Jonathan Bennett's "Dancing With the Stars" appearance almost never happened ... because he suffered a humongous fall as he jokingly tried to prep for the show ... and it was all caught on tape.

So here's the deal ... the "Mean Girls" star shot this video a few months back, but kept it hidden until "DWTS" finished its run (congrats, Alfonso).

In the video, he and a few friends attempt an "interpretive dance" ... but Bennett tried a move on the railing of a staircase that ended ... well, you gotta see it. He ended up with 2 broken ribs and his arm in a sling for 2 months.

The best part -- they were dancing to "Wind Beneath My Wings."

Seeing as he was the one who allowed the video to be posted ... Bennett's ego escaped unscathed.

The man who claims he was attacked by Johnny Manziel and his "entourage" last week says he's still in pain from the alleged hotel beatdown ... and says he's got pics to prove it.

TMZ Sports spoke with Chris Gonos ... who claims his left elbow was jacked up by Johnny's crew during a fight at The 9 hotel in Cleveland late Friday night.

Gonos says the picture shows severe swelling and a nasty bruise -- but claims he also suffered a swollen lip and and injuries to his eye.

Gonos says he's not sure who hit him -- but he's confident the assailant was part of Johnny's crew.

Police are investigating the incident ... but so far, no arrests have been made.

As for Johnny, he tells a very different story ... saying he was "accosted by a very aggressive man and his associates" at the hotel that night.

Janel Parrish just had her chances of winning the Mirrorball Trophy derailed ... at least that's the way it looks after she suffered a gnarly injury during rehearsal.

TMZ just got this video of the "Pretty Little Liars" star and Val Chmerkovskiy practicing lifts. Something bad clearly happened because you see her dissolve into tears as she hobbles to a room where doctors examine her.

It looks like she suffered a broken or compressed rib.

She hasn't been counted out, but it doesn't look good. The finals are tonight.

Kansas City Chiefs star safety Eric Berry has been placed on the non-football injury list after doctors discovered a mass on the right side of his chest ... which they believe to be lymphoma.

Berry is expected to go to Emory University hospital for further evaluation.

Chiefs head coach Andy Reid said of the situation, "Right now, he needs to be a beast. He'll attack this."

Berry released a statement, saying, "I am truly thankful for all of the support from family, friends, coaches, teammates and the entire Chiefs kingdom. At first I was in shock with the diagnosis on Saturday and did not even want to miss a game, but I understand that right now I have to concentrate on a new opponent."

Armie Hammer thinks he's such a badass boxer ... he dared to step in the ring against a former NFL linebacker whose nickname was "Lights Out" -- which explains the shiner on Armie's eye.

TMZ Sports got this shot of Armie's bruised face after his sparring match with ex-Chargers star Shawne Merriman -- who's 6'4" and 280 lbs! Nonetheless, we're told Hammer's the one who sought out Merriman as a sparring partner.

The bout went down Wednesday at Unbreakable Performance -- a new gym on the Sunset Strip owned and run by ex-Bears star Brian Urlacher and Jay Glazer from FOX Sports. We're told Armie's been training with Glazer for several months ... and word is everyone in the gym was stoked Armie wanted to challenge Shawne.

We're guessing Armie's agent isn't so stoked about his swollen face -- which makes it even cooler he's doing it. Can't wait for the rematch!

The mangled finger that "fell off" Jose Canseco's hand during a poker game WILL NOT be popping up on eBay -- 'cause the whole thing was a giant prank ... TMZ Sports has learned.

As we previously reported ... the former baseball star blew off part of his finger last month while cleaning a gun -- but got it reconnected in surgery.

Last week, Jose claimed the reattached digit went rogue ... tweeting, "I was playing in a poker tournament last night and my finger fell off."

Disgusting. Funny. And totally untrue. Jose now admits it was a HOAX.

We contacted Canseco about video he says he shot of the finger fail -- and when push came to shove ... he had to cop to the prank. Jose says he cooked up the plan when he came across the fake finger in a Halloween store.

As for the eBay auction he was pimping on Twitter -- sorry, sickos ... the real deal's still attached.

"American Idol" singer Michael Johns died from an enlarged heart, but a "fatty liver" contributed to his death ... this according to the Coroner's report.

The 35-year-old singer died suddenly Aug. 1 after his leg became swollen and bruised ... reportedly from a sprained ankle. Family sources claimed the injury had triggered a blood clot, but his best friend claimed the singer drank himself to death.

The Orange County Coroner's report concludes Johns died of natural causes -- the result of a disease called dilated cardiomyopathy, a condition that inhibits the flow of blood to the body. The disease caused his heart to enlarge.

As for the "fatty liver" ... it can be caused by excessive alcohol intake, but there are other causes as well. Law enforcement told us ... Johns often drank to excess and did so hours before he died.

Monday night's alright for fighting too -- unless you're the dude whose foot was damn near torn from his body in a brutal West Hollywood street brawl last night.

The fight went down outside DBA Nightclub around 2 AM ... when no fewer than 3 guys surrounded another guy in the middle of Santa Monica Boulevard and unleashed holy hell -- repeatedly kicking and punching the victim.

When he ends up on all fours ... things really get gruesome, because he gets rolled ... and his ankle fully SNAPS IN TWO!!! It's unclear what started the fight --- but it's obvious from the video what ends it.

Richard Simmons' close friends are now mad, in addition to being extremely concerned for his welfare ... because they believe his spokesperson is shoveling crap with the story that Richard's OK.

The spokesman told the NY Daily News ... Richard is "sad" but not clinically depressed, a shut-in or a recluse.

Now extremely close friends of Richard are pissed ... saying Richard's people are not leveling. For a variety of reasons, they believe something dire has happened.

-- The last contact any of his closest friends had with him is March 10.
-- The mother of an extremely close, long-time friend died a few months back, and Richard never contacted her, which is completely out of character.
-- Richard was close with Joan Rivers and, as one friend put it, "There is no way he wouldn't have gone to her funeral if he was OK."
-- He has abandoned his exercise studio, where he was a fixture. He hasn't been there in months, and everyone there says they have no idea why.

As we reported ... one of Richard's key people told us a looming knee replacement sent him into a massive depression ... exacerbated by the death of his beloved dog. But the close friends say they don't believe knee surgery or the dog would cause Richard to cut himself off from the outside world.

As one friend said ... "If Richard is really OK and is aware people are extremely concerned about him, he'd pick up the phone and tell them to relax. But he hasn't, and we're worried sick."

Glen A. Larson -- who created the hit shows "Magnum P.I.", "Knight Rider" and "Quincy M.E." -- has died from esophageal cancer at the age of 77.

The incredibly successful writer died Friday night at UCLA Medical Center ... according to his son James.

Larson was one of the most prolific producers of the '70s and '80s ... creating a string of iconic hit shows -- including "Battlestar Galactica," "The Fall Guy," "B.J. and the Bear," and "Buck Rogers in the 25th Century."

He also wrote the original "Six Million Dollar Man" pilot and TV movies.

In case you're wondering ... Quincy, Magnum, Rider and Fall Guy alone account for 513 hours of television -- as well as 21 combined seasons from 1976-88.

Safe to say this guy helped define an entire generation of TV watchers.

Iggy Azalea has no tolerance for pain ... 'cause she claims she "smashed" herself in the face while dunking a basketball ... despite strong video evidence to the contrary.

Iggy was hoopin' around with her BF Nick Young on Friday when she decided to test out her dunk skills.

Dierks Bentley sings about being drunk on a plane, but now the country superstar is also an expert on vomiting on a stage ... thanks to a drunk fan -- and the footage is disgustingly awesome!!

Dierks was performing at the Hershey Giant Center in Pennsylvania last night and brought two fans on stage to sing along to "Drunk On a Plane."

Bret Michaels is back on stage, and somehow lovin' it ... just 2 days after going under the knife to have 2 stents ripped out of his body.

If Bret was in pain, he masked it well while he performed in Jacksonville Thursday night. Pretty amazing ... the guy's been in and out of 6 different hospitals over the last couple weeks to deal with complications from his diabetes.

Normally, stent removal is incredibly debilitating ... it can take several weeks to recover.

But Brett's pure bad ass. He's got 6 more shows over the next 2 weeks.

Alfonso Ribeiro -- the front-runner on "Dancing with the Stars" -- may have to pull out of the competition days before the semifinals ... and it's all because of The Carlton Dance.

Our DWTS sources say Ribeiro injured his back during rehearsal Wednesday night as he tried to overcompensate for a groin injury which he suffered on the show, performing his signature move.

The back injury is bad -- he's having trouble even walking. Alfonso skipped rehearsals Thursday so he could get an MRI.

Doctors are telling him the next 48 hours are critical, but they say there's a real chance he can't dance.
