Michael K. Williams Official Cause of Death ... Fentanyl, Heroin Overdose

Michael K. Williams died of a fatal drug overdose ... according to the Chief Medical Examiner in NYC.

The Medical Examiner has determined ... "The Wire" actor's official cause of death is acute intoxication by the combined effects of fentanyl, p-fluorofentanyl, heroin and cocaine. His manner of death has been ruled accidental.

As we told you ... this was suspected by the authorities since Williams was found dead in his Brooklyn apartment on Labor Day, as drug paraphernalia and narcotics were discovered at the scene.

We broke the story ... the apparent drug overdose has triggered an NYPD criminal investigation to locate the source of the drugs that led to the actor's death.

Michael's death is the latest in a tragic string of accidental fentanyl OD cases. The same weekend he died, 4 comedians in L.A. overdosed on fentanyl-laced cocaine ... killing 3 of them, including Fuquan Johnson.

The powerful opioid has also claimed the lives of Shock G, 'Flash' actor Logan Williams, Lil Peep, Mac Miller and Prince ... to, sadly, name just a few.

Williams, a prolific actor known for playing Omar on "The Wire" along with starring roles on "Boardwalk Empire" and "Lovecraft Country," was just 54.

21 Savage Charged with Gun, Drug Possession Stemming from ICE Case

21 Savage's notorious ICE case from 2019 is circling back again ... the rapper's now facing gun and drugs charges in connection with the original incident with ICE agents.

According to the DeKalb County Sheriff's Office ... Savage is facing 2 new charges related to his arrest in Atlanta back in Feb. 2019 -- possession of a schedule II controlled substance, and possession of a gun.

The rapper turned himself in to authorities Thursday and bonded out right away. According to 21's lawyer, Charles Kuck, this is a state law charge related to his incident with ICE 2.5 years ago.

Kuck tells us, "Last night’s manufactured charges are yet another example of how our justice system, from ICE down to the local level, unjustly targets young Black men who seek to exercise their rights. There is no legitimate basis for these charges nor for ICE’s continued antics, and we will fight until Mr. Joseph is justly vindicated.”

As we reported back then ... Savage was arrested and placed in ICE custody because it was discovered that his visa expired and he's been here illegally for years. ICE is still trying to deport Savage to England ... something the rapper has been fighting.

What TMZ has now learned, is that during the Feb. 2019 arrest ... authorities allege 21 threw a bottle of liquid that had codeine in it out of his car, and a handgun was found inside the vehicle.

Hence, he's now been formally charged with drug and gun possession as his upcoming hearing in immigration court approaches. It's set for November.

As we first told you ... Savage's career was essentially derailed by the ICE case, and it seems like it's far from being wrapped up.

Richard Sherman Makes Pitch To NFL GMs ... Don't Judge Me Off My Arrest

Richard Sherman is still gunning to play football in 2021 ... and he's imploring any GM interested in signing him to judge him off his game tape -- not his wild July arrest.

"I've got a decade worth of resume that should stand more firmly than a momentarily lapse in judgment," Sherman said. "I've got a decade worth of character and tape to be judged off of."

Sherman, speaking in-depth about the crazy incident at his in-laws' home for the first time since the arrest, made the statements to NFL reporter Doug Farrar ... and explained he's ready to go if any NFL team wants to sign him.

Sherman also said he understands the July 14 tirade -- one in which he nearly beat down his wife's parents' door in a fit of rage -- wasn't exactly the best look for him.

Still, the 33-year-old corner said he thinks none of it should deter any NFL squads from putting him on their rosters.

"If you're judging me off a momentarily lapse then I think it's going to be -- then I'm probably not the player for you either way," said Sherman, who then vowed to burn any team who passes on him.

Sherman has only been linked to one squad since the incident ... with Tampa Bay Buccaneers coach Bruce Arians confirming they've checked in on the 5-time Pro Bowler recently.

As for any potential discipline from the NFL that might come down on Sherman due to the arrest -- the cornerback said he has not heard any news on that front.

Sherman's case, meanwhile, is still playing out in the court system ... with the football player saying he doesn't anticipate it wrapping up until after the season.

Mama June's Ex-BF Geno's Wanted in Alabama ... Doing Rehab in S. Carolina

Mama June's ex-boyfriend isn't getting any breaks from the judge -- even though he's in rehab, he's in rehab in the wrong state ... which is why he's now a wanted man.

According to new legal docs, obtained by TMZ ... a bench warrant's been issued for Geno Doak in Macon County, Alabama for failing to comply with the terms of his sentencing in his crack cocaine case.

The warrant says Geno's to be taken into custody by any Alabama law enforcement officer and held without bail until he can have a hearing.

The thing is -- as we first told you -- Geno's in South Carolina after checking into a recovery center there earlier this week, and the facility said he made a 120-day commitment to inpatient treatment.

Doak filed docs to loop in the judge from his crack cocaine case about his whereabouts, but unfortunately for him ... he was supposed to be in Alabama as part of his sentence, and failed to relocate there as required.

So, rules are rules, and rehab or not ... the judge has made Geno a wanted man in 'Bama.

PSU Coach Bruce Barnum I Spent $14K On Beer ... For Fans At Game

If you thought your bar tab was bad this weekend ... there's no way it topped Bruce Barnum's -- 'cause the Portland St. head coach revealed this week he dropped $14,000 to pay for fans' beers on Saturday!!

The coach -- who promised supporters he'd pick up ALL of their suds if they showed up to PSU's game against Western Oregon -- showed off the receipt Tuesday ... and it's pretty staggering.

Barnum ended up paying $14,448 for 786 Deschutes Fresh Squeezed IPAs and 1,260 Coors Lights -- a fairly shocking number, considering only about 3,000 fans showed up!

Of course, we're pretty sure Barnum was happy to foot the bill ... 'cause, after all, his team did beat the Wolves 21-7 in a laugher.

Unclear if Barnum is planning similar things for PSU's upcoming games -- but, hey, can't mess with something that's working, right?

Mama June Ex-BF Geno Doak Enters Rehab in S.C.

Mama June's estranged BF appears to finally be getting the help he needs ... 'cause we've learned he's checked into rehab.

According to new legal docs, obtained by TMZ, Geno Doak was hospitalized in Fort Pierce, Florida earlier this month. It's unclear why he landed in the hospital, but we do know doctors there referred him to a recovery center in South Carolina, and he checked into that facility on Monday.

In the docs, the director of the center says Geno will be a resident in the long-term intensive program and be required to make a minimum of a 120-day commitment. But, he also has the option to stay for up to a year if necessary.

The program is designed to help individuals develop their capacity for a healthy transition to sober living ... and all residents are required to eventually seek employment and are encouraged to assume responsibility of themselves and their recovery.

The documents were filed to loop in the judge from Geno's crack cocaine case. As we reported ... Mama June and Geno were both busted outside an Alabama gas station back in March 2019 and charged with felony drug possession.

While their relationship status had remained unclear for months, it was back in August when June announced Geno was no longer a part of her life, so many fans assumed they split.

Geno was sentenced to 16 months in Macon County Community Corrections. He wasn't going to physically be behind bars, but had to follow a set of rules.

For instance, he was supposed to relocate from Florida to Alabama within 30 to 45 days of his July sentencing date ... but he failed to do so.

Now the court knows he'll be in treatment in South Carolina for at least the next 4 months. At least that's the plan.

Ex-NFL QB Colt Brennan Death Caused By Accidental OD Methamphetamine, Fentanyl Found

Colt Brennan died of an accidental drug overdose, TMZ Sports has learned ... after a toxicology report found the former NFL QB had ingested fentanyl and other dangerous substances prior to his death.

Brennan tragically died at the age of 37 in May 2021. The former University of Hawaii quarterback had been receiving treatment at a drug rehab facility in Southern California when he experienced a medical emergency. He was rushed to the hospital ... where he passed away.

Now, after completing an autopsy and tox screen, health officials tell us the death was accidental, caused by a lethal concoction of drugs (polydrug), including the dangerous and illicit substance, fentanyl.

In addition to the incredibly powerful opioid, officials also found methamphetamine, amphetamine and ethanol in his system.

It's unclear if any of the drugs were prescribed by doctors ... or if Colt took them on his own.

After starring at Hawaii, Colt was a 6th-round draft pick in 2008. He bounced around the league ... playing with Washington and Oakland, before playing ball with the CFL in Canada.

Brennan was from Newport Beach, California.

Jason London Dazed, Confused and Arrested ... For Public Intoxication

Jason London is harkening back to his most popular movie role, but in the worst way possible -- cops say he was so drunk he swerved off a road and crashed his car ... multiple times.

According to a police report, obtained by TMZ, the "Dazed and Confused" star was arrested late Tuesday around 11:30 PM in Ocean Springs, Mississippi when cops found him behind the wheel of a 2010 Nissan Maxima that was on fire and stuck in a ditch.

Cops say London's passenger was standing near the car -- which had sustained heavy damage on the passenger side -- and London claimed someone had sideswiped them and run them off the road.

According to cops, London -- who played football star Randall "Pink" Floyd in the 1993 Matthew McConaughey film -- reeked of booze, could barely keep his balance and had slurred speech.

Cops investigated the scene and believe he swerved off the road, mowed down a sign, got back on the road, and then hit a concrete object before driving into a ditch.

London was booked for public intoxication and leaving the scene of an accident. When they got him to jail in Jackson County, cops say he was so drunk and out of it they couldn't fingerprint him.

As for why they didn't hit him with a DUI -- an Ocean Springs PD spokesperson tells us the officer used his discretion, and instead decided to arrest him on the other charges. We're guessing the thought process was he hadn't observed London driving.

Lil Mo Fat Joe’s ‘Dusty Bitches’ Jab Triggering … I've Struggled with Addiction!!!

Lil Mo is calling out Fat Joe for referring to her as a dusty bitch crackhead during "Verzuz" ... saying he's way outta line, and totally insensitive to all people battling addiction.

Here's the deal ... Fat Joe came under fire after ripping Lil Mo and another female rap legend, Vita, during his Tuesday night musical showdown with Ja Rule. He said Ja must have found them in a "crack house."

Lil Mo tells TMZ ... Fat Joe's comments are "vile" and "disrespectful" and he should know better than to use those degrading terms, especially in light of recent celeb deaths, like Michael K. Williams, from drug problems.

In fact, Lil Mo says Joe's disses were actually triggering to her, because -- even though she's clean now -- she's dealt with her own opioid addiction issues in the past.

Fat Joe ended up apologizing publicly -- he also says he told Lil Mo and Vita sorry to their faces -- and Remy Ma rushed to his defense on social media, but Lil Mo says he shouldn't get off the hook so easy.

However, she was quick to say she doesn't want Fat Joe canceled. Watch ... she told us what she does need from Joe after his painful words.

Kate Quigley Seen Out in Public for First Time Since Nearly Fatal OD

Kate Quigley is back on the outside after her scary overdose incident ... which left 3 of her friends dead, and her hospitalized in critical condition.

The comic was spotted out Tuesday for the first time since being released from the hospital. She was cruising through Venice in her convertible -- stopping at a barbershop for a quick chat, and then got back on the road.

She looked pretty casual -- rocking a black T, cut-off jean shorts, flip-flops and a cap -- as she seems to be getting back to her normal life after the ordeal.

As we reported ... Kate was rushed to the hospital a couple weeks ago after cops got a call about a possible overdose situation. Three comedians -- Kate's friends Fuquan Johnson, Rico Angeli and Natalie Williamson -- were found dead on the scene. Kate was still alive.

We're told cocaine laced with fentanyl is what they're suspected of using that night. Autopsies are underway for an official cause of death.

She's since spoken out about the ordeal, saying she's thankful to be breathing ... while still mourning the deaths of her comedy pals. She's also announced that she's going to get a move on with her life, and make the most of it.

Vili Fualaau Mary Kay Letourneau's Estranged Husband ... Busted for DUI

Vili Fualaau -- estranged husband of the late Mary Kay Letourneau -- was arrested on suspicion of driving under the influence after nearly hitting a police car ... law enforcement tells TMZ.

A Washington State Patrol rep tells TMZ ... Fualaau was driving erratically on the highway around 4 AM Saturday when he made an abrupt lane change and nearly struck a Seattle PD vehicle.

We're told Vili was pulled over and a state trooper was also called to the scene. Cops say Fualaau had bloodshot, watery eyes and performed poorly in field sobriety tests and after he submitted to a voluntary breath test ... he was arrested.

The result of his test has not been disclosed, but it clearly didn't help get him off the hook -- Vili was taken to the Snohomish County Jail and booked on a DUI charge.

As you'll recall ... Fualaau is best known for his notorious relationship with his teacher, Letourneau, which made headlines in the late '90s. She was 34, married and had 4 children when she began having sex with him -- her 6th-grade student. He was either 12 or 13 at the time.

Mary pled guilty in 1997 to 2 counts of felony second-degree rape of a child and ended up serving 7 years in prison, but also gave birth to 2 of Vili's kids ... and after she was released, they got married in 2005.

The couple split in 2017 and Mary died last year of cancer. We were told Vili was by her side while she was in hospice until the end, helping take care of her.

Comedian Kate Quigley Breaks Silence ... After OD Deaths of 3 Friends

Kate Quigley has come out the other side of a scary overdose incident that claimed 3 of her friends' lives -- and the first thing she's doing as she continues to recover ... honoring them.

The comedian -- who was one of 4 people who apparently ingested a bad batch of coke last weekend -- broke her silence Saturday on the passing of Fuquan Johnson, Rico Angeli and Natalie Williamson ... all of whom suffered suspected ODs while partying in the Venice neighborhood of L.A.

Quigley was with them, of course, but miraculously survived the ordeal -- and now, she's remembering her late pals after a whirlwind week of overdoses affecting Hollywood.

She starts out by thanking everyone who reached out to her with words of encouragement as she healed in the hospital, while also giving a s/o to the docs, nurses and first-responders who treated her and, as she puts it, saved her life. Quigley has since been released.

KQ also writes, "I am still shocked & devastated by the loss of my friends Fu, Rico and Natalie. Words cannot describe the pain I am feeling & I will be changed forever by their passings. Fu was a generous, loving soul ... Rico was always filled with enthusiasm & Natalie was so kind and endearing." She says they all left a mark on her life, and she'll miss them.

Quigley finishes by saying  ... "There are so many more things i want to say, but I'm not ready. When I am, I will. Until then, all I can say is: Tell your people that you love them everyday. Stay safe. And please don't take this life for granted. I did, and I never will again."

2 Chainz Throws 'No Fent' Birthday Party

2 Chainz has made a statement about the fentanyl epidemic that has ripped through Hollywood and the nation, by throwing a "no fent" birthday party for himself.

Chainz is turning 44 Sunday, and he marked the occasion a few days early with his party Friday night.

The party, which went down in Atlanta at the Members Only Lounge, was not exactly drug-free. He posted a vid of bags that appear to be psilocybin mushrooms.

The bash had a theme -- Hippie's in the '70s -- which sounds kinda awesome.

Chainz and his wife, Kesha Ward, posted pics of them in their party getups.

As for the "no fent" theme ... there have been a rash of fentanyl-related deaths recently. TMZ broke the story, comedians Fuquan Johnson, Rico Angeli and Natalie Williamson all died of a suspected accidental fentanyl OD last week in Venice, California.

And, 16-year-old Logan Williams, who starred in "The Flash," died last year from an accidental fentanyl OD.

Michael K. Williams was found dead last Monday in his Brooklyn apartment, and authorities found drug paraphernalia at the scene. Authorities believe the death was heroin-related and toxicology tests are being run to determine, among other things, if fentanyl was involved.

Tiffany Haddish got emotional with us talking about her friends she lost to fentanyl-related OD's.

"Intervention" host Ken Seeley told us on "TMZ Live" this week how fentanyl hooks people who use drugs and how it takes many of their lives because of its extreme toxicity.

Andre Royo -- one of Michael K. Williams' co-stars on "The Wire" -- says the entire 'Wire' family is feeling the loss ... but he's encouraging people to keep spreading the late actor's positivity.

Andre -- who played the drug addict and informant "Bubbles" on the popular HBO series -- was out in NYC Friday when he reflected on Michael's life ... saying he was someone who cared about everybody.

Royo tells us Williams' death was a stunner, but he hopes people will rally together to keep his name alive, remember his work, and shine his light ... because his former co-star was a great person, and clearly left a major impact.

As we reported ... Williams -- who played the beloved character Omar on "The Wire" before going on to star on several other hit shows -- was found dead Monday of an apparent overdose in his Brooklyn apartment.

There's been an outpouring of love and respect for Michael this week, and like Andre points out ... the tributes are warranted.

'The Flash' Actor Logan Williams Died from Fentanyl ... Coroner's Report Reveals

Logan Williams -- best known for his role on the CW series "The Flash" -- died from a fentanyl OD.

Canada's BC Coroners Service says the 16-year-old Logan died from an "unintentional illicit drug toxicity (fentanyl)." The manner of his death has been listed as "accidental."

According to the coroner's report, obtained by the NY Post, Logan "struggled with mental health concerns and had a history of consuming illicit substances." And three months before his death, Logan was reportedly unresponsive after consuming substances, though the report wasn't specific.

Logan was later resuscitated and diagnosed with a significant brain injury that messed with his ability to function.

Logan, who played a teenage Barry Allen on the CW series, died back on April 1, 2020 in Burnaby, British Columbia. The day he died, the coroner says Logan left a residential care home and was reported missing. Once Logan returned he appeared agitated and under the influence. He went to bed to sleep it off ... and staff checked on him hourly. But, when staff went to wake him up he was unresponsive.

As we've reported ... there's a fentanyl epidemic that's hitting Hollywood hard right now. Logan's news comes on the heels of comedian Fuquan Johnson and 3 others dying after an apparent overdose. Our sources say cocaine laced with fentanyl appears to be what they ingested.

Kate Quigley On the Mend After OD ... Thanks Friends for Support

Kate Quigley's gonna be okay after overdosing in L.A. with friends ... an incident that left 2 of her fellow comedians and another woman dead.

The model/comedian revealed she's on the mend, saying ... "I'm still healing physically & mentally. But reading all the positive messages from u guys is makin me cry tnight."

She added she's grateful for all the support and love, and it's helping her recover in the hospital ... slowly but surely.

We broke the story ... Quigley was at a get-together last weekend in the Venice neighborhood where 3 people -- comedian Fuquan Johnson, comedian Rico Angeli and Natalie Williamson -- all died of suspected accidental fentanyl ODs.

Kate was also hospitalized in critical condition, but she's stable now and says she's getting a little stronger every day.

As we told you ... it's suspected that the victims at the party died from ingesting cocaine laced with fentanyl. The LAPD is investigating.
