an ambulance showed up, but Mama June tells TMZ ... the EMTs were going to take them to a hospital they didn't like so they went to another hospital in an SUV driven by their bodyguard.

Pumpkin says Sugar Bear has a contusion on his back, Mama June has a back and neck sprain, Honey Boo Boo hit her head against a passenger window and has a headache, and Pumpkin herself has severe chest bruises and is having panic attacks.

And June tells TMZ ... "We've only had that Suburban for 6 months and it's not even paid off yet."

Rapper Young Jeezy was arrested in Atlanta for battery, false imprisonment, and making terroristic threats against his own son ... TMZ has learned.

According to the charging documents -- obtained by TMZ -- Jeezy got into a fight with his son in a bathroom and he threw his son into the glass shower door, causing the son to hit his head. Then Jeezy allegedly punched him in the face, dragged him into the bedroom, threw him on the bed and pummeled him.

According to the incident report, Jeezy's son attempted to escape but the rapper's bodyguard blocked the stairwell.

The docs go on -- claiming Jeezy subsequently choked his son and said, "I will kill you." According to police docs, Jeezy's son claims his dad said, "I will put a bullet in your head right now," adding, "If I could get away with it, I would kill you." The incident allegedly occurred in Sept., 2012.

We've learned Jeezy turned himself in to authorities Friday after a grand jury warrant for each charge was issued. He bonded out soon after for $45,000.

Camille Grammer is safe from her ex-boyfriend for the next 3 years -- after he allegedly bruised and battered her during a blowout fight last year -- because TMZ has learned, she just won a permanent restraining order against him.

TMZ broke the story ... Camille won a temporary restraining order against Dimitri Charalambopoulos after the October 16th incident inside a Houston hotel room, when Dimitri allegedly attacked Camille, grabbing her by the hair, pulling her head into the bed and its headboard, and yanking out clumps of her hair in the process.

Now, the temporary restraining order has been extended for three years ... barring Dimitri from contacting Camille and her two kids until January 2017.

What's more ... the judge ordered Dimitri to give up any guns he may have ... the restraining order bars him from owning, having, possessing, buying or even trying to buy any firearms or ammunition while the order is in force.
