Houston Texans' head coach Gary Kubiak remains in a Houston hospital after collapsing on the field at halftime yesterday ... team officials say he's being monitored by doctors .... but he's "alert, coherent and in good spirits."

Kubiak had to be carried off the field on a stretcher ... after he fell to the ground complaining of dizziness during the Texans-Colts game. Team officials say he didn't have a heart attack and never lost consciousness.

Team doctors treated Kubiak on the field before he was taken by ambulance to the hospital. A statement from the Texans' front office says he was admitted overnight and will be watched by doctors for the next 24 hours to try to figure out why he collapsed.

Colorado Avalanche goalie Semyon Varlamov showed no mercy during his alleged attack on his girlfriend this week -- his GF not only told police he kicked her ... she says he stomped her, dragged her around by the hair ... then said he'd beat her more if he could.

The alleged domestic violence incident occurred on Monday -- according to the Denver police report, Varlamov came to his girlfriend's house and assaulted her, kicking her in the chest and knocking her down.

Once on the floor, his girlfriend claims he stomped her chest, then grabbed her hair and dragged her out of her bedroom.

Still holding her hair, the GF says Varlamov pushed her face to the floor and told her (in Russian) that if this were Russia, he would have beat her even more. It's unclear what, if anything, provoked the argument.

The GF was interviewed by police yesterday, and the detective observed bruising on the girlfriend's arm and hip.

An arrest warrant was subsequently issued for Varlamov, but he has already turned himself in to authorities. He has since been arrested.

Camille Grammer just got a restraining order against her boyfriend, claiming he brutalized and terrorized her in a hotel room, leaving her bruised and battered.

In her declaration, filed by attorney Larry Stein and obtained by TMZ, Camille claims on October 16, she was in a Houston hotel room with boyfriend Dimitri Charalambopoulos ... recovering from cancer surgery just days before.

At 1:30 AM Camille says a woman started texting Dimitri and ended up calling him and screaming.

The phone call triggered an argument between Camille and Dimitri. She claims at that point he grabbed her by the hair, twisted it around his hand and repeatedly pulled her head into the bed and its headboard. She says he pulled out clumps of her hair in the process.

Camille says she tried to call for help, but Dimitri smashed her iPhone.

But it didn't end there. Camille claims Dimitri "squeezed my nose between his finger and pushed upward forcefully, covering my mouth in an attempt to silence me to prevent me from breathing."

Camille also claims Dimitri wrestled her on the hotel bed, "repeatedly slamming my head and face into the furniture and completely immobilizing me." She claims he said, "I wanna smash your head in. I'll give you a reason to call the cops."

Camille says the abuse went on for 40 minutes to an hour as she feared for her life and begged him to stop.

She says as Dimitri was about to leave he said he would knock her out if she moved from the bed.

Camille, who claims she felt Dimitri would kill her, ended up bruised and battered.

A judge just granted a temporary restraining order ... ordering Dimitri to stay 100 yards from Camille and her 2 kids.

TMZ has obtained the 911 call made by a bystander moments after Dolly Parton was involved in a car accident in Nashville yesterday.

During the call, the male witness says he saw the car Dolly was riding in (a Nissan) get t-boned by a Mitsubishi.

The witness says the driver of Dolly's car, the singer's longtime friend Judy Ogle, was trapped in the driver's seat and needed help to get her out of the car.

The witness never identifies Dolly by name -- and says that no one appears to be in grave danger.

Green Bay Packers tight end Jermichael Finley says he's finally been transferred out of the ICU -- and was able to walk today -- after badly injuring his spinal cord during Sunday's game against Cleveland.

Finley -- who was removed from the field on a stretcher yesterday -- tweeted moments ago ... "I'm happy to report that I have been transferred out of the intensive care unit and that I have full feeling in my arms and legs."

He added, "As importantly, I was able 2 walk to & from the shower today, which was badly needed after yesterday's victory!"

Dolly Parton was rushed to a hospital in Nashville this morning -- and luckily her boobs are O.K. ... TMZ has learned.

A rep for the Nashville PD tells us Dolly was a passenger in a car being driven by her longtime friend Judy Ogle this morning ... when it was struck by another vehicle.

So far, the details surrounding the crash are unclear ... but cops say Ogle was not to blame for the collision.

Dolly was taken to a nearby hospital ... and was later released with minor injuries.

The 67-year-old singer later tweeted about the crash saying she's "tired and sore" but resting at home.

UFC heavyweight champion Cain Velasquez didn't let a busted face stop him from celebrating his victory last night -- the guy partied hard, bandages and all.

Cain and his crew hit up a club called The Mark in Houston, TX following Cain's epic win over top contender Junior Dos Santos at UFC 166 -- and despite the injuries, Cain kept a smile on his face.

In case you missed it, it was an awesome match. Velasquez eventually won out with a brutal TKO in the 5th round, but not without taking some damage himself.

Casey Kasem will die in the next few months ... this according to a rep for Casey's wife Jean.

As we reported, Casey and Jean were sued by a former caregiver who claimed she was mistreated and cheated by Jean during her employment with the couple. The housekeeper, Hilda Loza, sued in small claims court and the case was heard Friday.

Jean did not appear at the hearing. The wife of her lawyer appeared on Jean's behalf. The woman told the judge ... Jean wanted a continuance for a few months because that's how long it will take for "the situation to resolve or end, unfortunately."

If that seems a little cryptic, Jean submitted legal docs in which she said, "My husband is terminally ill and unable to leave home."

David Cassidy maimed a woman because of his reckless cell phone antics ... so claims the lady in a new lawsuit.

Novlet Lawrence claims in her lawsuit ... the "Partridge Family" star was negligently driving his car when he slammed into her, causing her to suffer bodily injuries, disability, disfigurement, mental anguish, and loss of enjoyment of life.

As for the cell phone thing ... Lawrence wants Cassidy to fork over his phone records -- presumably because she believes he was illegally yakking away just before the crash

Bad news for singer Austin Mahone -- his rep tells TMZ, Mahone was hospitalized yesterday morning with a 104-degree fever, and he's currently being treated for a blood clot and "extreme inflammation" in his throat.

The rep says 17-year-old Mahone is also being treated for severe dehydration. The rep didn't explain what might have caused the medical emergency.

Mahone posted the above photo to his Instagram yesterday with the cryptic caption, "I've never felt so bad."

Casey Kasem is in terrible shape ... his mind is almost gone and he's unable to function -- this according to new legal docs obtained by TMZ.

The document outlines the mental state of the legendary founder of "America's Top 40" radio program. According to the examining physician, Casey has

-- major impairment to short term, long term and immediate recall memory

-- major impairment to verbally communicate

-- major impairment to perform simple calculations, plan simple tasks and reason logically.

-- severe disorganized thinking

-- moderate hallucinations

The document was filed by Julie, one of Casey's daughters who is trying to become conservator. Casey's wife of 33 years, Jean, is fighting the move to take control away from her.

The doctor says Casey has been sliding downhill for the last 3 to 4 years. He's suffering from advanced Parkinson's and dementia.

Three-time Indy 500 winner Dario Franchitti is rolling again -- just in a wheelchair for now -- but it's a start after his horrific crash earlier this month ... and TMZ has photos of his recovery.

Dario was wheeled out of an Indianapolis hospital today after undergoing surgery Monday on his fractured right ankle.

Dario got seriously messed up -- multiple fractures and a concussion -- during an IndyCar race in Houston earlier this month. His car bumped another one, sailed into the air, and smashed into a fence.

Sources close to Dario -- a 4-time Indy series champ -- tell us he's doing well after surgery and focusing on "getting back on the racetrack" next season.

Ricky Schroder has such fears his daughter's Malibu school causes cancer ... he's yanked her out, and we're told other high-profile parents have similar concerns.

Ricky tells TMZ ... the facts are scary -- 3 teachers at Malibu Middle School have been diagnosed with thyroid cancer in the last 6 months. Seven other teachers have experienced severe migraines and others say they have skin rashes, hair loss and other ailments.

Ricky says the buzz around the school is that soil around the school is contaminated with PCBs, lead and pesticides. Teachers also think the classrooms might be contaminated with mold.

Parents are now demanding tests for various contaminants, including radioactivity, asbestos and Ricky tells us ... he's physically removed his daughter from the school "as an abundance of caution" ... until it's determined it's safe for her to return. Until then, she's taking classes at home via independent study ... and about 25% of the students in her class are doing the same thing.

As for Ricky ... he says until he knows it's safe for his daughter to return to classes, she'll be homeschooled.

School officials are conducting more testing this week. According to a statement, preliminary tests showed "no unusual findings" for mold contamination ... and any mold found so far should not pose a health problem.

Famed director Michael Bay almost got clobbered by what appears to be a drugged out crazy who swung an air conditioner at his head on the set of "Transformers 4."

The incident occurred today in Hong Kong. The attacker somehow made it on the set, accosted several members and then set his sights on Bay. When Bay realized what was up, he saw the crazy man swinging an air conditioner right at his noggin.

Bay then wrestled the unit out of the attacker's hands and security quickly subdued him.

Cal Ripken Jr. tells TMZ ... his mother is "doing well" after she thwarted a gun-toting bad guy who allegedly tried to jack her car in a bank parking lot in Maryland.

Cops say Violet Ripken was held up at gunpoint on Tuesday afternoon -- but calmly pressed the panic button on her car key and scared the guy away. The suspect was later arrested after cops obtained a photo of the suspect from a nearby ATM.

Scary stuff ... but last night outside Madeo in L.A. -- where Cal was having dinner with fellow Orioles legend Eddie Murray -- Ripken told us his mom is on the mend after the frightening encounter ... her second incident with a gunman in 2 years.

Lamar Odom ate out last night -- Armenian, of course -- but emerged from the restaurant looking scarily thin ... especially for a guy trying to make an NBA comeback after a months-long drug binge.

Lamar and some friends hit up Gavar Cuisine in Glendale, CA -- and he walked out sporting his customary Nike warmup, white tee, and interestingly ... a rosary.

LO thinks he can kick his drug habit without rehab AND return to game-shape for the upcoming NBA season, so ... if anyone could use religion it's him.

But judging by his gaunt frame ... what he really needs is a couple of double-cheeseburgers.
