2:30 PM PT -- Warner Bros. tells us, "We take all allegations of workplace harassment very seriously. These allegations were brought to our attention and were thoroughly investigated earlier this year. Our findings did not support the plaintiff’s characterization of the events claimed to have taken place, which is why we are disappointed by the filing of this lawsuit."

The company that produces the 'Bachelor' franchise, along with several of its producers, has a sexual harassment case on its hands -- a former employee now claims producers repeatedly asked her sexually invasive questions that she says had nothing to do with the show.

Becky Steenhoek is suing Warner Bros. Entertainment and several producers from the show claiming they asked her gratuitous questions, including:

-- How often do you masturbate?

-- Do you own sex toys/dildos?

-- Have you ever had anal sex?

-- Do you enjoy giving hand jobs?

-- Is your vagina shaved? Do you have a landing strip?

-- Has a man ever got your "taco" talking?

Steenhoek says these questions were designed to embarrass her because of her "sexual inexperience and conservative views about sex."

She claims a week after complaining about the comments she was fired.

Katherine Jackson is bowing out of the guardian biz when it comes to Michael Jackson's youngest child, Blanket.

According to docs filed by Katherine ... there's no longer any need for her to be the 15-year-old's guardian. Katherine had been appointed joint guardian -- along with TJ Jackson -- of all 3 of Michael's kids in 2012.

Both Paris and Prince are adults now, but Katherine says because Blanket has reached his adolescence ... TJ can handle guardianship on his own. She also points to her own age, 87, as a reason she's stepping down.

Worth noting ... Katherine had not seen her grandchildren for several months earlier this year. It's still unclear how much contact she's had with MJ's children.

A judge still needs to approve Grandma Jackson's request.

Jeremy Meeks is an absentee father whose success as the so-called "Hot Felon" went straight to his head ... so claims his estranged wife.

Melissa Meeks filed docs demanding child and spousal support, as well as sole legal and physical custody ... claiming Jeremy sees their 7-year-old son "maybe 2 days a month."

In docs obtained by TMZ, Melissa says Jeremy "basically lives as a playboy with a rich girlfriend" ... TopShop heir Chloe Green. Melissa claims he drives a $150k car and has lots of jewelry now.

She doesn't list how much she wants in support because "I am not yet sure of his income, but I am sure it is more than mine."

She says Jeremy's "success went to his head and he left us." As we reported ... he filed docs back in July to separate from Melissa after 8 years. He filed for divorce in October.

A Sephora cosmetics store in Hollywood cared more about peddling lipstick than the oral health of customers, and now a woman is stuck with a lifelong disease ... according to a new suit.

A California woman claims she visited the store in October 2015 and sampled a lipstick from one of the "common use" tubes on display ... and ended up with herpes on her lip. She says the infection was diagnosed by medical pros and she claims she never had herpes or cold sores before this Sephora visit.

According to the docs ... Sephora failed to clearly warn the woman and other customers of the risk of getting herpes or other diseases from trying on the lipstick samples. She says if she would have known ... she would've avoided them like the plague.

The woman claims other lipstick companies have proven methods of avoiding herpes exposure -- like individual samples or assistance from trained professionals -- but Sephora cared too much about making a quick buck to bother.

She's suing the cosmetics chain for the emotional distress over an "incurable lifelong affliction" she says she now has to deal with as a result of using its lipstick samples.

10/30/17, 4:40 PM -- We spoke with a rep for Dr. Phil who tells us he never hit the skateboarder, and adds the only contact happened when the skateboarder put his hand on the roof of Phil's car to keep his balance.

Dr. Phil's not going to skate after hitting a dude with his car, because the skateboarder he struck is gearing up to go after him in court ... TMZ has learned.

Terrence Bembury's lawyer John Mardoyan tells us ... he's already filed an insurance claim over the accident, and it's very likely a lawsuit's coming next. The lawyer says Terrence sustained a serious shoulder injury and possible head trauma.

Bembury's also got a major problem with how he says Dr. Phil handled the situation. He claims he was in a state of shock after being hit by a celeb and was definitely not OK before Phil took off ... even though cops say he told Phil he was fine -- which gave the Doc the right to leave.

Terrence thinks whatever he said, Dr. Phil had a civic duty to stay until help arrived.

As we reported ... the skateboarder posted a pic from an exam room after the accident and lashed out at Dr. Phil online, but cops dispute some of his allegations toward the TV host.

Steve Aoki settled his lawsuit with the dude who sued him after biting it hard on the DJ's signature flying cake.

Raymond Collins claimed he suffered a serious head injury in March after slipping on cake Aoki threw from the stage during a set at Hakkasan.

Aoki's lawyer, Barry Thompson, told TMZ, "Steve loves and is extremely dedicated to his fans. He was concerned to hear that Raymond Collins had fallen at his show after a long day and night celebrating with friends at a bachelor party."

No word on a monetary amount.

Thompson added, "Steve wishes Mr. Collins all the best."

Justin Bieber's obsessed fan has nothing but time on her hands while she sits behind bars ... the price to pay for showing up uninvited to his crib.

Mirabelli Stefania pled no contest to one count of misdemeanor aggravated trespass and the judge immediately sentenced her to jail, but not a lot. She got 8 days, but only has to serve 4. She's been in jail since her arrest earlier this week so she'll be back on the streets within a day.

Stefania will be on probation for 12 months, and she's been ordered to stay 100 yards clear of J.B.

TMZ broke the story ... Stefania was busted at Justin's Bev Hills pad after cops say she wandered onto the property for the third time in a week. Bieber was home at the time but she never made it inside the residence and he never made contact with her.

Here's the problem ... obsessed fans are the least likely to obey stay away orders because they're often not operating rationally.

Steve Harvey's right-hand woman on his morning radio show was a months-long freeloader when it came to rent and then bounced when the landlord put his foot down ... this according to a new suit.

Shirley Strawberry -- who appears with Steve on "The Steve Harvey Morning Show" -- is being sued by the homeowner's leasing agent, Pacific Shore Property Management, claiming Shirley and her hubby were deadbeat tenants.

In the docs, obtained by TMZ, PSP says Shirley had a 1-year lease for $7,500 a month. Shirley paid the first month's but May's payment was 25 days late. After that Shirley paid zilch.

The homeowner tells TMZ ... Shirley made nothing but excuses about why rent was late. The owner adds sheriff's finally came to evict her in September but Shirley bounced minutes before they arrived.

Pacific Shore is suing for rent owed and late fees. We've reached out to Shirley, so far no word back.

Here's something a little strange -- Charlie Heaton got kicked out of the U.S. because he was allegedly caught with cocaine at the airport ... TMZ has learned.

Law enforcement sources tell us ... the "Stranger Things" star flew into LAX last Saturday from London, but was denied entry because a random baggage check uncovered trace amounts of cocaine in his bag.

We're told since Heaton didn't have a previous record, he was allowed to withdraw his application to enter the country and get on a plane back to London instead of being arrested.

Our sources say he will be allowed to enter the U.S. again ... but not until he reapplies for admission.

This airport incident is why Heaton was noticeably missing from the Netflix Season 2 premiere party for "Stranger Things" Thursday night in L.A.

Jay-Z's sampling a Shaggy hit to respond to a lawsuit over the Roc Nation logo, basically saying ... it wasn't me!

You'll recall, Jay got sued by Iconix Brand Group, which had purchased the Roc Nation trademark for $204 million, and therefore controls rights to all merchandise with the logo.

In new docs, obtained by TMZ, Jay fires back at Iconix, saying its beef is with the Roc Nation corporation, and not Jay-Z, personally. In essence, he's saying ... don't confuse me with my company, we're separate entities.

The Roc Nation owner says because Iconix hasn't proven he was personally involved in the deals to slap the logo on a bunch of MLB merch ... he should be removed from the lawsuit.

Ex-boxing champ -- turned "Dancing with the Stars" contestant -- Victor Ortiz struck a plea deal in his DUI case ... and he's on thin ice for the next 3 years.

TMZ Sports broke the story ... Ortiz was arrested on Sept. 24 of last year in Ventura County, CA around 7 PM when cops pulled over his BMW and smelled booze.

Officials say Ortiz's BAC was MORE than .15 ... which is a huge deal because in California, that triggers a "special allegation" which carries a tougher penalty if convicted ... more jail, higher fines, etc.

But Ortiz caught a huge break -- cutting a deal with prosecutors in which he plead guilty to a misdemeanor DUI and in exchange the "special allegation" was dismissed.

He was sentenced to 3 years probation and 8 days in county jail -- but he'll get credit for time served for 4 days and the remaining 4 days will be served as part of a work release program.

Tyrese will get some daddy time with his daughter, Shayla, after all, but the restraining order battle with his ex-wife is far from over ... TMZ has learned.

Sources connected with the case tell us both sides agreed, and the judge later ordered that Tyrese be allowed a monitored visit with his 10-year-old daughter on Sunday. We're told the existing restraining order was modified to allow for the 6-hour visit with a court-appointed monitor present.

However, Norma Gibson's effort to get a permanent restraining order against Tyrese will continue. In fact, the court has blocked off all next week for testimony.

Sunday will mark the first time in 2 months Tyrese has seen Shayla.

Tiger Woods was calm, cool and collected in court, even cracking a smile on his way out after pleading guilty at his DUI hearing in Florida.

The golf legend agreed to enter a DUI diversion program at a hearing that lasted about 10 minutes. He'll also be on probation for 12 months and he must perform 50 hours of community service. The judge was adamant -- the deal means no jail time but he could face up to 90 days in jail if he's found to violate parole.

Tiger was arrested for DUI on May 29 in Jupiter, FL after cops found him asleep at the wheel of his 2015 Mercedes. Once he awoke, Tiger was a discombobulated mess and claimed he was all jacked up from a cocktail of Rx drugs.

Tiger's car had damage and it was clear he was in no shape to be behind the wheel of a car. He was initially charged with 3 misdemeanor counts -- DUI, reckless driving and improper stopping.

If Tiger completes his DUI diversion program, DUI education classes and stays out of trouble for 12 months ... the reckless driving conviction will be wiped from his record.

As part of the diversion program, Tiger is prohibited from drinking alcohol or taking drugs during the probation period.

Tiger has since apologized for the arrest -- saying he didn't know his prescription meds would have such a strong reaction. He also went to rehab.

Tyrese insists the permanent restraining order his ex is trying to get against him is based on a story his daughter was coached into telling.

Tyrese filed docs saying the only evidence his ex-wife, Norma Gibson, has is hearsay from their 10-year-old daughter, Shayla. He firmly believes the little girl has been coached by someone while under Norma's care. Tyrese is not necessarily pointing the finger at Norma, but says someone coached Shayla.

He also says the only recent domestic violence incident was when Norma texted him saying she wanted to have his fingers cut off. She confessed, in a court declaration, to sending the message.

As we reported ... Norma's trying to keep Tyrese away from her and Shayla, claiming the singer went into a rage and pushed Shayla to the ground and beat her.

One of Usher's herpes accusers is reconsidering going forward with her $20 million lawsuit after suddenly ditching her attorney in the case ... TMZ has learned.

Laura Helm's attorney, Lisa West, filed a motion Thursday to withdraw from the case, saying she could no longer "effectively represent" her client. However, sources connected to Helm tell us what really happened, in plain terms, is Helm fired her lawyer.

And the bigger news -- we're told Helm is now uncertain about continuing the legal battle with Usher at all. His attorney has already filed a motion to dismiss the case and for now, Helm has until November 5 to respond.

Lisa Bloom -- who said she resigned from representing Harvey Weinstein after learning of his sexual misdeeds -- was deeply involved in investigating various female accusers to discredit them ... sources familiar with the situation tell TMZ.

We've learned Bloom actively investigated Rose McGowan back in July ... months before she left the case. We're told she had combed through private investigator's reports and discussed the findings with Harvey Weinstein and others.

We also learned Bloom investigated compromising information to use against another alleged victim, Ambra Battilana Gutierrez, specifically documents from Italy. We're told the endgame here was to kill a story. Gutierrez was mentioned in The New Yorker article.

We're told Bloom was also involved in lobbying the New York Times and NBC to back off their stories on Weinstein.

Our sources say Bloom had been extremely active in honing in on the alleged victims as early as April ... pouring over non-disclosure agreements and reviewing investigative reports prepared on at least one rape accuser.

Our sources say Bloom was also involved in efforts to secretly record at least one sexual assault victim.

We're told Bloom's involvement in the Weinstein sexual assault/harassment matters go back as far as January 2017.

When Bloom left the case she told the LA Times, "I was never aware that there were allegations of sexual assault. Should I have? Yes."
